La enfermedad avanzada por VIH sigue siendo un reto importante. A pesar de grandes avances en la última década de la terapia antirretroviral (TAR), muchos pacientes mueren a causa de enfermedades oportunistas relacionadas con el VIH. La criptococosis es una infección micótica oportunista causada por especies de Cryptococcus. Desde el punto de vista clínico y epidemiológico, la criptococosis se asocia con las condiciones de inmunosupresión celulares y es causada principalmente por Cryptococcus neoformans (serotipo D) y Cryptococcus grubii (serotipo A). En los últimos años, el aumento de número de casos de síndrome de inmunodeficiencia adquirida (SIDA), ha conllevado a un aumento de la incidencia de criptococosis. La infección normalmente se produce por inhalación del agente causal. Presentamos un caso de infección diseminada con Cryptococcus neoformans, con una exuberante presentación cutánea tras el inicio de la terapia antirretroviral (TAR), con una buena evolución clínica.
