Criança do sexo masculino, 12 anos, deu entrada em pronto atendimento com quadro de dor em genicular esquerdo com início há dois dias. Paciente possui histórico de episódios de crise de dispneia, em que automedicava com Salbutamol para alívio, no entanto, não possui diagnostico prévio de qualquer doença respiratória. Ao exame físico constatou-se obesidade infantil e artralgia de genicular esquerdo associado a edema local. Após medicação, houve melhora temporária, mas a dor migrou para genicular direito associada a edema e também evoluiu com dispneia, taquipneia e hipossaturação. O início dos sintomas respiratórios, dentro do atual cenário de pandemia, ainda em ambiente hospitalar, levantou a suspeita de infecção por COVID-19. Todas as medidas de isolamento imediato foram realizadas, suporte clínico e exames complementares para detecção do vírus. Foi realizada radiografia torácica, tomografia de tórax e ecocardiograma transtorácico com importantes achados cardiológicos. Neste cenário, foi elaborado um plano de cuidados levando em conta as necessidades dentro do contexto de saúde pública, prezando a visão holística do paciente considerando seus determinantes sociais, alcançando o alívio dos sintomas para alta hospitalar e contrarreferência para a atenção especializada com o intuito de promover vínculo longitudinal ao paciente e sua família.
Relato de caso de lactente de 1 ano e 1 mês, venezuelana, que imigrou para o Brasil em fevereiro de 2020 que, com histórico de pneumonia, desenvolveu sintomas respiratórios considerados suspeitos de infecção por COVID-19. Possui antecedentes de broncoespasmo, ausência de suplementação de ferro e imunização desatualizada. Os determinantes sociais influenciaram diretamente no enredo clínico presente, principalmente o idioma distinto, tanto da criança quanto dos seus genitores, além disso, o cenário atual de pandemia pelo SARS-COV-2, agregaram complexidade à perspectiva. Dessa forma, foi traçado um plano de cuidados individual e familiar congruente às necessidades dentro do contexto do Sistema Único de Saúde, capaz de alcançar resultados satisfatórios e duradouros no que tange ao alívio dos sintomas, fortalecimento e melhoria do bem-estar geral da paciente, bem como a contrarreferência para a atenção primária com o intuito de um cuidado continuado à família.
