Hintergrund: Die Geschichte von Simethicon umfasst eine Periode von Über 50 Jahren. Die wichtigsten Eigenschaften sind die entschÄumende Reduktion der OberflÄchenspannung, die Reduktion der OberflÄchenviskositÄt und die HydrophobizitÄt, wodurch Simethicon sich rasch Über OberflÄchen verbreitet. Simethicon wird nicht resorbiert und ist absolut ungiftig. WÄhrend Simethicon einen festen Platz bei diversen diagnostischen Prozeduren gefunden hat, ergaben die therapeutischen Studien zum Teil widersprÜchliche Ergebnisse. Zielsetzung: Ziel dieser Übersicht war eine Beurteilung der therapeutischen Wirksamkeit und Sicherheit von Simethicon nach klinisch relevanten Kriterien und gemÄss den Richtlinien der Cochrane Collaboration. Methodik: Als Datenquellen dienten bibliographische Datenbanken sowie Referenzen von Übersichtsarbeiten und BÜchern und persÖnliche Kontakte mit Experten, bis Juli 2007. Alle Publikationen wurden ÜberprÜft und diejenigen, die prospektive Studien betrafen, wurden nach Indikation, Studiendesign und methodologischer QualitÄt tabellarisch zusammengefasst. Ergebnisse: Es wurden insgesamt 83 Publikationen identifiziert; von diesen konnten 14 Studien beim Einsatz von diagnostischen AbklÄrungen und 23 therapeutische Studien nÄher analysiert werden. Die Wirksamkeits-Evidenz war «gut» bezÜglich entschÄumender Wirkung bei diagnostischen Eingriffen und als Therapeutikum bei: 1.) Funktioneller Dyspepsie (4 Studien; 266 Patienten mit Simethicon vs. 310 Kontrollen), wobei Simethicon sowohl Plazebo wie auch Cisaprid Überlegen war; 2.) ReisediarrhÖ, (2 grossangelegte Studien; 248 Simethicon Patienten vs. 244 Plazebo) wobei Simethicon dem Plazebo Überlegen war (gesteigerte Wirksamkeit wenn mit einem µ-opioid-Agonisten kombiniert). Die Wirksamkeits-Evidenz war «nicht-schlÜssig» bei: 1.) «Reizdarm-artigen» Beschwerden (2 Studien; 80 Patienten mit Simethicon vs. 54 Kontrollen); 2.) Normalisierung der postoperativen DarmaktivitÄt (4 meist Ältere Studien; 847 Simethicon Patienten vs. 631 Kontrollen); 3.) 3-monats Koliken (7 Studien; 306 SÄuglinge mit Simethicon vs. 296 Kontrollen); 4.) In Kombinationen bei Symptomen von gastroesophagealen Reflux (3 Studien) und bei adhÄsionsbedingten partiellen DÜnndarmobstruktionen (1 Studie). Schlussfolgerungen: Simethicon kann bei verschiedenen Indikationen bei welchen eine entschÄumende und abdeckende Wirkungen erwÜnscht sind, von Nutzen sein. Randomisierte klinische Studien haben die Wirksamkeit von Simethicon bei verschiedenen Indikationen nachgewiesen, zusÄtzlich zu den gut etablierten Anwendung bei diagnostischen AbklÄrungen. Weitere randomisierte klinische Studien bei nicht gesicherten Indikationen sind notwendig, besonders in Anbetracht der sehr grossen Sicherheitsspanne von Simethicon.
Hintergrund: Zur Zeit stehen wenige pharmakologische Therapien bei der Behandlung der funktionellen Dyspepsie zur VerfÜgung. Aufgrund der bekannten hohen choleretischen Wirkung kÖnnen Extrakte von ArtischockenblÄttern nÜtzlich sein. Studienziel: Nachweis der Wirksamkeit und VertrÄglichkeit des hochdosierten ArtischockenblÄtterextraktes LI 220 bei Patienten mit funktioneller Dyspepsie (FD). Design: Offene, prospektive, multizentrische Phase-IV-Studie. Methoden: 169 Patienten (59 MÄnner, 110 Frauen, Durchschnittsalter 47.7 Jahre) mit seit mindestens drei Monaten wiederkehrenden funktionellen dyspeptischen Beschwerden und negativem Endoskopiestatus erhielten LI 220 (3x tÄglich 2 Kapseln) wÄhrend 6 Wochen. Grundlage zur Stratifizierung nach unterschiedlicher Symptomatik der Dyspepsie-Typen (refluxartige, ulkusartige und dysmotilitÄtsartige FD) und zur Beurteilung der Wirksamkeit und VertrÄglichkeit war ein validierter Patienten-Fragebogen («Digest Fragebogen»). Ergebnisse: Von 169 behandelten Patienten beendeten 144 die Studie. Die Intent-to-Treat Analyse ergab, dass sich bei 74% der Patienten die dyspeptischen Beschwerden verbesserten (Reduktion der HÄufigkeit und des Schweregrads der Symptome), als auch geringere beschwerdebedingte StÖrungen der tÄglichen AktivitÄten auftraten. Es ergab sich kein signifikanter Unterschied zwischen den funktionellen Dyspesie-Subgruppen. Bei den spezifischen Symptomen wie VÖllegefÜhl, BlÄhungen, Nausea, rasche SÄttigung und Schmerz in der Magengrube war die Verbesserung besonders deutlich. Die PrÜfmedikation zeigte insgesamt eine gute VertrÄglichkeit. Schlussfolgerung: Eine Behandlung mit dem ArtischockenblÄtterextrakt LI 220 kann deshalb, vom Dyspepsietyp unabhÄngig, als Alternative zu den hier Üblicherweise eingesetzten Medikamenten empfohlen werden.
Die AminosÄuren sind die Bausteine der Proteine. Zum Aufbau aller Proteine sind 20 AminosÄuren notwendig. Die Synthese der Proteine erfolgt aus den im KÖrper vorhandenen freien AminosÄuren. FÜr die langfristige Aufrechterhaltung des Stickstoffgleichgewichtes und der Proteinsynthese ist die Zufuhr von lediglich 8 AminosÄuren notwendig. Die anderen 12 AminosÄuren kÖnnen im Organismus selbst gebildet werden. Aus diesem Grund werden die AminosÄuren heute in entbehrliche, unentbehrliche und sogenannte bedingt unentbehrliche unterteilt. Bei den bedingt unentbehrlichen AminosÄuren besteht eine unzureichende endogene Synthese oder ein vermehrter Verbrauch bei bestimmten Erkrankungen. Aufgrund der Tatsache, dass die meisten AminosÄuren vom Organismus selbst gebildet werden kÖnnen, sind MangelzustÄnde sehr selten. Es gibt nur wenige Erkrankungen, welche mit einem isolierten AminosÄuremangel einhergehen. Unter bestimmten Bedingungen (meist schwere Erkrankungen) kÖnnen gewisse AminosÄuren wie Arginin, Cystein, Glutamin, Histidin, Serin und Tyrosin bedingt unentbehrlich werden. Die Zufuhr dieser AminosÄuren kann unter solchen UmstÄnden sich im Verlauf einer Erkrankung positiv auswirken.
