El objetivo de este estudio fue conocer las prácticas y la percepción sobre los riesgos de los plaguicidas domésticos (PDs) en madres y padres de niños en etapas tempranas del desarrollo. Sujetos y Método: Investigación de diseño cualitativo mediante entrevistas a madres y padres en 18 domicilios con niños de 0 a 3 años afiliados al Seguro de Salud del Hospital Italiano de Buenos Aires en el año 2015. Se identificaron categorías analíticas, se conceptualizaron categorías analíticas interpretativas de mayor nivel de abstracción y finalmente se construyeron diagramas para representar y organizar los hallazgos. Resultados: Las entrevistas mostraron que las madres y padres: 1) Utilizaban habitualmente PDs, 2) Mostraron en general baja preocupación por los peligros para la salud de los PDs y productos químicos hogareños excepto un grupo pequeño más sensibilizado, 3) Les preocupaba más las picaduras de insectos en sus hijos que la exposición a PDs, 4) Confiaban en los servicios de fumigación y/o en el marco normativo que los regula, y 5) Manifestaron interés en que el pediatra les brindara información acerca de los productos químicos que se utilizan diariamente en el hogar. Otras categorías analíticas mostraron un ajuste constante entre toxicidad, patogenicidad, higiene y bienestar. Conclusiones: Dado que los niveles de exposición hogareña considerados inocuos para adultos pueden ser toxicológicamente relevantes para individuos en desarrollo, los entes de gobierno y salud deberían proveer a los padres de herramientas intelectuales para decodificar los mensajes del mercadeo de PDs, así como para conceptualizar la relación entre contacto con PDs y trastornos de salud infantil.
Conceptos clave:El Bisfenol A (BPA) es un químico utilizado a nivel mundial en la fabricación de envases de alimentos y bebidas. Se comporta como disruptor endócrino, actuando a concentraciones séricas bajas pudiendo alterar la función hormonal, modificando la expresión del ADN pudiendo interferir en los momentos críticos del desarrollo embrionario. En todos los estudios publicados, se obtuvo como resultado una exposición generalizada al BPA. En nuestro país no se han desarrollado aún estudios que evalúen la presencia de BPA en poblaciones humanas por lo que la misma se desconoce. En Argentina, se reguló el uso de BPA en la fabricación y comercialización de mamaderas. Debido a la exposición generalizada al BPA, y que no se cuenta con suficiente evidencia de efectos a largo plazo de la exposición transplacentaria, este trabajo puede aportar conocimiento sobre la exposición de nuestra población y aportar datos basales para futuros estudios de riesgo. Resumen:Introducción: El Bisfenol A (BPA) es un disruptor endócrino (DE) posiblemente vinculado a eventos adversos reproductivos y del desarrollo neurológico que se utiliza como aditivo en envases plásticos de alimentos y bebidas. En la población argentina no existen datos sobre niveles de BPA en orina. El objetivo del estudio fue medir la presencia de BPA en orina de embarazadas. Métodos: Estudio transversal. Detección de BPA en orina de embarazadas atendidas en el Hospital Italiano de Buenos Aires en el año 2013. La técnica utilizada fue cromatografía gaseosa asociada a espectrometría de masa con un límite de cuantificación de 2 ng/ml y de detección de 0,8 ng/ml. Se describe la proporción de mujeres con exposición al BPA con su intervalo de confianza del 95% (IC95%). Resultados: se analizaron 149 muestras de orina de embarazadas de edad promedio 32 años (Desvío estándar 5,3), el 81% (IC95% 73,9-86,9) trabaja y el 83,7% (IC95% 76,7-89,1) posee nivel universitario y/o posgrado. El 66,4% (IC95% 58,2-74,0) de las muestras urinarias presentaron BPA cuantificable, siendo la mediana de BPA de 4,3 ng / mg creatinina (Rango Intercuartilo 3,2-6,7). Conclusiones: Este estudio muestra que la exposición a BPA durante el proceso de formación embrionaria en un sector de la población de Argentina, es generalizada. Objetivo: analizar motivos de consulta, experiencias y formas de atención de las afecciones dermatológicas de la población de un asentamiento de la Provincia de Buenos Aires, ubicado sobre un relleno sanitario con altos índices de contaminación ambiental. Materiales y métodos: investigación cualitativa, mediante entrevistas en profundidad a vecinos/as y elaboración de registros etnográficos de las recorridas. La investigación se realizó en el contexto de un dispositivo de atención dermatológica, planificado en conjunto con referentes barriales, bajo el marco teórico de la Epidemiología Sociocultural. Resultados: se identificaron múltiples barreras de accesibilidad a la atención dermatológica en el sistema público de salud, y prácticas de autoatención como el uso de cremas, medidas de higiene y uso de elementos de protección personal para trabajos que significaban un riesgo para la salud. Las consultas al dispositivo más frecuentes fueron “granos”, “manchas”, “bultitos” en zonas visibles del cuerpo, que generaban molestia, dolor y/o afectaban estética y emocionalmente a lo/as entrevistado/as. Estas consultas se relacionaban principalmente con piel extremadamente seca y patologías por alta exposición solar, coincidiendo con la escasez de sombra en el barrio. Para prevenir patologías relacionadas a la exposición solar, se impulsó una iniciativa para plantar árboles. Conclusión: a partir de la inmersión reflexiva del equipo de atención, en las experiencias de atención de lo/as habitantes del barrio, se desplegaron estrategias según las necesidades percibidas por la población.
Background Medications delivered to primary care centers (PCC) all over the country through the Remediar program (RP) covers 80% of the most prevalent diseases. Among them, the prevalence of hypertension is 77% of 65 years or older . Objective To describe the pharmacological treatment and its appropriateness, and to understand needs and barriers among hypertensive beneficiaries of RP at Buenos Aires suburbs. Method Qualitative-quantitative triangulation: cross-sectional study with revision of medical records (MR) and in-depth interviews between December/2008 and February/2009 to hypertensive patients of 65 years or older and CAPS’s staff. Prevalence of correct treatment (CT); appropriate Indication (AI); Optimal control (OC) were analyzed Results The following PCCs were selected: Moreno, Tigre, Lomas de Zamora, Lanus and San Isidro. Prevalence of: CT: (10.2%; 95%CI 6,7-15,1); AI: (84.8%; 95% CI 80-89 ), and OC: 11,6% (95%CI 7-16) and OC among patients with good registry: 35.7% (95%CI 24,9-48,1). Eighty two % of patients received a prescription for enalapril. Deficient registry was detected in MR. Interviewees expressed difficulties in accessing treatment, lack of medication and use of alternative circuits to get them. Both, staff and patients were exposed to barriers generated by CAPSs and RP. Conclusions A low degree of blood pressure control was observed, even with adequate prescription. Multifactorial problems explain several barriers to access and maintain hypertension treatment.
