For each strategy the following points were addressed: the information to be given to the consultee, the procedure and the indications. In addition, the committee's opinion about BRCA1 and BRCA2 mutation screening is that population-based, or even large-scale, implementation are not justified. Although no scientific evidence is available, the committee feels that specific management is indispensable and advocates the use of defined and evaluated procedures, and participation in clinical trials.
Quelles sont les raisons qui expliquent la place qu'ont prise les associations dans les structures médi-cales ? Elles tiennent à nos yeux aux profondes modifications survenues dans le domaine de la santé.La prise en compte de la personne humaine sur ce plan n'a cessé de se développer : initiée dans la loi Huriet-Sérusclat de 1988, elle s'est poursuivie avec la loi de 1999. Elle est enfin devenue centrale dans la loi du 4 mars 2002 qui exprime la volonté du législateur d'informer complètement les patients et de leur permettre de participer aux décisions les concernant.Ces modifications dans la prise en charge des patients expliquent, à nos yeux, le rôle nouveau et croissant des associations de patients ou d'usagers dans la société. Notre présence ici, auprès des politiques, en est un témoignage.La transformation du patient en acteur du soin s'est faite rapidement. Si l'éthique et la loi s'accordent sur ces modifications des relations soignants-soignés, celles-ci ne sont pas simples à appliquer dans les faits et demandent une réflexion attentive de chacun. La modification du statut du patient s'accompagne de celle de celui du médecin : ses obligations changent, il était maître de la décision il doit devenir celui qui informe y compris des choix thérapeutiques. Dans ce contexte si la place des associations est inscrite dans la loi, il reste à la préciser car il s'agit de modifications récentes. L'expérience de notre association, Europa Donna Forum France, nous montre combien le rôle conféré aux patientes peut être angoissant, combien une information « claire et loyale » peut être difficile à entendre en particulier s'agissant d'une pathologie aussi traumatisante que le cancer. Plus angoissant encore à assumer est le choix thérapeutique, ou ce qu'on appelle la décision partagée. Si ces changements dans la relation médecin-malade sont irréversibles, ils doivent s'accompagner d'une réflexion en commun : soignants, soignés, associations sur le rôle qui devrait être imparti à chacun. La responsabilité des associations est ici majeure. Comment informer assez pour soulager l'angoisse et offrir un soutien ? Comment se situer par rapport à l'information donnée par les soignants ? Quelle est notre place entre le patient et le soignant ? Quelle est notre place dans le système de soins et la vie civile ? Il n'y a pas encore de réponses précises à ces questions.Les associations ont un rôle de liens, d'interprètes, de relais de l'information. L'association est un lien : les membres s'y rencontrent, échangent leurs expériences et y trouvent un soutien. Le désir de partager des expériences communes est un motif fréquent d'adhésion. Le partage des objectifs permet une utile mobilisation des personnes.L'association est un interprète des besoins et des revendications de la collectivité qu'elle représente, elle a un rôle social et politique, il est de mieux en mieux reconnu. Une formation est nécessaire pour légitimer les revendications et savoir où et comment les exprimer : relations avec les administrations, les media par exemple.La ...
