IntroductionThrombolysis has radically changed the prognosis of acute ischemic stroke. Tenecteplase is a modified form of rt-PA with greater specificity for fibrin and a longer half-life. We report the experience of a Moroccan tertiary hospital in thrombolysis using Tenecteplase.MethodsWe conducted an open prospective study of all patients who were treated with Tenecteplase for an acute ischemic stroke admitted to our emergency department. Tenecteplase was administered intravenously at a dose of 0.4 mg/kg single bolus. The primary outcome measure was the proportion of patients achieving significant early neurological recovery defined as an improvement of 4 or more points on the NIHSS score at 24h.Results13 patients had been treated by intravenous thrombolysis. 31% were women. Mean age was 63 years old. The mean NIHSS score at admission was 14.3 and 24h after was at 9.1. The right middle cerebral artery was involved in 69% of cases. The carotid atherosclerosis was predominant 63.3% and the cardio embolic etiology 27%. The mean time to the first medical contact after the onset of symptoms was 3h 30 min. One patient presented a capsulo-lenticular hematoma of 5 mm3 in the same side of the ischemic stroke.ConclusionTenecteplase is a more interesting thrombolytic than alteplase, it seems to be more suitable for thrombolysis in our center.
IntroductionLes accidents domestiques de l'enfant représentent un vrai problème de santé publique dans les pays industrialisés. Au Maroc, la priorité en santé publique est toujours donnée aux pathologies infectieuses, et bien qu'elle soit très peu décrite, la pathologie accidentelle de l'enfant devient de plus en plus fréquente dans notre pays avec une mortalité importante. L'objectif est de mettre le point sur la prévalence, la gravité, les aspects étiologiques, les facteurs de risque et les circonstances de survenue de ces accidents, ainsi que les moyens de prévention active et passive.MéthodesEnquête rétrospective descriptive sur une période de douze mois portant sur tous les enfants pris en charge pour accident domestique grave au service de réanimation pédiatrique polyvalente de l'hôpital universitaire d'enfants de Rabat.RésultatsParmi 698 admissions, 108 cas d'accidents domestiques graves ont étaient colligés (soit 15,5%), L’âge moyen des enfants était de 04ans tout accident confondu, avec un sex-ratio de 1,08 en faveur des garçons. L’évolution générale était marquée par le décès de 16 enfants (soit 14,8%) parmi 164 décès toute pathologie confondue au cours de la même période d’étude (soit 9,75% des décès) avec une durée moyenne d'hospitalisation de 04jours. les brûlures constituaient le premier accident dans notre série par 37cas, et elles étaient la première cause de mortalité par huit cas; par ailleurs, la population la plus à risque de brûlure était les nourrissons (67,6%). L'inhalation intrabronchique d’épingle à foulard (accident particulier dans notre contexte islamique) à été retrouvée chez six cas.ConclusionLes accidents domestiques de l'enfant constituent rarement une préoccupation de premier plan dans la population alors qu'ils sont parfois très graves et source d'une mortalité importante. Le meilleur traitement reste la prévention active et passive.
IntroductionIntranasal fentanyl provides rapid and powerful analgesia which is particularly interesting in patients without intravenous access. We propose to use it for analgesia in adults presenting renal colics.MethodsA prospective study was conducted from the 2nd January to February 2013 in our emergency department. Patients aged up to 18 years old who presented with renal colic were included in this audit. Patients were excluded if they had loss of consciousness, cognitive impairment, acute or chronic nasal problems. A formal written consent was obtained from patients. The research team was alerted by medical and nursing staff. A member of the research team would check with medical or nursing staff whether administration of Intra nasal (IN) fentanyl was required. It was administered at a pre-calculated dose of 1.5 mg/kg and 50 mg/ml concentration was used. Data was prospectively collected by one of the researchers at various intervals during the patient's presentation and recorded on a pre-formatted data sheet. Pain scores were collected at 5, 15, 30, 45 and 60 minutes following IN fentanyl using a visual analogue scale pain. Observations routinely collected for patients receiving IV opiates and any adverse events were also recorded.Results23 eligible patientswere enrolled; median age was 51,3years. 47,8% were women and the mean weight was 73 kg. Median dose of IN fentanyl was 106 μg. Two patients have required morphinic analgesia despite having received adapted dose of IN fentanyl. The initial pain scores before IN fentanyl were high with a median of 82,2 mm (59-100). Five minutes after IN fentanyl administration the median pain score dropped to 48mm(36-63) and achieved the lowest score of 8mm(0-22) at 30 min. Pain scores were significantly lower at 5 min (P < 0.001) and at all subsequent time points (P < 0.001). No side effects were recorded.ConclusionIntranasal fentanyl seems to be efficient for analgesia in adult patients with renal colic.
Le syndrome de Mirizzi est une complication rare de la lithiase vésiculaire chronique, avec une incidence de 0,7% à 1,4% chez les malades cholécystéctomisés. Il est caractérisé par un ictère cholestatique en rapport avec une compression de la voie biliaire principale par un calcul enclavé dans le collet vésiculaire ou le canal cystique. La maladie peut évoluer vers l’érosion de la paroi du canal hépatique commun et par conséquent, provoquer la formation d’une fistule cholécysto-bilaire. Nous présentons ici un syndrome de Mirizzi type I en soulignant l’importance du diagnostic préopératoire, celui-ci étant rendu plus aisé par la cholangiographie rétrograde endoscopique ou par la cholangio-IRM, qui permet d’éviter des lésions iatrogènes de la voie biliaire principale. Le présent article passe en revue la littérature disponible sur les divers aspects de ce syndrome, y compris sa pathogenèse, son diagnostic et sa prise en charge.
Cette étude à pour objectifs de décrire les pratiques des prescriptions, évaluer leur pertinence et leur conformité aux règles d’utilisations et étudier les facteurs susceptibles de les influencer. Il s’agit d’une étude transversale d’évaluation des prescriptions antibiotiques portant sur 105 patients réalisée au service des urgences médico-chirurgicales de l’H.M.I.Med V de Rabat sur une période d’un mois. Le recueil des données était fait à l’aide d’un questionnaire rapportant les données démographiques et anamnestiques, les antécédents, la notion d’allergie, les données spécifiques de l’examen clinique, les données para cliniques, la prescription détaillée de l’antibiotique. Les données récoltées ont été ensuite évaluées par un médecin référent, chargé d’indiquer les éventuelles erreurs de traitement. Parmi les infections ayant motivé la prescription des antibiotiques, les affections des systèmes respiratoires et urinaires étaient au premier rang, les familles d’antibiotiques les plus couramment employées sont les pénicillines, les quinolones et les céphalosporines. 74 prescriptions soit (70,5%) étaient à la fois pertinentes et conformes contre 9 prescriptions soit (8,6%) justifiées mais non pertinentes et 6 prescriptions soit (5,7%) étaient jugées injustifiées par le médecin référent par absence d’infection. Les évaluations des pratiques médicales sont rarement menées dans les établissements de santé; c’est dans ce cadre que nous avons voulu nous inscrire en produisant cette étude afin d’améliorer la pertinence de nos prescriptions antibiotiques et d’optimiser leur conformité aux différentes recommandations.
