Переливание гемокомпонентов -значимая составляющая терапии при кровопотере, анемии, хирургических операциях. Одна из основных задач учреждений службы крови -заготовка качественных трансфузионных сред, эффективность применения которых напрямую зависит от функционирования системы безопасности. Реализация данного направления работы невозможна без лабораторного обследования донорского биоматериала, обязательным условием которого является стандартизация процедур, обеспечивающая достоверность и воспроизводимость результатов. Цель настоящего исследования -провести анализ существующих методов определения гематологических показателей и оценить их пригодность для контроля качества компонентов крови. Материал и методы. Тестировали образцы эритроцитсодержащих компонентов крови, свежезамороженной плазмы, концентрата тромбоцитов, а также эталонные буферные растворы для рН-метрии. Для определения количества клеток крови использовали кондуктометрию с гидродинамическим фокусированием, подсчет в камере Горяева и лазерную проточную цитофлуориметрию. Величину рН измеряли потенциометрически. Содержание общего и свободного гемоглобина оценивали фотометрическим, гемиглобинцианидным и SLS-методом. Значение гематокрита определяли на гематологическом анализаторе и путем центрифугирования в гематокритных капиллярах. Проверку активности фактора VIII проводили клоттинговым методом. Результаты и их обсуждение. В статье приведены обобщенные результаты метрологической оценки и сравнительного анализа перечисленных лабораторных методов с учетом их чувствительности и прецизионности. Заключение. Сформирован перечень рекомендуемых методик контроля качества компонентов крови, для каждой из которых установлена приписанная характеристика сходимости.Ключевые слова: контроль качества, компоненты крови, показатели безопасности, метрологическая оценка. Human anti-D immunoglobulin preparations derived from human immune plasma are much needed and highly effective for specific anti-D prevention of perinatal complications and treatment of primary immune thrombocytopenia. The effectiveness of immune suppression is a direct function of the active ingredient dose received with the medicinal product. To improve the accuracy of anti-D antibody quantification, it is recommended to use certified reference materials with values assigned in international units (IUs). The aim of this study was to analyse the main stages in the development of the international standards (ISs) for human anti-D immunoglobulin potency testing and to substantiate the need for a national standard for anti-Rho(anti-D) antibody quantification. The article describes the creation of the first and subsequent ISs, the procedure for establishing the IU equivalent for the anti-Rho(anti-D) antibody concentration, the characteristics of the raw materials and preparations used, and the anti-Rho(anti-D) antibody assay methods applied to certify the ISs. According to the study conclusions, it is necessary to develop and certify a national standard for the content of anti-Rho(anti-D) antibodies that will meet the requirements of the corresponding Russian regulations.
Quality assurance of transfusion media is a fundamental principle of the functioning of all blood service institutions, allowing guaranteeing the safety of the recipient. An integral part of the procuring process of blood components is their laboratory testing, which results reliability and reproducibility can be proved by carrying out of validation of analytical methods. The aim of the study was to define the rules for planning and carrying out validation tests of methods for quality control of blood components, including verification of their compliance with established acceptance criteria. Material and methods. The analysis of the literature about the safety of blood components and the quality of laboratory tests was carried out with an assessment of the applied significance of the information proved. Results and discussion. As a result of the work done, a unified model for confirming of the accuracy of determining of the safety indicators of transfusion media was created, which contributes to improving of the quality of donor biomaterial and the effectiveness of therapy. Conclusions. The presented validation design is relevant for medical organizations working in the sphere of blood donation and its components.
