Introducción: La Discitis o Espondilodiscitis es una entidad poco frecuente en pediatría, la cual, en términos de fisiopatología y manifestaciones clínicas, es muy controvertida. Se presenta un reporte de caso, de una paciente de 3 años, quien consulta al servicio de urgencias por presentar marcha antálgica progresiva. Se hizo un enfoque diagnóstico inicial mediante una resonancia magnética nuclear (RMN) que reporto una Espondilodiscitis. Se dio tratamiento integral con antibioticoterapia y antiinflamatorio, logrando una evolución satisfactoria. Presentación del caso: Paciente de 3 años de edad, de sexo femenino, quien consulto por cuadro clínico de 20 días de evolución, consistente en marcha antálgica progresiva asociada a irritabilidad, signos y síntomas ventilo-respiratorios, sin antecedentes de importancia. Ante hallazgos clínicos e imageneológicos se realizó el diagnóstico de Espondilodiscitis. Se inició antibioticoterapia, manejo antiinflamatorio y, adicionalmente, se ordenó reposo y rehabilitación física. La paciente evolucionó de manera satisfactoria por lo cual se dio egreso hospitalario. Conclusiones: Aun cuando la Espondilodiscitis es una patología poco frecuente y auto-limitada, es importante tenerla en cuenta entre los diagnósticos diferenciales debido a sus potenciales complicaciones. La resonancia magnética es el estándar de oro para el diagnóstico temprano. El tratamiento integral incluye reposo en cama inicial, antiinflamatorios, en algunos casos, antibioticoterapia y rehabilitación física.
