Hemoglobin A1c (HbA1c) diyabetin uzun dönem komplikasyonlarını hassas olarak belirlemektedir. HbA1c ölçümleri hasta başı test cihazları (HBTC) ile yapılabilmekte olup, bu HBTC'lerin yüksek performanslı sıvı kromatografi (HPLC) ile kıyaslanması önem arzetmektedir. Bu çalışmada hemoglobinopatisi olan ve olmayan hastaların HbA1c sonuçlarının HBTC HbA1c cihazı (Tri-stat Analyzer, Trinity Biotech) ve HPLC (Hb Premiere) ile karşılaştırılması amaçlanmıştır. Yöntemler: Hemoglobinopatili 26 diyabetik, hemoglobinopatisiz 51 diyabetik hasta çalışmaya alındı. Hastaların HbA1c analizleri Tri-stat HBTC kullanılarak (Trinity Biotech, Ireland) venöz ve kapiller kandan, Premier Hb 9210 (Trinity Biotech, Ireland) HPLC kullanılarak sadece venöz kandan çalışılmıştır. Bulgular: Hemoglobinopatisi olmayan hastaların HbA1c düzeyleri karşılaştırıldığında HPLC-kapiller kan HBTC arasında (p=0,392), HPLC-venöz kan HBTC arasında (p=0,167) ve venöz kan HBTC-kapiller kan HBTC arasında (p=0,288) istatistiksel olarak anlamlı bir fark saptanmamıştır. Hemoglobinopatisi olan hastaların HbA1c düzeyleri karşılaştırıldığında HPLCvenöz kan HBTC arasında da (p=0,076) istatistiksel olarak anlamlı bir fark saptanmamıştır. Sonuç: Hastalarda, Tri-stat HBTC'yi HbA1c sonuçları HPLC ile karşılaştırıldığında istatistiksel olarak farklı değildi. Bakanlıkça onaylı ve otoriteler tarafından kabul gören HBTC'yi HbA1c analizlerinde güvenle kullanılabilir. Klinisyenler, yakın takibe ihtiyaç duyan ve tedavi rejimi bu takipten etkilenecek tip 2 diyabetli hastalarda her iki yöntemi de tercih edebilirler.
Objective: Complete urinalysis (CUA) is one of the indispensable screening tests of clinical laboratories. The compatibility of this test with urine culture is of indispensable importance in the diagnosis and treatment of urinary tract infections. We aimed to evaluate the suitability of the leukocyte parameter measured in the microscopic units of the fully automated urine analyzers which replace traditional methods, by grouping them according to the results of chemical analysis. Materials and Methods: Leukocyte counts in the reported CUA results of 4685 outpatients and the results of 113 urine cultures studied on the same day were analyzed. Noncentrifugated urine samples were included in the analysis. Cells were digitally imaged by flow microscopy. Chemical analyzes were performed using dual wavelength reflectance method. Urine samples were evaluated after 24 hours of incubation. Results: High power field (HPF) values were recorded by grouping the leukocyte counts as negative, trace, 1+, 2+ and 3+. The arithmetic means of HPF values of the groups were calculated as 1.2, 2.1, 5.0, 11 and 208 white blood cell (WBCs/HPF). Bacterial growth was detected in 19 of 113 patients and no reproduction was observed in the remaining 94 cases. When results of microscopic examinations and chemical analysis were compared with the culture results, the analytical sensitivity, specificity, positive, and negative predictive values for microscopic urinalysis were 25%, 86%, 61.3%, and 58.3%, respectively. While, the analytical sensitivity, specificity, positive, and negative predictive values for chemical analysis of urine were 25.8%, 87.7%, 69.7%, and 51.4%, respectively. Conclusion: The workload of medical laboratories is increasing, and the use of urine autoanalyzers may be preferred for busy laboratories. In the diagnosis and follow-up of urinary tract infections, complete urinalysis by autoanalysers in which the harmony of their microscopy and chemical units are closely monitored, may reduce the need for unnecessary requests for urine culture, but it cannot replace urine culture.
The major considerations for the STAT testing performed in laboratories are to obtain a fast and accurate test result. Increasing number of instruments and methods have been developed for making it possible to perform the STAT testing on plasma rather than serum. The aim of this study is to compare the STAT test results of the serum and plasma sample tubes. Venous blood samples of 78 patients were collected into BD Vacutainer®SST™II Advance serum gel and Vacutainer®Barricor™ plasma tube. The sampling orders of the tubes were switched during blood collection and loading the tubes into the instruments in order to ensure randomization. It was ensured the tubes were inverted, completely filled, and were allowed serum tubes to clot for a minimum of 30 minutes from the time of blood collection. It was determined that the chloride and sodium tests had a good concordance and the remaining tests had a strong level of concordance.
Tip 2 Diabetes Mellitus (T2DM) en sık endokrin hastalıktır. Yüksek glikolize hemoglobin (HbA1c) düzeylerinin kardiyovasküler hastalıklarda (KVH) risk faktörü olduğu ve KVH'ların da diyabetik hastalarda ölüm ve sakatlığa neden olduğu bilinmektedir. HbA1c düzeylerinin doğru ölçülmesi, hastaların tanı ve tedavi süreçlerinin değerlendirilmesinde son derece önemli rol oynamaktadır. Ülkemizde başta talasemi minör olmak üzere hemoglobin varyantları yaygın olarak görülmektedir. Diyabetin kardiyovasküler etkileri nedeniyle de asetilsalisilikasit (ASA) sıklıkla diyabetik hastalar tarafından kullanılmaktadır. Diyabetik nefropati ise son dönemde hemodiyaliz gerektiren böbrek yetmezliğine ve üremiye neden olmaktadır. Bu çalışmada immünoturbidimetri ve yüksek performanslı sıvı kromatografi (HPLC) yöntemlerinin 3 hasta grubunda HbA1c düzeylerini ölçmek üzere karşılaştırılması amaçlanmıştır. Gereç-Yöntem: Bu çalışmaya her biri 20 hastadan oluşan üç grup dahil edilmiştir. Bu çalışmaya 20'şer kişilik 3 hasta grubu dahil edilmiştir. İlk grup hemoglobin elektroforezi kullanılarak "hemoglobinopati" tanısı alan hastalardan, ikinci grup en az 6 aydır günde 100 mg veya üzerinde ASA alan hastalardan ve üçüncü grup kronik hemodiyalize giren hastalardan oluşmaktadır. Hastalardan alınan EDTA içeren tüplere toplanan venöz kan örnekleri, Premier Hb 9210 (Trinity Biotech-USA) ve Cobas c501 (Roche-Almanya) cihazları kullanılarak analiz edilmiştir. Sonuç: Her 3 grupta immünoturbidimetri ile HPLC yöntemi karşılaştırıldığında grupların içinde HbA1c düzeyleri arasında anlamlı bir istatistiksel fark bulunamamıştır (p=0,462, p=0,065 p=0,062). Tartışma: İmmünoturbidimetri ve HPLC, HbA1c ölçümlerinde yaygın olarak kullanılan yöntemlerdendir. İmmünoturbidimetrik yöntemin HbA1c ölçümlerinde HPLC yöntemi ile arasında güçlü bir korelasyon olduğu gösterilmiştir. HbA1c analizini etkileyebilecek hastalıklar ve ilaçlar göz önünde bulundurulmalıdır.
