Introduction: This study aimed to show the clinical outcomes of implants inserted by guided surgery supporting mandibular overdentures in edentulous patients. Patients and methods: Mandibular edentulous patients were diagnosed with an oral examination, cone-beam computerized tomography, and diagnostic casts for intermaxillary relations and treated with overdentures over two implants by guided surgery. After flapless surgery, implants were early loaded with an overdenture at 6 weeks. Results and discussion: Fourteen patients (nine females and five males) were treated with 28 implants. Four patients (28.6%) had a previous history of periodontitis. Five patients (35.7%) were smokers. Nine patients (64.3%) suffered from systemic diseases (i.e., diabetes, cardiovascular diseases). The clinical follow-up of the study was 44.7 ± 31.4 months. Clinical outcomes showed a global success of 100% of implants. Fourteen overdentures were placed in the patients over the implants. Mean marginal bone loss was 1.25 mm ± 0.95 mm. Four patients (28.6%) showed some kind of mechanical prosthodontic complications. Six implants (21.4%) were associated with peri-implantitis. Conclusions: This study indicates that treatment of mandibular edentulous patients with overdentures by guided surgery and early loading of implants placed appears to be a successful implant protocol.

La planificación preoperatoria de la posición de los implantes constituye una parte importante de la coordinación quirúrgica y prostodóncica que se ha ido incrementando para conseguir resultados funcionales y estéticos. La cirugía guiada de implantes ha aumentado su popularidad, particularmente debido a los avances y a la utilización de la tomografia computarizada de haz cónico (CBCT) y a la planificación del tratamiento con implantes mediante programas informáticos 3D que valoran la localización del implante. De hecho, los pacientes edéntulos pueden ser diagnosticados con una CBCT y tratados con varios implantes por cirugía guiada y carga inmediata. La CBCT constituye un método no invasivo para describir las estructuras maxilofaciales evaluando la cantidad y calidad ósea de los rebordes alveolares. La incorporación de softwares específicos de implantología guiada puede mejorar la planificación virtual de la cirugía sin colgajo y los resultados de los implantes colocados con una férula quirúrgica guiada. En muchos casos, la carga inmediata optimiza el éxito de la técnica de cirugía guiada con muchos beneficios como la reducción del tiempo del tratamiento y el confort del paciente. RESUMENIntroducción: La implantología oral representa en la actualidad, una modalidad terapéutica odontológica en los pacientes con pérdida dental total y parcial. El estudio muestra la evaluación de los pacientes tratados mediante carga precoz de implantes de titanio con conexión interna y superficie arenada y grabada. Métodos: 39 pacientes con pérdidas dentales fueron tratados con implantes con conexión interna y superficie arenada y grabada Frontier GMI®. Los implantes fueron cargados funcionalmente tras un periodo de tiempo de 6 semanas en la mandíbula y 8 semanas en el maxilar superior. Los hallazgos clínicos (implantológicos y prostodóncicos) se han seguido durante 1 año. Resultados: 111 implantes fueron insertados en ambos maxilares (50 en el maxilar superior y 61 en la mandíbula) para su rehabilitación prostodóncica. 44 implantes fueron insertados en el sector anterior y 67 implantes en el sector posterior. Después de 1 año de seguimiento clínico, los resultados indican una supervivencia y éxito de los implantes del 99,1%. Durante el periodo de cicatrización libre de carga funcional, se perdió 1 implante por movilidad. Como complicaciones tardías hubo un caso de periimplantitis. Las restauraciones prostodóncicas incluyeron 30 coronas unitarias, 15 puentes fijos, 5 sobredentaduras con bolas y 2 rehabilitaciones totales fijas. Conclusiones: Los hallazgos clínicos del presente estudio indican que la rehabilitación prostodón-cica mediante la inserción de implantes de titanio con conexión interna y superficie arenada + grabada, y cargados precozmente, representa una terapéutica odontológica con éxito elevado.PALABRAS CLAVE: Implantes dentales, superficie arenada y grabada, conexión interna, carga precoz, oseointegración, implantología oral. with 72 external connection Galimplant® implants with a sandblasted and acid etched surface. In each patient 4 implants were inserted. The implants were functionally loaded with a locator attachment after an 8-week period. Results: 18 maxillary overdentures anchored on 4 implants were made. Early complication were found on the unloaded healing period, caused by an implant that was removed due to mobility. These results indicate a 98.7% survival and success rate after an average following time of 63.7 months (range: 54-80). The average bone loss was 0.9mm (0.4-1.7mm). The prosthodontics complications were related to attachment changes in 2 patients (11%). Conclusions: The clinical findings of the present study indicate that maxillary overdentures using 4 implants represent a successful implant technique in the oral rehabilitation of completely edentulous geriatric patients.
Introduction and Objectives: Today, oral implantology constitute a therapeutic modality in the prosthodontic treatment of edentulous geriatric patients. This study reports the long-term evaluation of edentulous patients treated with mandibular overdentures by early loading of implants with acid-etched surface. Materials and Methods: 13 edentulous patients were treated with 45 TSA Defcon ® acid-etched surface implants for prosthodontic rehabilitation with overdentures in the mandible. All implants were inserted in onestage. Implants were loaded after a healing free-loading period between 6 weeks. Clinical findings (implant and prosthodontics) were followed during 15 years. Results: Clinical results indicate a survival and success rate of implants of 94.4%. One implant were lost during the healing period. After, two implants were lost by periimplantitis. 76.9% of patients were treated with overdentures with bar (61.5% by 4 implants and 15.4% by 3 implants), and the rest of patients (23.1%) with overdentures retained with 2 implants. 77% of patients need attachment changes. Two overdentures were removed. Conclusions: Clinical results of this study indicate that rehabilitation with mandibular overdentures of geriatric patients with etched-surface implants early loaded can achieve long-term successful outcomes.
