Objectivo: O presente estudo pretende avaliar os fatores de risco preexistentes associados ao agravamento de infecção pela COVID-19 em pacientes atendidos no Centro de Saúde de Metangula. Método: Estudo transversal e retrospectivo realizado a partir de dados secundários extraídos do boletim epidemiológico do Centro de Saúde de Metangula num período de 01 (um) ano. Foram obtidos os dados demográficos, condição clinica e resultado laboratorial da COVID-19. Os dados foram analisados e apresentados em forma de gráficos e tabela com números absolutos e relativos. Resultados: Os dados revelaram que ocorreram incidências de infecção por COVID-19 em todas as faixas etárias, com predominância na faixa etária de 30-39 anos e predominância de caso de COVID-19 nos homens em comparação com mulheres. As comorbidades como diabetes mellitus, doenças respiratórias crónicas e os pacientes infectados com vírus da imunodeficiência humana (HIV), foram fatores de alto risco para o agravamento de infecção por COVID-19 entre os pacientes atendidos no Centro de Saúde de Metangula. Conclusão: Este estudo pode facilitar os prestadores de cuidados de saúde na identificação de pacientes com fatores de risco e, consequentemente, a diminuição no número de casos e de óbitos por essa doença. Os achados serão úteis para os tomadores de decisão na implementação de protocolos de manejos de pacientes com complicação gerados por determinada comorbidade e co-infecção aquando da sua associação à infecção por coronavírus.
O aparecimento do SARS-CoV-2, levou os laboratórios de análises clínicas a se adequar na mesma aceleração da pandemia para responder com segurança à crescente demanda pela comprovação etiológica e produzir resultados de testes que são cruciais para o diagnóstico de pacientes. As boas práticas de biossegurança nunca foram mundialmente tão centralizadas como nos cenários atuais da pandemia. Este trabalho tem como objetivo demonstrar o papel da biossegurança em laboratórios de análises clínicas no cenário da pandemia do novo coronavírus (SARS-CoV-2), além disso também busca descrever as medidas para prevenção de transmissão pelo COVID-19 nos laboratórios de análises clínicas. Foi realizada um estudo de revisão, utilizando como referência a busca de artigos de revisão, originais publicados nas bases de dados: PUBMED, SciELO e Google Acadêmico e documentos com diretrizes técnicas de autoridades sanitárias publicados, e científicas, fazendo uso dos descritores: Biossegurança, Laboratório, Análises Clínicas, SARS-CoV-2 e COVID-19. Através desta revisão foi possível verificar que a biossegurança desempenha um papel crucial na segurança dos profissionais, segurança no transporte de amostras biológicas e segurança no processo de diagnóstico laboratorial no enfrentamento à pandemia de COVID-19, concomitantemente, as boas práticas de biossegurança são indispensáveis em todos os níveis do laboratório. Os laboratórios com poucos recursos, soluções improvisadas devem ser adaptadas para oferecer a proteção para equipa de laboratório e deve ser tomadas as medidas de segurança para garantir um diagnóstico de qualidade prestada aos pacientes.
As infecções respiratórias agudas estão entre as principais causas de morbimortalidade em crianças menores de 5 anos de idade em Moçambique. O risco de mortalidade infantil nesses pais é devido á exposição a fatores ambientais. Este estudo tem como objetivo estabelecer a proporção de infecção respiratória aguda e os fatores de risco relacionados às condições do ambiente domiciliar em crianças de 0 a 59 meses na Localidade de Beleluane, Província de Maputo, no sul de Moçambique. Para tal, entre Maio e Novembro de 2016, foi conduzido um estudo transversal em 330 agregados familiares com crianças menores de 5 anos de idade. Foram aplicados questionários padronizados aos chefes de agregados familiares daquela localidade, e coletaram-se informações sobre características ambientais domiciliares, condições de habitação e nível socioeconómico da família e, adicionalmente, foram recolhidos dados sobre as principais queixas durante a assistência médica de crianças menores de 5 anos no Centro de Saúde local. Para análise dos principais fatores de riscos foi realizada regressão logística. Foi observado que 45.9 a 68.2% das crianças procuram cuidados de saúde devido a infecção respiratória aguda, representando a primeira razão de busca dos cuidados de saúde. A nível dos agregados familiares a infecção respiratória aguda foi referida em 41.5%. Os principais fatores de risco identificados foram: quatro ou mais pessoas dividindo o quarto da criança (OR = 4.56; 95%CI: 2.49 – 8.35), animais no interior do domicílio (OR = 6.64; 95%CI: 3.11 – 14.16), uso de combustíveis sólidos (OR = 4.22; 95%CI: 2.02 – 8.80), tabagismo domiciliar (OR = 5.97; 95%CI: 3.21 – 11.09), e renda do agregado familiar mensal menor do que um salário mínimo ($ 58,00) (OR = 10.265; 95%CI: 5.168 – 20.391). Os resultados deste estudo mostram que as características ambientais domiciliares e a renda familiar estão entre os principais determinantes das infecções respiratórias agudas nesse grupo etário no sul de Moçambique.
O cruzamento de doenças de COVID-19, HIV e Tuberculose em países com uma alta carga de infecções por HIV e TB apresenta vários desafios à saúde pública do século XXI, até então nunca descrita, é potencialmente fatal. Objectivo: avaliar os aspectos diagnósticos e prognósticos de pacientes coinfectados com COVID-19, HIV e tuberculose. Metodologia: Trata-se de um estudo de revisão integrativa de literatura, de carácter descritivo e de abordagem qualitativa, nas bases de dados Google Scholar, PubMed, SciELO e LILACS, utilizando os descritores “COVID-19 e co-infecção; COVID-19, HIV e Tuberculose”. Resultados: Foram incluídos 6 artigos. A maioria dos pacientes com co-infecção tripla foi do sexo masculino, com média de idade de 43.5 anos. Os sintomas mais relatados foram febre, tosse, cefaleia, mialgia e astenia. As principais alterações laboratoriais incluíram CD4 com níveis diminuído, alta carga viral, linfopenia, trombocitopenia, Hemoglobina com níveis diminuído, níveis elevados de proteína C reativa, velocidade de hemossedimentação aumentada, elevação de D-dímero e hiperferritinemia. Achados imaginológicos como infiltrados apical esquerdo, opacidades irregulares peri-hilares, cavitações, empiema pleural, vidro fosco e opacidades nodulares foram frequentes. A letalidade foi de 2,6%. Conclusão: O presente estudo mostrou que os casos da COVID-19 associados ao HIV/TB são mais comuns em homens e observou-se uma tendência dos pacientes coinfectados a evoluírem com quadro moderado ou grave, incluindo necessidade de internamento. Por isso é fundamental que os profissionais de saúde estejam actualizados e capacitados no manejo de pacientes com co-infecção HIV/TB aquando da sua associação à infecção por COVID-19 com vista a melhoria de prognóstico
