La violencia basada en género, que expresa el imbalance de poder entre hombres y mujeres, es una transgresión a los derechos humanos de las mujeres y un problema de salud pública muy antiguo y que compromete todas las sociedades del mundo. Se admite hoy en día que más de un tercio de las mujeres ha padecido violencia (psicológica, física o sexual) en algún momento de sus vidas. La violencia sexual es la versión más extrema de violencia contra la mujer y concurre usualmente con los otros dos tipos de violencia. Se puede presentar en todas las etapas de la vida y ni siquiera el embarazo constituye un factor protector. Las personas más cercanas a las víctimas (pareja actual o anterior, sus familiares o personas conocidas), generalmente con poder sobre ellas, son los perpetradores más comunes. Se puede identificar algunos factores asociados, como el alcoholismo, la ignorancia, la permisividad social, la mala relación entre las parejas, el uso de drogas y otros. Las consecuencias pueden ser inmediatas o de corto, mediano y largo plazos y se dan sobre la salud física (suicidio, homicidio, muertes derivadas de sida o por complicaciones del embarazo; lesiones leves y graves, infecciones pélvicas y de transmisión sexual, embarazos ectópicos, embarazos no deseados, abortos espontáneos o inducidos, peso bajo al nacer) y mental (depresión, ansiedad, síndrome postraumático, síndrome posviolación, trastornos del sueño, obesidad, consumo de alcohol, drogas y tabaco y conductas sexuales de riesgo, entre otras) y tiene también impacto social (costos derivados de la atención de la salud y de la atención legal, implicancias sobre la productividad y el empleo). Por las graves repercusiones que tiene la violencia sobre las mujeres y la sociedad en general, debe ser enfrentada con información, educación, trabajo con grupos de varones y mejor preparación de los servicios de salud, apoyo legal y social.
Objetivo. Determinar la relación existente entre la hemoglobina materna de gestantes adolescentes y el peso de los recién nacidos en el Instituto Nacional Materno Perinatal (INMP). Material y métodos. Transversalmente se analizaron 914 gestantes seleccionadas mediante criterios de inclusión y exclusión de 2729 partos de adolescentes atendidas en el INMP correspondiente al año 2017. Los datos se recogieron del registro de partos y las historias clínicas, las variables de interés fueron la hemoglobina materna y el peso del recién nacido. Resultados. De los 914 partos 299 madres (32%) presentaron anemia y 615 (68%) no la presentaron, el grupo etario predominante fueron las adolescentes en la etapa tardía 73% (con anemia) y 71% (sin anemia). En el control prenatal predominó los insuficiente o sin control 74% (con anemia) y 73%(sin anemia). Las solteras fueron el grupo mayoritario, 91% (con anemia) y 93% (sin anemia). La procedencia de los distritos con quintiles IV y V fue el 90% (con anemia) y el 87% (sin anemia). No se encontró asociación entre el peso del recién nacido y la hemoglobina del tercer trimestre de la madre (r de Pearson = -0,0141; valor p = 0,6702). Conclusiones. No encontramos relación entre la hemoglobina materna baja y el peso al nacimiento de gestaciones a término de adolescentes, esto podría deberse a que en el control del peso fetal existirían múltiples factores que podrían ser más influyentes en este grupo de gestantes adolescentes.
Objetivos: Socializar y discutir las investigaciones recientes sobre el estado actual de los servicios de planificación familiar y su relación con las cifras de mortalidad materna a fin de consensuar una propuesta guía para su uso en el sector público. Metodología: Se convocó tres Foros Regionales con la participación de un grupo de expertos a nivel nacional y un grupo de expertos a nivel de cada una de las tres regiones involucradas en la intervención (Arequipa, Piura y Ucayali). Se consensuó el contenido temático y la metodología a seguir en el desarrollo del evento. Los contenidos se centraron en: una revisión de los Derechos Sexuales y Reproductivos, los resultados de la Encuesta ENDES 2010, las cifras de mortalidad materna recientemente procesadas por la Dirección General de Epidemiología del Ministerio de Salud, la evaluación de la mortalidad materna, la planificación familiar y de la Mesa de Concertación para la Lucha Contra la Pobreza a nivel regional. Cada uno de estos contenidos fue expuesto por los expertos y después se abrió un espacio para preguntas, respuestas, opiniones, comentarios y críticas, luego de las cuales se sucedieron aportes y propuestas. Resultados: Hubo una amplia participación de los representantes de muchas instituciones luego de lo cual se pudo obtener que existen evidentes avances en lo referente a salud sexual y reproductiva, particularmente en la atención prenatal, atención institucional del parto y otras intervenciones obstétricas que han permitido reducir la Razón de Muerte Materna en los últimos 10 años; sin embargo existen brechas a superar en la atención obstétrica entre los grupos vulnerables y en las actividades de planificación familiar, dado que la fecundidad no ha bajado según el deseo de las mujeres, las adolescentes se siguen embarazando en cifras altas, hay un número importante de embarazos no deseados y como consecuencia las cifras de aborto inseguro son alarmantes. Conclusión: Si se produce un fortalecimiento de las actividades de planificación familiar podemos cumplir con las metas del milenio en relación al acceso a los servicios de salud sexual y reproductiva, sin discriminación y reducir aún más la razón de mortalidad materna.
