A patient is described who initially presented with subarachnoid hemorrhage and later with acute hypopituitarism. She had an aneurysm situated entirely within the pituitary fossa. The usual clinical features of parasellar aneurysms are described, and the endocrine assessment of the case is discussed.

We believe that early treatment with cyclophosphamide in this case, based on the finding of serum ANCA, prevented further deterioration in renal function and possible pulmonary involvement.Serum testing for ANCA is a useful noninvasive investigation in patients who present with unusual neuropathies, particularly if there is evidence of sinusitis.
