Elsevier has created a COVID-19 resource centre with free information in English and Mandarin on the novel coronavirus COVID-19. The COVID-19 resource centre is hosted on Elsevier Connect, the company's public news and information website. Elsevier hereby grants permission to make all its COVID-19-related research that is available on the COVID-19 resource centre-including this research content-immediately available in PubMed Central and other publicly funded repositories, such as the WHO COVID database with rights for unrestricted research re-use and analyses in any form or by any means with acknowledgement of the original source. These permissions are granted for free by Elsevier for as long as the COVID-19 resource centre remains active.
Нейробластома (НБ) -редкая злокачественная опухоль, которая характеризуется крайне вариабельным биологическим поведением, включающим спонтанную и индуцированную регрессию, созревание и прогрессию. Для оценки прогноза и ответа на терапию при НБ используется определение ряда онкологических маркеров, таких как нейрон-специфическая енолаза, лактатдегидрогеназа, ферритин. Тем не менее разработанные до настоящего момента модели оценки прогноза не позволяют полностью предсказать специфику течения заболевания. В связи с этим создание методов мониторинга статуса опухоли в режиме реального времени, основанное на анализе циркулирующих опухолевых биомаркеров, представляется чрезвычайно актуальным. Характерной особенностью НБ является экспрессия на поверхности опухолевых клеток различных ганглиозидов. В частности, до 30 % всех ганглиозидов достигает содержание дисиалоганглиозида GD2. При этом уровень экспрессии GD2 зависит от дифференцировки нейрогенных опухолей: наибольшие концентрации определяются на клетках НБ, наименьшие -на ганглионейробластоме и ганглионейроме. Феномен «шеддинга», так называемого смывания, GD2 с мембраны клеток НБ позволяет оценивать его концентрации в периферическом кровотоке. В статье рассмотрен эволюционный путь методологии детекции GD2 в сыворотке крови с акцентом на недавно пришедшую в медицинскую отрасль методику жидкостной хроматографии/ тандемной масс-спектрометрии, применение которой может стать отправной точкой в становлении GD2 как растворимого биомаркера опухолевого процесса, безусловно, являющегося мощным приложением в клинической практике, дающим надежду на улучшение диагностики, прогноза и стратегии риск-адаптированной терапии, основанных на четко определенных, измеримых детерминантах в соответствии с концепцией доказательной медицины.
Infantile fibrosarcoma (IFS) is a rare malignant soft tissue tumor characterized by local invasion, a low rate of distant metastasis (1–13%), and manifestation during the first years of life. Overall survival rates range from 89 to 94%, event-free survival rates – from 81 to 84%. Classic IFS is characterized by t(12;15)(p13;q25) translocation resulting in the formation of the ETV6-NTRK3 fusion transcript. However, over the past few years, there have been numerous reports of IFS-like tumors with non-canonical genetic aberrations (BRAF, NTRK1, MET genes) whose prognosis is less predictable. Here we report a rare case of congenital IFS with involvement of subcutaneous fat, indolent course and a non-canonical TPM3-NTRK1 fusion transcript identified in the soft tissue and intradermal tumor components with different histological features. The patient’s parents gave their consent to the use of their child’s data, including photographs, for research purposes and in publications. The literature review explores modern algorithms for the diagnosis and treatment of IFS in children, including the use of new therapies, such as tropomyosin receptor kinase inhibitors, as well as algorithms for detecting NTRK1, NTRK2, NTRK3 gene rearrangements in solid neoplasms in general, and soft tissue tumors in particular.
