Amaç: Miyelodisplastik sendrom (MDS) tedavisinde hipometile edici ilaçlar (azasitidin ve desitabin) küratif tedavi seçeneği olmamakla birlikte, sıklıkla kullanılmaktadır. Bu çalışmada, MDS tedavisinde yaygın olarak kullanılan hipometile edici ilaçların (azasitidin ve desitabin) karşılaştırılması amaçlanmıştır. Hastalar ve Yöntem: Retrospektif olarak yapılan bu çalışmada, Erciyes Üniversitesi Tıp Fakültesi Hematoloji Bilim Dalı'nda Mayıs 2009-Kasım 2016 tarihleri arasında MDS tanısı konulup hipometile edici ilaçlar ile tedavi edilen 60 hastada her iki ilacın etkinliğini, AML'ye transformasyon oranlarını, riskli durum değişkenlerinin sağkalıma etkisini ve toplam sağkalım oranlarını karşılaştırdık. Hastalık risk sınıflaması IPSS-R'ye göre ve tedaviye yanıt durumu da modifiye Uluslararası Çalışma Grubu (IWG) kriterlerine göre belirlendi. Bulgular: Hastaların %63.3'üne azasitidin, %36.7'sine desitabin tedavisi uygulandı. AML'ye transformasyon oranı, tüm çalışma grubunda %40 olup, azasitidin ve desitabin kollarında benzer bulundu (%39.5 ve %40.9, p= 0.563). Azasitidin ve desitabin tedavilerine yanıt oranları benzerdi (%42.1 ve %50, p= 0.599). Her iki ilaç kolunda tedaviye yanıt alınan hastalardaki ortalama kür sayısı 4 olup aralarında anlamlı farklılık görülmedi (p= 0.765). AML'ye transformasyonların %75'i ilk bir yıl içinde gözlenmiş olup; azasitidin kolunda AML'ye ilerleyenlerin %80'inde, desitabin kolunda AML'ye ilerleyenlerin %66.7'sinde ilk bir yılda AML tanısı konulduğu gözlendi (p= 0.635). Riskli durum değişkenlerinin analizinde orta ve yüksek riskli hastalık, AML'ye transformasyon varlığı ve blast artışı ile tanımlanan MDS RAEB-2 alt tipinde anlık ölüm riskinin istatistiksel olarak anlamlı düzeyde arttığı gözlendi. İleri yaşın (≥ 65) anlık ölüm riskini anlamlı düzeyde azalttığı görüldü (p= 0.018). Genel sağkalım oranları azasitidin ve desitabin kollarında sırasıyla, medyan 16 ve 12 ay saptanmış olup, aralarında istatistiksel olarak anlamlı farklılık gözlenmedi (p= 0.323). Sonuç: MDS tedavisinde özellikle orta ve yüksek risk grubunda, takip ve tedavi planı açısından anlamlı sonuçlar elde etmek için, hipometile edici ilaçlarla daha fazla hastanın dahil edileceği randomize kontrollü çalışmaların yapılması gerekmektedir.
