ResumenEl mecanismo de Frank-Starling es un concepto básico de la fisiología cardíaca y su aprendizaje. Constituye la base fundamental para el entendimiento del funcionamiento del corazón y patologías prevalentes y con alta morbimortalidad como lo es la insuficiencia cardíaca. Por lo cual, su relación con un proceso más interactivo como la erección del pene, podría asegurar un aprendizaje perdurable y práctico. La relación se fundamentó en el reemplazo del eje X de la curva por precarga en el caso del corazón y estimulación en el caso del pene, para así lograr una relación incluso con la disfunción eréctil, que sería el equivalente a la insuficiencia cardíaca. De esa manera, se encontró que a mayor estimulación hay mayor erección y a mayor precarga hay mayor eyección ventricular, teniendo ambas curvas una meseta. Asímismo, farmacológicamente, se encontró relación con el uso de estimulación β-adrenérgica y de inhibidores selectivos de la fosfodiesterasa tipo 5 como el sildenafil, los cuales desplazan la curva hacia arriba y a la izquierda.
Introducción: La prediabetes es un estado intermedio de hiperglicemia, con una alta prevalencia en la población colombiana. Su relación con enfermedad cardiovascular y complicaciones macro y microvasculares se ha establecido, incrementando la evidencia científica en la literatura reciente. Objetivo: Describir las enfermedades cardiovasculares (complicaciones macrovasculares) relacionadas con prediabetes, su diagnóstico, fisiopatología, el enfoque de manejo y el tratamiento avalado para prevenir estas complicaciones. Materiales y métodos: Se realizó una revisión narrativa de la literatura, mediante búsqueda de artículos científicos en Medline entre 2015 y 2021. Resultados: Se seleccionaron 83 artículos. Se describe la relación de prediabetes con enfermedad cardiovascular, mortalidad, diagnóstico y tratamiento conforme a la literatura actualizada. Conclusiones: La importancia de la prediabetes radica en su asociación con complicaciones macrovasculares y su mortalidad, lo que pone en evidencia la importancia de estudiarla, seguirla y tratarla para evitar la progresión a diabetes mellitus, el desarrollo de desenlaces que finalizan en aumento de la morbimortalidad, alteración de la calidad de vida e incremento de los costos en salud.
Objetivo: Evaluar la efectividad de los análogos de somatostatina, lanreótide, octreótide y pasireótide en el control de GH, IGF-1 y tamaño tumoral en una cohorte pacientes con acromegalia. Materiales y métodos: estudio observacional, analítico, tipo cohorte retrospectiva, unicéntrico, de pacientes con acromegalia en un centro de alta complejidad en la ciudad de Cali – Colombia. Para evaluar la efectividad, se usó análisis bivariado, se hicieron tablas de contingencia, chi-cuadrado, ANOVA de una vía en un modelo lineal general de medidas repetidas de una y dos vías, con un ? ? 0.05. Resultados: Se incluyeron 29 pacientes, 52% eran hombres. La edad promedio fue de 54.8 años (rango: 28-79 años). Los niveles promedio de GH e IGF-1 fueron de 13.7ng/dl y 776 ng/ml, respectivamente. La media de tamaño tumoral fue de 17.2 mm. Se encontró disminución de los niveles de GH e IGF-1 en todos los pacientes, independientemente del medicamento recibido. Se observó disminución del tamaño tumoral en el seguimiento a 12 meses del 41.6%. En cuanto a los criterios de control adecuado, el 27.6% de pacientes lo alcanzó tras un año de tratamiento, de los cuales, 62.5% eran mujeres y 37.5% se encontraban recibiendo pasireótide, el resto recibió lanreótide. Conclusiones: Los análogos de somatostatina fueron efectivos en la disminución de los niveles de GH, IGF -1 y tamaño tumoral, tras un seguimiento de 12 meses.
Introducción: la hipertensión arterial sistémica (HTA) constituye el principal factor de riesgo para morbilidad y mortalidad cardiovascular a nivel global, afecta a todas las edades, sin distinción de género y etnicidad. Su tratamiento continúa constituyendo un reto, dada la persistencia del pobre control, especialmente en países como Colombia. Objetivo: mostrar la evidencia disponible respecto al tratamiento actualizado de la HTA y la elección certera de los agentes antihipertensivos acorde con la individualidad de cada paciente. Asimismo, consolidar y comparar el efecto hipotensor de cada agente antihipertensivo más usado. Metodología: se realizó una búsqueda avanzada con los términos DeCS y MeSH: hipertensión, agentes antihipertensivos, hipertensión esencial y terapia combinada, en los motores de búsqueda PubMed, Clinical Key, Lilacs, Scielo. Un total de 109 artículos se seleccionaron para elaborar en la presente revisión de la literatura. Conclusiones: la individualización del manejo de la HTA lleva al reconocimiento de los distintos fenotipos, la presencia de complicaciones, el examen físico, el género y la raza como puntos fundamentales para elegir el agente antihipertensivo más adecuado que permita alcanzar las metas de control y propenda por la reducción y prevención de las complicaciones derivadas de un control no óptimo.
