Les tumeurs primitives du duodénum sont rares. Elles représentent 0,04 % de toutes les tumeurs. L'adéno-carcinome est le type histologique le plus fréquent. Objectif : Les auteurs rapportent deux cas d'adénocarcinome primitif du duodénum, soulignant l'apport de l'imagerie dans le diagnostic positif et le bilan d'extension de cette pathologie. Observations : Deux patients de sexe masculin, âgés respectivement de 37 et 60 ans, tabagiques chroniques, consultent pour des épigastralgies atypiques et hémorragie digestive haute. La fibroscopie avec biopsie a mis en évidence un adénocarcinome. L'échographie a montré un épaississement pariétal circonférentiel dans un cas, et le scanner abdominal a révélé une masse tissulaire dans les deux cas avec signes d'extension locorégionale dans un cas. Le traitement était palliatif dans un cas. Conclusion : Le diagnostic positif de l'adénocarcinome duodénal repose sur la fibroscopie. L'imagerie permet de détec-ter les petites lésions et de contribuer au diagnostic devant un tableau clinique atypique ou une tumeur infranchissable et d'assurer le bilan d'extension surtout avec le développement du duodénoscanner à l'eau et l'entéro-IRM.Mots clés Duodénum · Adénocarcinome · Échographie · TDM · Échoendoscopie · IRM Abstract The primitive tumours of the duodenum are rare. They account for 0.04% of all the tumours. The adenocarcinoma is the histological type most frequent.Objective : The authors report two cases of primitive adenocarcinoma of the duodenum underlining the contribution of the imagery in the positive diagnosis and the extension of this pathology. Observations : Two patients of male sex respectively aged of 37 and 60 years, consulting for atypical pain and high digestive haemorrhagic. The fibroscopy with biopsy highlighted an adenocarcinoma. Ultrasound showed a circumferential parietal thickening in one case, the abdominal computed tomography revealed a tissue mass in both cases with signs of the neighbour extension in one case. The treatment was palliative in one case. Conclusion : The positive diagnosis of the adenocarcinoma of duodenum rests on the fibroscopy. The imagery makes it possible to detect the small lesions and to contribute to the diagnosis in front of an atypical clinical signs or an insuperable tumour and to especially ensure the assessment of precise extension of this tumour with the development of the computed tomography and MRI.
