Introdução: O envelhecimento da população e os avanços tecnológicos propiciaram o aumento de idosos com doenças crônicas e ameaçadoras à vida, exigindo uma demanda por profissionais de saúde preparados e com competência em cuidados paliativos. Objetivo: examinar sistematicamente a literatura sobre a oferta de educação em cuidados paliativos para profissionais de saúde. Método: Realizou-se uma revisão sistemática de literatura. Referente à busca de publicações, ocorreu através das bases de dados Scientific Electronic Library Online (SciELO), Literatura Latino-Americana e do Caribe (LILACS) e Literatura Internacional em Ciências da Saúde (PUBMED). Resultados: Foram encontrados 335 estudos, sendo 138 no Lilacs, 67 do Scielo e 130 do Pubmed, dos quais 09 publicações apresentaram os critérios para inclusão no estudo. Conclusão: Da análise dos estudos selecionados, verifica-se que há necessidade de aumentar e melhorar a ofertas de programas educativos em Cuidados Paliativos para profissionais de saúde, nos quais as formações ainda priorizam os programas voltados ao tratamento das condições clínicas agudas e crônicas dos pacientes, sem dar atenção aos cuidados que o paciente deve receber no final de vida, quando não há mais terapêutica modificadora da doença.
Introdução: A síndrome do encarceramento é uma condição neurológica na qual o paciente desenvolve tetraplegia e anartria, se tornando incapaz de executar movimentos dos membros e falar, porém, mantém preservada a propriocepção e a consciência, além do movimento vertical dos olhos, sendo este o único modo do paciente se comunicar. Dentre os vários tipos de monitorizações intraoperatórias, o BIS e o espectrograma nos fornecem a análise do nível de consciência do paciente, permitindo a manutenção de um plano anestésico adequado, reduzindo o despertar intraoperatório e as complicações de um plano anestésico muito profundo. Objetivo: Descrever um caso de uma paciente que desenvolveu a síndrome do encarceramento devido acidente vascular isquêmico de ponte e foi submetida à anestesia geral com monitorização de nível de consciência com o índice bispectral (BIS) e espectrograma. Relato de caso: Paciente de 50 anos, sexo feminino, dona de casa, deu entrada na emergência da UPA São Sebastião - Brasília DF, com quadro de rebaixamento de nível de consciência. Filha relata que paciente referiu queixa de cefaléia intensa, seguida de síncope. Foi avaliada pela equipe médica que optou por intubação orotraqueal e transferiu para serviço de referência em neurologia pela suspeita de AVE. Conclusão: Apesar do estado aparente de coma, o paciente portador da síndrome do encarceramento apresenta dados de monitorização de profundidade anestésica com BIS e espectograma similares à população normal, o que torna seu uso importante na prevenção de desfechos negativos, como a consciência intraoperatória.

Objective: to investigate in the literature the effects of the association of dexamethasone and dexmedetomidine adjuvants in peripheral nerve blocks with local anesthetics. Method: This is a systematic literature review, through the search in the databases, namely: Medical Literature Analysis and Retrieval System online (MEDLINE/PubMed), Latin American Literature in Health Sciences (LILACS), Scientific Electronic Library Online (SCIELO). Data collection took place from September to November 2022. The inclusion criteria were: randomized, retrospective, prospective, cross-sectional and case report studies, without distinction of gender or race, which used dexamethasone or dexmedetomidine or both for blockade of the peripheral nerves and published in the last 10 years (2012-2022). Regarding the exclusion criteria were: the use of other substances that are not characterized as anesthetics or vasoconstrictors; memorials, editorial letters, reviews, research with individuals who had comorbidities, such as peripheral vascular disease, allergy to local anesthetics, pregnant women, history of cardiovascular or liver disease, and studies with more than 10 years of publication. Results: 532 relevant studies were identified from the searched databases, of which 329 were excluded after checking the titles, and 203 of the remaining articles were analyzed based on the title and its compatibility with the inclusion and exclusion criteria. These had their abstracts read, 09 of which were selected for full-text evaluation for eligibility and subsequently included in the qualitative analysis. Conclusion: The review demonstrated that dexamethasone and dexmedetomidine are good adjuvants when used in peripheral nerve blocks, increasing the duration of analgesia, but with some differences in effects.
