Измеряли содержание проинсулина в сыворотке крови детей (82 чел.) в возрасте от 3 до 14 лет, больных сахарным диабетом 1-го типа различной продолжительности. Выявлены 3 группы больных с низким (54%), нормальным (42%) и высоким (4%) уровнями этого прогормона. Не обнаружено зависимости содержания проинсулина от продолжительности заболевания. Высказано предположение о возможности использования содержания проинсулина в крови в качестве параметра, уточняющего патогенез сахарного диабета 1 типа.Ключевые слова: сахарный диабет 1-го типа, проинсулин.ВВЕДЕНИЕ. Согласно общепринятым представлениям, снижение уровня инсулина, являющееся характерным признаком сахарного диабета I типа (СД1), обусловлено аутоиммунной гибелью b-клеток островков Лангерганса. Однако недавно в литературе появились работы, результаты которых не вполне согласуются с указанными представлениями. Так, Vauhkonen с соавт. показали, что у больных данным заболеванием уровень проинсулина в крови был не только не снижен, как можно было бы ожидать, учитывая, что этот гормон является предшественником синтеза инсулина, но и, напротив, оказался выше принятой нормы [1]. Сходные результаты получены и другими авторами при изучении составивших группу риска родственников больных СД1 [2,3]. Это позволило ряду исследователей высказать мнение об этиологической и патогенетической гетерогенности заболевания [1,4].Учитывая, что СД1 болезнь преимущественно молодого возраста, целью настоящего исследования явилось изучение содержания проинсулина в крови детей, страдающих СД1 различной продолжительности, и выявление корреляции этого показателя с содержанием С-пептида.МЕТОДИКА. Обследуемый контингент составили дети в возрасте от 3 до 14 лет с впервые выявленным (n=10), а также продолжительностью менее (n=15) и свыше 1 года (n=57) инсулин-зависимым сахарным диабетом.
