INTRODUCTION Smoking prevalence in Russia is one of the highest in the world. It leads to significant damage to the National Health Service. The purpose of the present research was to assess the effectiveness of a qualified smoking-cessation (SC) center in Moscow, and to identify the main determinants of smoking cessation. METHODS The subjects of this study were a cohort of smokers (n=524) who had attended the SC service in Moscow between 2012-2015. They were followed, for at least 6 months after receiving the professional counselling in smoking cessation, to assess the results of the assistance and to identify determinants of successful smoking cessation. RESULTS Of the smokers, 19% succeeded in reducing by more than half the number of smoked cigarettes, more than 46% of patients completely stopped smoking for different terms: 38% for more than a month and 24% for more than half a year. Odds ratios (ORs) indicated that the probability of successful smoking cessation significantly increased if there was a previous success in smoking cessation: OR 3.71 (95% CI 1.70-8.12); and if there was a high level of motivation to stop smoking OR 4.3 (95% CI 1.92-5.61). The probability of successful smoking cessation decreased with intensity of smoking > 10 cigarettes a day: OR 0.57 (95% CI 0.31-1.02); and an elevated (>7 points) Fagerström-test score OR 0.64 (95% CI 0.37-1.07). CONCLUSIONS Degree of motivation and willingness to quit smoking were the principal determinants of the effectiveness in the attempt to quit smoking within our study cohort.
ФАКТОРЫ РИСКА НЕИНФЕКЦИОННЫХ ЗАБОЛЕВАНИЙЭпидемия табакокурения (ТК) приводит к огромным невосполнимым потерям для здоровья населения. Экс-перты ВОЗ среди десяти ведущих причин смерти населе-ния Земли на второе место (после артериальной гипертен-зии) ставят ТК. По данным статистических расчетов [1], табачная интоксикация и табакозависимые болезни явля-ются причиной более 17% случаев смерти в России. По-казано также, что с ТК связано 43% всех случаев смерти мужчин в возрасте 35-69 лет от злокачественных опухо-лей и 89% -от рака легкого [2]. Высокий уровень распро-странения активного ТК среди населения ведет к большо-му распространению и пассивного ТК, т.е. вторичной Проведено специальное обследование выборки работников кафе и ресторанов численностью 104 человека. Обследование включало опрос для уточнения курительного статуса и анамнеза, а также взятие и анализ биопроб для объективной количественной оценки содержания биомаркеров табака в организме обследованных лиц. Результаты анализа биопроб, взятых у обследуемых лиц, показали, что во всех видах биопроб содержание маркеров табака у курящих лиц было многократно больше, чем у некурящих. В то же время и у многих некурящих лиц установлено наличие в биопробах в небольших количествах специфических маркеров табака, что следует отнести к последствиям их пассивной экспозиции к табачному дыму. Значения медианы концентраций двух биомаркеров табака (котинина в моче и никотина в волосах) были практически одинаковыми для групп лиц, курящих только сигареты, и лиц, курящих только кальян. Полученные в настоящем исследовании данные могут служить доказательством необходимости включения предприятий с использованием кальянов в законодательные правила по созданию зон, свободных от табака.

It encompassed an inquiry to specify their smoking status and history data, as well as the taking and analysis of biosamples to objectively quantify the levels of tobacco biomarkers in the examinees. The analysis of the examinees' biosamples showed that all their types taken from smokers showed a manifold increase in the level of tobacco markers than those from non-smokers. At the same time, the biosamples from many non-smokers were found to contain a small number of specific tobacco markers, which should be referred to the sequels of their passive exposure to tobacco smoke. The median concentrations of two tobacco biomarkers (urine cotinine and hair nicotine) were virtually equal in people smoking only cigarettes and in those smoking only hookah. The findings may serve as evidence for the necessity of including the hookah establishments into the legislation regulations to create tobacco-free areas.Keywords: active and passive tobacco smoking, smoking behavior, biomarkers of tobacco intoxication, hookah smoking.
