Der traumatischen Kniegelenkverrenkung daft man unter den Luxationen eine Sonderstellung zumessen, da sie verh~ltnism/~ltig selten vorkommt. Nach den Untersuchungen yon K~6~L~I~, K~6Mv,~ und SONNTAO stellen sie 1--2% aller Luxationen dar. Roc~oLL errechnete 1927 an unserer Klinik eine Hi~ufigkeit yon 3%.Die Sel~enhei~ dieser Verletzung bedingt es, dal~ im Schrifttum h~ufig nut Berieh~e fiber einen oder einige wenige F~lle vorgefunden werden, denen auf Grund des ,,Einmaligen" der grol3e ~berbliek fiber Entstehung und Beh~ndlung bei Kuiegelenkverrenkungen versagt bleib~. So konnte tr (1936) Beobaehtungen fiber 10 Fi~lle yon Kniegelenkverrenkungen --die gr61]~e bis dahin verSffentliehte Anzahl derartiger Verletzungen --aus dem Arbei~sunfa]H~rankenhaus Wien mitteflen und L. B6m~ fiihrt 1957 aus don letz~en 23 Jahren 32 yon ihm beobaehCe~e Fi~lle (under EinsehluB der yon K~5~E~ beschriebenen) an.Es schien uns lohnenswert, die an unserer Klinik beobachteten F/~lle zusammenzustellen, uncl uns durch Nachtmtersuchung der Verletzten yon den verbliebenen Folgen der Verletzungen zu fiberzeugen.Seit MALO~G~E wird grunds/~r die Art der Verrenkung nach der Verschiebung des kSrpeffernen Gelenkantefls bezeichne~, eine Feststellung, die zwar sehr klar und eindeu~ig ist, aber h~ufig bei Beschreibungen der Luxationen nicht berficksichtigt wird. Bei der Kniegelenkverrenkung wird je nach der Verschiebung des Schienbeinkop~es eine vordere, hintere, seitliche oder Drehverrenkung beschrieben. Der Nachweis eines traumatisehen Pneumocephalus bei einem unserer Frischverletzten veranlaI3te uns, diese mSgliche Folge einer Schadel-Hirnverletzung eingehender zu studieren. ])abel mul~ten wit festste]len, dab zwar das Krankheit, sbild als solches durchaus bekannt ist, jedoch sind die Angaben im Schrifttum.sehr spar]ich, da moistens nut Einzelfs beschrieben wurden (BAY, JAEGER, KILLIAN, LINK und SCHLEVS-SING, Sc~n~z u. Mitarb., PIA). Bei diesen Fallen handelt es sich aber fiberwiegend um einen sekund~ren Pnenmocephalus, d.h. um Vergnderungen, die erst lgngere Zeit nach einem Unfall oder naeh einer anderen Erkrankung bemerkt wurden. Wghrend diese Formen bereits als sehr se]ten bezeiehnet werden, fehlen Angaben fiber einen Pneumoeephalus, der unmittelbar naeh einer Schadel-Hirnverletzung erkannt wurde, fast ggnzlieh. Aus diesem Grunde haben wir einmal al[e in den letzten ]0 Jahren an unserer Klinik beobachteten Fi~l]e yon Pneumocepha]us zusammengestellt, um Unterlagen fiber dieses I~rankheitsbild aus einem gr6Beren Krankengut zu erha]ten. Bevor wir fiber die yon uns gesehenen Falle beriehten, mul3 eine Klarstellun(j der Bezeichnung und der Einteilung dieser mSglichen Folge einer SehAdel-Hirnverletzung erfolgen. Der Nachweis yon L~t im Bereieh der Schi~delhShle win-de im Sehri~ttum mit zahlreiehen Namen belegt (K~sP~, K~LL~, T6NNIS und F~ow~I~). ~Venn es sich um eine umsehriebene Luftansammlung in'einem begrenzten Raum handelte, sprach man yon elner Pneumoeele, Pneumatoeele, Pneumocysta oder Aerocele. Breitete sieh die Luft dagegen um*egelmaBig im Schadelinneren aus, so nannte man diese Vers Pneumocephalus, Pneumatocephalus, Pneumoeranium, Pneumatosis intracranialis, eerebrales Emphysem oder Emphysema capitis. Durch Hinzv_ffigen yon Adjektiven versuehten auBerdem manche Autoren eine genauere Ortsangabe des Luftnachweises im Schadel zu geben, beispielsweise Pneumoeephalus externus, internus, intracerebra-
