Background Tranexamic acid reduces surgical bleeding and reduces death due to bleeding in patients with trauma. Meta-analyses of small trials show that tranexamic acid might decrease deaths from gastrointestinal bleeding. We aimed to assess the effects of tranexamic acid in patients with gastrointestinal bleeding. Methods We did an international, multicentre, randomised, placebo-controlled trial in 164 hospitals in 15 countries. Patients were enrolled if the responsible clinician was uncertain whether to use tranexamic acid, were aged above the minimum age considered an adult in their country (either aged 16 years and older or aged 18 years and older), and had significant (defined as at risk of bleeding to death) upper or lower gastrointestinal bleeding. Patients were randomly assigned by selection of a numbered treatment pack from a box containing eight packs that were identical apart from the pack number. Patients received either a loading dose of 1 g tranexamic acid, which was added to 100 mL infusion bag of 0•9% sodium chloride and infused by slow intravenous injection over 10 min, followed by a maintenance dose of 3 g tranexamic acid added to 1 L of any isotonic intravenous solution and infused at 125 mg/h for 24 h, or placebo (sodium chloride 0•9%). Patients, caregivers, and those assessing outcomes were masked to allocation. The primary outcome was death due to bleeding within 5 days of randomisation; analysis excluded patients who received neither dose of the allocated treatment and those for whom outcome data on death were unavailable. This trial was registered with Current Controlled Trials, ISRCTN11225767, and ClinicalTrials.gov, NCT01658124.

Background. Next-generation sequencing methods have been developed and proposed to investigate any query in genomics or clinical activity involving DNA. Technical advancement in these sequencing methods has enhanced sequencing volume to several billion nucleotides within a very short time and low cost. During the last few years, the usage of the latest DNA sequencing platforms in a large number of research projects helped to improve the sequencing methods and technologies, thus enabling a wide variety of research/review publications and applications of sequencing technologies. Objective. The proposed study is aimed at highlighting the most fast and accurate NGS instruments developed by various companies by comparing output per hour, quality of the reads, maximum read length, reads per run, and their applications in various domains. This will help research institutions and biological/clinical laboratories to choose the sequencing instrument best suited to their environment. The end users will have a general overview about the history of the sequencing technologies, latest developments, and improvements made in the sequencing technologies till now. Results. The proposed study, based on previous studies and manufacturers’ descriptions, highlighted that in terms of output per hour, Nanopore PromethION outperformed all sequencers. BGI was on the second position, and Illumina was on the third position. Conclusion. The proposed study investigated various sequencing instruments and highlighted that, overall, Nanopore PromethION is the fastest sequencing approach. BGI and Nanopore can beat Illumina, which is currently the most popular sequencing company. With respect to quality, Ion Torrent NGS instruments are on the top of the list, Illumina is on the second position, and BGI DNB is on the third position. Secondly, memory- and time-saving algorithms and databases need to be developed to analyze data produced by the 3rd- and 4th-generation sequencing methods. This study will help people to adopt the best suited sequencing platform for their research work, clinical or diagnostic activities.

A comparison of 10 most popular Multiple Sequence Alignment (MSA) tools, namely, MUSCLE, MAFFT(L-INS-i), MAFFT (FFT-NS-2), T-Coffee, ProbCons, SATe, Clustal Omega, Kalign, Multalin, and Dialign-TX is presented. We also focused on the significance of some implementations embedded in algorithm of each tool. Based on 10 simulated trees of different number of taxa generated by R, 400 known alignments and sequence files were constructed using indel-Seq-Gen. A total of 4000 test alignments were generated to study the effect of sequence length, indel size, deletion rate, and insertion rate. Results showed that alignment quality was highly dependent on the number of deletions and insertions in the sequences and that the sequence length and indel size had a weaker effect. Overall, ProbCons was consistently on the top of list of the evaluated MSA tools. SATe, being little less accurate, was 529.10% faster than ProbCons and 236.72% faster than MAFFT(L-INS-i). Among other tools, Kalign and MUSCLE achieved the highest sum of pairs. We also considered BALiBASE benchmark datasets and the results relative to BAliBASE- and indel-Seq-Gen-generated alignments were consistent in the most cases.

Purpose: Rural advisory services ensure agricultural information is disseminated to rural populations, yet they are less accessible to women. This research provides insight on gender differences in information access by investigating frequency of use and preference of agricultural information sources by gender in a rural setting, differentiated according to literacy and age. Design/Methodology/approach: This study interviewed 401 male/female individuals in farm households in Jhang and Bahawalpur district of Punjab, Pakistan in 2016. Findings: Men and women farmers' use and preferences in accessing information sources are extremely different. Women hardly use sources for agricultural information, and value interpersonal communication from informal sources. In contrast, men use and value official agencies more. Radio, surprisingly, was very rarely used, contradicting previous findings of research elsewhere. Age and literacy affect differences between women more than it does between men, particularly for convenient locations to access information. Practical implications The study identified and refined major gender differences regarding use and preference for agricultural information in relation to age and literacy, and helps to articulate options to improve gender equality of access to agricultural information in Pakistan. Theoretical implications: The focus and outcomes regarding gender intersecting with age and literacy in agricultural information access imply the need for more refined socioeconomic models, discerning and interrelating gender and other social dimensions beyond the standard of male-headed households. Originality/value: This paper adds to the growing body of evidence on information access according to gender, highlighting the need to investigate deeper socio-cultural issues around age and literacy.

Malignant ascites is a manifestation of advanced malignant disease that is associated with significant morbidity. Mainstays of treatment include diuretics and recurrent large volume paracentesis. Although lymphatic obstruction has been considered the major pathophysiologic mechanism behind its formation, recent evidence suggests that immune modulators, vascular permeability factors, and metalloproteinases are contributing significantly to the process. These new observations offer the opportunity for development of new, more targeted therapies for the treatment of malignant ascites. This article uses a clinical case to highlight the problem, then reviews these new concepts in the pathophysiology of malignant ascites formation. The diagnosis and management of this challenging medical problem are subsequently discussed, with emphasis on how these new pathophysiologic insights are being applied to the development of novel therapies that may soon change how we manage this troubling clinical condition.

In recent years, enterprise service bus (ESB) has become a favorable adoption as a technology category in the IT industry as it provides secure and guaranteed delivery of services. The elasticity of Enterprise Service Bus (ESB) enables numerous applications to exchange information makes it a significant middleware layer responsible for transferring information in a Service-Oriented Architecture (SOA). ESB is presently the utmost promising tactic for the integration of business applications in distributed and diverse environments. It also offers essential infrastructure support for transforming messages or data, intelligent routing, and protocol transformation. The idea of ESBs emerged from the requirements to move out from traditional integration patterns, that becomes difficult to manage with the passage of time. Our study aim is to understand and provide ongoing research topics, challenges and future directions concerning ESB applications. A systematic mapping study (SMS) is therefore implemented to categorize the selected papers into the following classification: contribution type, ESB applications, research type and their approaches. We have extracted a total of twenty-two papers for this systematic study and they are classified according to defined criteria. The findings of this SMS are discussed and researchers were provided with suggestions on possible directions for future research.

Purpose The monetary and psycho-social ramifications of the COVID-19 pandemic have been extensive and unimaginable around the globe. The purpose of this study was to have a psycho-social analysis of people confronting this situation and its impact in the situation of a health crisis on them. Design/methodology/approach In the current study, participants were divided according to their different backgrounds and professions based on the types of measures taken by the Pakistani government to limit the spread of the virus and people who are not disturbed by any of the measurement categories but are troubled by the coronavirus. The interviews based on five open-ended questions were conducted with the eight participants, comprising questions that helped participants in remembering and realizing the purpose of the interview in the mode of an open questioning. Phenomenological interpretative analysis was used to understand how participants make meaning of the phenomenon being studied, an in-depth analysis of the human subject was considered in its singularity, and personal accounts and experiences were noted regarding this pandemic. Findings The results indicated that the participant’s experiences were marked by stress, insecurity, anger, fear, anxiety, traces of painful emotions and a feeling of weakness. Also the prevalence of unknown precariousness and vulnerability of coronavirus aggravated the psychological frailness among people existing in an anxiety-prone situation created by the novel coronavirus. Confronted with these painful experiences, the participants however, used a variety of cognitive and behavioral efforts which needs to be strengthened by psychological care. Research limitations/implications This study based on qualitative method was performed on diverse socio-professional levels, giving an insight to what they are undergoing, their current fears, desires and their feeling. The outcome of this study reveals traces of uncertainty, anxiety, fears, insecurity and hidden imminent death from coronavirus, therefore highlighting a need for immediate psychological interventions. Originality/value This study explores the individual understanding regarding current coronavirus pandemic situation on subjective lived experiences and psychological health in an anxiety-prone context manifested by the health crisis created by the novel corona virus.
