RESUMEChez les hommes &g6s, il existe une d6croissance progressive du taux de testost6rone.Apres 50 ans, la chute de ce taux est beaucoup plus marqu~e et peut entrainer une vari6t6 de signes cliniques incluant une d6ficience sexuelle, une baisse de la force musculaire, une an6mie, des sympt6mes d6pressifs.Dans plusieurs cas, il reste ~ d6montrer que ces sympt6-mes sont la cons6quence du deficit androg6nique. Sous traitement substitutif, ces sympt6mes peuvent r6gresser. II existe une augmentation progressive de la SHBG (Sex Hormone Binding Globulin) avec I'&ge. Le diagnostic de deficit androg6nique n6cessite de prendre en compte la forme bioactive de la testosterone (c'est-~-dire testost6-rone libre + testosterone li6e & I'albumine). De nombreux tests ont 6t~ r6alis~s pour d~tecter la d6ficience androgenique. La plupart de ces tests ont une bonne sensibilite rnais une faible sp6cificit~ ; ceci est dD & I'interf~rence de la d6pression ind6pendamment des androg~nes et du niveau de qualit6 de vie.
