Perianal tuberculosis. Report of two casesPerianal tuberculosis is an uncommon variant of extrapulmonary tuberculosis and mimics other common perianal conditions. We report two patients with perianal tuberculosis. A 82-year-old a tuberculosis (TBC) se puede presentar como enfermedad pulmonar, extrapulmonar o ambas. Aunque la morbilidad y mortalidad por TBC van en disminución, se ha descrito un aumento de los casos de TBC, principalmente debido a la incidencia creciente del síndrome de inmunodeficiencia adquirido (SIDA) 1 . Este aumento se ha dado tanto en la variante pulmonar como en la extrapulmonar de la enfermedad 2 . La TBC perianal es una forma de presentación extremadamente rara de la enfermedad extrapulmonar 3 , que posee características semejantes a otras patologías frecuentes de la zona 4 . Existe un amplio abanico de diagnósticos diferenciales, que muchas veces lleva a un diagnóstico tardío. El diagnóstico definitivo habitualmente se realiza por biopsia, luego de un prolongado cuadro de ulceración, o de pobre respuesta a terapia antibiótica empírica 5 .La TBC perianal se puede asociar a la tuberculosis intestinal, ya sea como extensión de la lesión original, o como diseminación por vía linfática 6 . La enfermedad perianal puede manifestarse de forma ulcerativa, verrugosa, lupoide o miliar. El tipo más común es la presentación ulcerativa. El tipo verrugoso frecuentemente se extiende desde el margen anal hacia el conducto anal, con un patrón de crecimiento similar al de los condilomas. Sin embargo, se puede presentar como un nódulo hemorroidal, absceso perianal o fístula ano-rectal 3,4 .Clínicamente, la forma ulcerativa se presenta como úlceras poco profundas, dolorosas, de bordes bien definidos, biselados con tinte azul, cubiertas de material pseudomembranoso y que presentan descarga mucopurulenta 7 . Las úlceras pueden ser la manifestación inicial de la enfermedad tuberculosa. Si no se tratan, las ulceras por TBC perianal no se mejoran espontáneamente 8 y pueden llevar a la muerte por diseminación miliar de los bacilos 9 .Dada la amplia gama de diagnósticos diferenciales de ulceración perianal, de lo infrecuente que es esta patología y de la importancia clínica de su diagnóstico precoz es que presentamos dos casos de ulceración perianal de causa tuberculosa, en pacientes sin diagnóstico previo de TBC. Casos ClínicosCaso 1: Paciente de sexo masculino, 82 años de edad, con antecedentes mórbidos de hipertensión arterial e insuficiencia cardíaca congestiva.
