Ursprünglich wurde Lebensqualität (LQ) in den Wirtschafts-und Sozialwissenschaften als objektiv verankertes Konstrukt verstanden, das durchEinkommenshöhe, Lebensstandard und Wohnverhältnisse determiniert war. Seit Mitte der 1970er Jahre wird der Begriff im medizinischen Kontext immer geläufiger, induziert auch durch die 1948 von der Weltgesundheitsorganisation (WHO) publizierte Definition von Gesundheit als Zustand vollkommenen körperlichen, seelischen und sozialen Wohlbefindens und nicht als die bloße Abwesenheit von Krankheit. Hiermit setzten eine fundamentale Wendung des Verständnisses von Gesundheit und Krankheit im medizinischen Kontext sowie auch eine Infragestellung der traditionellen medizinischen Ergebnisbeurteilungen medizinischer Interventionen ein. Hintergrund Bis in die 60er Jahre des 20. Jh.s findet sich kaum eine Erwähnung des Begriffs LQ im medizinischen Kontext. Zunächst emanzipierte er sich vom politischen Schlagwort zum sozioökonomischen Barometer westlichen Standards. Seit Mitte der 1970er Jahre ist der Begriff der LQ in den USA auch im medizinischen Kontext geläufig und erhält seine konzeptuelle Präzisierung und Pointierung mit dem Schlagwort "health-related quality of life". Lebensqualität ist hier durch die Subjektivität der Bewertung der eigenen LQ charakterisiert [2, 22]. Die Betroffenen oder Patienten werden als beste Experten ihrer Gesundheit, ihrer LQ und ihres Wohlbefindens gesehen. In Deutschland finden sich in den 1980er Jahren und verstärkt in den 1990er Jahren erste Buchpublikationen, die den Begriff der gesundheitsbezogenen Lebensqualität (gbzLQ) theoretisch und methodologisch beleuchten. Konzeptuelle und theoretische Überlegungen zum Begriff sind bis heute allerdings eher rar; dafür hat sich eine operationale Definition von LQ durchgesetzt, deren konstituierende Dimensionen das Konstrukt konsensfähig macht [2, 22]. Es besteht Übereinstimmung bei 4 Bereichen, über die gbzLQ operationalisiert werden kann: das körperliche und psychische Wohlbefinden, die sozialen Beziehungen und die Funktionsfähigkeit im Alltag. Lebensqualität sollte also multidimensional im Hinblick auf krankheitsübergrei-fende (generische), aber auch krankheitsspezifische Aspekte beurteilt werden, insbesondere von Patienten selbst. Nur wenn diese dazu nicht in der Lage sind, sollte die gbzLQ durch einen Beobachter fremdbeurteilt erhoben werden [2, 22]. Einige wenige Arbeitsgruppen [22, 23, 24] erweiterten diese Definition um die kognitive Dimension: Unter der psychischen Dimension werden hier Emotionen und kognitive Funktionen subsumiert. Dies kann insbesondere im Rahmen der Untersuchung von gbzLQ bei neurologischen Erkrankungen von Bedeutung sein. Patienten mit neurologischen Einschränkungen leiden oft unter kognitiven Einbußen und deren Folgen auf das subjektive Wohlbefinden. Trotzdem wird dieser Aspekt in der Erfassung von gbzLQ meist ausgeklammert.
