Angiología CARTAS CIENTÍFICASRotura de aneurisma venoso en la unión safeno femoral en contexto de recidiva varicosa ଝ Los cavernomas safenofemorales son una entidad relativamente frecuente que se produce a consecuencia de una recidiva varicosa en el cayado safeno; su rotura es excepcional.Se presenta el caso de una mujer de 84 años, con antecedentes personales de: arritmia cardíaca por fibrilación auricular, anticoagulada con acenocumarol, prótesis aórtica biológica con disfunción moderada, hipertensión pulmonar, insuficiencia cardiaca congestiva (NYHA II-IV) e intervenida de varices en miembro inferior derecho mediante safenectomía de vena safena interna con ligadura de cayado hace más de 20 años, con varices recidivadas en dicho miembro.Acude a urgencias por aparición de forma súbita, en relación con maniobra de Valsalva, de una masa inguinal derecha dolorosa. Sin cambios en el hábito intestinal. Sin náuseas ni vómitos. A la exploración presentaba una tumoración eritematosa, no pulsátil, dolorosa e irreductible en la región inguinal. Presentaba pulsos conservados a todos los niveles, con ausencia de soplos. Novo-Martínez).Ante estos hallazgos, inicialmente la sospecha fue de hernia inguinocrural estrangulada, solicitándose valoración por cirugía general que, dado el carácter irreductible de la masa y la ausencia de clínica de oclusión intestinal, solicita una ecografía abdominal completa con los siguientes hallazgos: hematoma agudo en tejido celular subcutáneo en región inguinal derecha en relación con aneurisma femoral venoso ( fig. 1).Posteriormente se realiza una TAC abdominopélvico con contraste que confirma una dilatación sacular de la vena femoral, en comunicación con un gran hematoma agudo (5 × 4,5 cm), siendo diagnosticado de aneurisma venoso en neocayado safenofemoral roto ( fig. 2).Con dicho diagnóstico se avisa al servicio de cirugía vascular, que realiza una intervención quirúrgica urgente. La cirugía se lleva a cabo bajo anestesia regional, mediante incisión longitudinal en región inguinal derecha y disección de vena femoral en sentido proximal para su control, con el hallazgo de neocayado en unión safenofemoral. Se procede a la ligadura de sus ramas, exclusión del aneurisma y ligadura en unión safenofemoral.El informe de anatomía patológica describe un aneurisma sacular venoso de 2 cm de diámetro máximo, con fenómeno trombótico.Durante el ingreso la paciente es tratada con antibioterapia de amplio espectro de forma profiláctica y heparinas de bajo peso molecular a altas dosis tras retirada de acenocumarol. Tras una semana, es dada de alta con ausencia de complicaciones, con buen aspecto de herida quirúrgica y mejoría adicional de su clínica varicosa. Se...
