Качество послеоперационной анальгезии может определяться генетическими особенностями паци-ентов, влияющими на метаболизм лекарственных средств. В результате биотрансформации трамадола под воз-действием изофермента цитохрома P-450 происходит образование активного метаболита -О-десметилтрамадола с более высоким сродством к µ-опиоидным рецепторам. В работе изучена эффективность анальгезии на основе трамадола у 48 пациенток после эндоскопических гинекологических операций (консервативная миомэктомия и гистерэктомия) в зависимости от наличия/отсутствия полиморфизмов гена CYP2D6 (C100T и G1846А). Установ-лено, что частота встречаемости указанных полиморфизмов может достигать 30%, что сопровождается более выраженным послеоперационным болевым синдромом. Определение указанных полиморфизмов может быть ак-туальным для подбора оптимальной схемы анальгезии в послеоперационном периоде. Ключевые слова: послеоперационное обезболивание, трамадол, цитохром P-450, генетический полиморфизм Важной задачей послеоперационного ведения пациентов является купирование болевого син-дрома. На сегодняшний день существует боль-шой выбор лекарственных препаратов и методик для анальгезии в послеоперационном периоде, однако часто их эффект остается неудовлетвори-тельным [1] в том числе, и при сравнительно ма-лотравматичных операциях. В последние годы выполняется все больше высокотехнологичных, (например эндоскопических) оперативных вме-шательств, но несмотря на очевидные преимуще-ства данной техники, различия с «открытыми» вмешательствами в плане интенсивности боли могут быть не столь заметными. [2, 4]. Послед-ствиями недостаточной анальгезии являются увеличение числа осложнений, времени восста-новления пациентов после операции и, как след-ствие, увеличение продолжительности госпита-лизации и экономических затрат на лечение [3].Генетические особенности пациента могут определять фармакокинетику препаратов. В частности, в реакции I фазы биотрансформации трамадола (препарата, рекомендуемого и ши-роко используемого при болевом синдроме сред-ней интенсивности) происходит деметилирова-ние с участием изофермента Р-450, CYP2D6. В результате образуется О-десметилтрамадол, об-ладающий существенно более высокой анальге-тической активностью [5]. Наличие полиморфиз-мов гена CYP2D6 может значимо изменять ско-рость биотрансформации и определять эффек-тивность применения трамадола [6]. Вместе с тем, исследования генетических аспектов клини-ческой эффективности и безопасности примене-ния трамадола в целях послеоперационного обез-боливания весьма немногочисленны и разноре-чивы.
The effectiveness of postoperative analgesia may be determined by genetic characteristics of patients that affect the pharmacodynamics and pharmacokinetics of drugs. In particular, as a result of the metabolism of tramadol by isoenzyme of cytochrome P-450 is formed O-desmethyltramadol having higher affinity to mu-opioid receptors. The paper explored the effectiveness of analgesia based on tramadol in 48 patients after endoscopic gynecological surgery depending on the presence/absence of polymorphisms of CYP2D6 gene.It was found that 15 patients with polymorphisms С100Т and G1846A had more intensive postoperative pain, which was accompanied by activation of the sympathetic nervous system. Determination of these polymorphisms may be useful to select the optimal postoperative analgesia.
Objective - to study the dynamics of clinical and laboratory parameters of inflammatory activity of the disease and cytokines in rheumatoid arthritis (RA) patients on a background of tofacitinib (TOFA) treatment.Material and methods. Ten patients with a reliable diagnosis of RA have been examined: patients' age was 51.0 (48.0; 62.0) years, duration of disease was 7.0 (3.0; 20.0) years. All patients had high disease activity: DAS28 -5.88 (5.53; 5.94), CDAI - 33.0 (29.0; 36.0), SDAI - 33.72 (30.75; 36.85). All patients were treated with TOFA at a dose of 5 mg 2 times a day on a background of methotrexate therapy, non-steroidal anti-inflammatory drugs, and glucocorticoids. Observations were performed before treatment and after 3 and 6 months of therapy. Serum levels of 15 cytokines (IL-1β, IL-4, IL-6, TNF-α, INF-γ, IL-10, IL-17A, IL-17F, IL-21, IL-22, IL-23, IL-25, IL-31, IL-33, sCD40L) were examined using multiplex xMAP technology.After 3 and 6 months of TOFA therapy, there was a significant decrease in DAS28 of 4.55 (3.47; 5.16) and 3.92 (3.80; 4.60); CDAI - 16.5 (11.0; 23.0) and 18.0 (15.0; 19.0); SDAI - 16.6 (11.23; 23.06) and 18.07 (15.06; 19.10); ESR - 19.0 (11.0; 26.0) and 7.0 (4.0; 18.0); CRP - 0.56 (0.50; 1.99) and 0.71 (0.51; 1.1) respectively. IL-6 levels decreased after 3 and 6 months of therapy (p<0.05). The concentration of INF-γ significantly decreased after 3 months (p<0.05), but remained unchanged thereafter. Concentrations of IL-25 and IL-31 decreased after 3 months (p<0.05), and by the 6th month of treatment there was an increase, however, not reaching the initial values.Conclusion. The results of the study show the efficacy of TOFA in RA and create prerequisites for further study of the cytokine-dependent mechanisms of inflammation in this disease.
