Abstract Heart failure has now become a global cardiovascular problem with high mortality and disability. Sleep disturbance is a common problem in heart failure patients. Sleep disorders that occur in patients with heart failure will cause increased levels of urine catecholamine, plasma leptin, and inflammatory markers. These changes are related to the development of hypertension, activation of the sympathetic nervous system, and blunting of chemoreflex, which will increase the heart burden and ultimately lead to a poor prognosis. Sleep disorders in heart failure patients can be treated with psychological interventions and behaviors including sleep hygiene and Benson relaxation. Nurses, as one of the health professionals, have an important role in this matter. This is because the scope of nursing is meeting basic human needs, namely sleep. Through intervention in sleep hygiene and relaxation, Benson is expected to be able to improve the patient's sleep quality. Keywords: benson relaxation, heart failure, sleep disorders, sleep hygiene.
Pendahuluan: Ketoasidosis diabetikum (KAD) diabetikum diketahui memiliki hubungan dengan prognosis ST Elevation Miocardial Infact (STEMI). Peningkatan kejadian gagal jantung, shock hingga terjadinya kematian. Perawat memiliki peran penting kolaboratif dalam penanganan kejadian tersebut. Penelitian ini bertujuan untuk menggambarkan manajemen asuhan keperawatan pada pasien STEMI dengan KAD. Metode: Pendekatan studi kasus digunakan dalam penelitian ini, dimana kami mempersembahkan penerapan asuhan keperawatan pada pasien laki-laki usia 42 tahun STEMI disertai ketoasidosis diabetikum di ruang gawat darurat. Hasil: Pada studi kasus ini pasien masuk unit gawat darurat dengan keluhan nyeri dada khas dan kondisi hiperglikemia. Hasil pemeriksaan penunjang pasien didiagnosis mengalami STEMI ekstensif anterior onset 3 jam dan KAD, diagnosis keperawatan pada pasien tersebut adalah nyeri akut (NANDA: 00132) dan defisit volume cairan (NANDA 00027), intevensi yang digunakan Manajemen nyeri dengan aktivitas tindakan tatalaksana STEMI dan manajemen cairan dengan aktivitas tindakan tatalaksana cairan KAD sesuai dengan evidence based practice. Evaluasi hasil tindakan keperawatan, nyeri masih terajdi dan defisit volume cairan tidak terjadi. Kesimpulan: STEMI dan KAD merupakan dua kasus gawatdarurat yang memerlukan penanganan yang tepat, manajemen keperawatan dengan pendekatan interpersonal kolaborasi dan berbasis evidence based practice pada pasien pada kasus tersebut dapat meningkatkan outcome yang baik bagi pasien.
Background: Postoperative hypoxia (POH) is a complication that often occurs in patients after Coronary Artery Bypass Grafting (CABG). Breathing exercises are considered effective in overcoming POH after CABG. However, this intervention has not been implemented using the dynamic nurse-patient relationship model from Ida Jean Orlando. The dynamic nurse-patient relationship model is expected to improve the quality of nursing care after cardiac surgery.Purpose: This study aimed to describe the application of breathing exercises in postoperative hypoxia after CABG using the dynamic nurse-patient relationship model.Methods: This study was the summary of the case presentation along with the interventions on three patients after CABG through the dynamic nurse-patient relationship model approach with the main intervention focusing on breathing exercises.Results: After three days of implementing the intervention, most patients showed improvement in POH. This condition was evidenced by an increase in the PaO2/FiO2 ratio from less than 200 to above 200, with an average increase of 89.3. The blood gas analysis results supported the results from a respiratory alkalosis condition to a normal PH and a PaCO2 value from <35 mmHg to the normal range (35-45 mmHg). Also, X-rays of the patients showed pulmonary atelectasis improvement.Conclusion:Applying breathing exercises in postoperative hypoxia after CABG using the dynamic nurse-patient relationship model can improve POH in post-cardiac surgery patients. We recommend using the model theory approach because this theory can see patients’ psychological and physical changes dynamically in post-CABG patients.
Objective Structured Clinical Examination (OSCE) merupakan metode untuk menguji kompetensi klinik secara objektif dan terstruktur dalam bentuk putaran/station dengan waktu tertentu. Artikel ini berupaya memberikan gambaran dan penjelasan terkait program pelatihan OSCE standar nasional secara komprehensif terhadap pengetahuan dosen. Penulisan artikel ini menggunakan pendekatan kuantitatif dengan model deskriptif-analitik. Hasilnya pengetahuan dan wawasan dosen meningkat dalam hal metode penilaian ujian keterampilan klinik dengan pendekatan OSCE. Hal ini dibuktikan dengan nilai pre-test dan post-test yang menunjukkan pengetahuan dosen terkait OSCE rata-ratanya mengalami rerata kenaikan pengetahuan setelah melakukan pelatihan OSCE sebesar 35,29%. Selain itu, dosen juga dapat mengaplikasikan dan merancang model ujian keterampilan klinik dengan OSCE di laboratorium keperawatan, sehingga dapat mempersiapkan mahasiswa dalam menghadapi Exit-Exam. Mahasiswa keperawatan yang terlibat dalam pelatihan OSCE ini juga mendapatkan manfaat berupa pengalaman nyata dalam melakukan ujian keterampilan klinik secara komprehensif. Artikel ini juga berkontribusi bagi institusi pendidikan keperawatan untuk menerapkan metode OSCE dalam ujian keterampilan klinik sebagai upaya meningkatkan kesiapan mahasiswa dalam program Exit-Exam.
