Amaç: İntihar gençlerde en sık ölüm nedenlerindendir ve sıklığı giderek artmaktadır. Bu araştırmanın amacı, intihar girişiminde bulunan çocuk ve ergenlerin ruhsal tanıları ile klinik ve demografik özelliklerinin saptanmasıdırYöntem: Ocak-Aralık 2017 tarihleri arasında Erciyes Üniversitesi Çocuk Acil Servisine intihar girişimi nedeniyle başvuran 18 yaş altı çocuk ve ergenler retrospektif olarak incelenmiştirBulgular: İntihar girişiminde bulunan 50 olgunun 42’si (%84) kız ve 27’si (%54) 14-16 yaş aralığındaydı. Yüksek doz ilaç alımı en yaygın intihar girişimi yöntemiydi (%96). Girişimde bulunanların % 72’sinde ruhsal bozukluk saptandı. En sık belirlenen ruhsal bozukluklar yaygın anksiyete bozukluğu, majör depresif bozukluk, davranım bozukluğu ve obsesif-kompulsif bozukluktu. İntihar girişimi için en sık tetikleyici faktör aile içi problemlerdi. Olguların % 8’inde hala ölüm isteği mevcuttu. Sonuç: Çalışmamıza göre ergenlerde intihar girişiminde psikiyatrik hastalıklar, aile içi problemler, karşı cinsle ilgili sorunlar, sosyoekonomik durum, okul başarısı gibi faktörler etkilidir. İntihar girişimleriyle ilgili risk faktörlerinin belirlenmelidir. İntihar riski olan ergenler daha sık takip edilmelidir.Aim: Suicide is the most common cause of death in young people and the incidence is increasing day by day. The aim of this study was to investigate psychiatric diagnoses, clinical and demographic characteristics of children and adolescents who attempted suicide. Materials and methods: Patients admitted to Erciyes University Pediatric Emergency Department between January and December 2017 due to suicide attempt were evaluated retrospectively. Results 42 (84%) of the 50 suicide attempts were girls and 27 (54%) were between the ages of 14-16. Drug overdose was the most common way of attempting suicide (96%). Psychopathology was diagnosed in 72 % of these patients. The most frequent psychiatric disorders were generalized anxiety disorder, major depressive disorder, conduct disorder and obsessive-compulsive disorder. The most frequent trigger factor for attempted suicide was family problems. In 8% of the cases, there was still a death wish. Conclusion: According to our study, factors such as psychiatric illnesses, family problems, sexual problems, socioeconomic status, school success in adolescents are effective in suicide attempts. Risk factors for suicide attempts should be determined. Adolescents at risk of suicide should be monitored more frequently.
The immature granulocyte (IG) and immature reticulocyte fraction (IRF) are new analytical parameters of the complete blood count, that have been studied as biomarkers of several inflammatory conditions. Here, our aim is to determine the effectiveness of IG and IRF percentages for the severity of acute bronchiolitis (AB). A single-center, prospective study was performed in patients who were hospitalized for acute bronchiolitis and healthy children were included as a control group. The demographic characteristics, white blood cell (WBC) count, platelet (PLT) count, eosinophil%, IG%, and IRF% values were analysed. Receiver operating characteristics (ROC) analysis was used to compare the diagnostic accuracies and predictive performances. We enrolled 168 infants in the acute bronchiolitis group and 80 in the control group. The Clinical Severity Score (CSS) showed that 48, 93, and 27 patients had mild, moderate, and severe bronchiolitis, respectively. The WBC, PLT, and IRF value were significantly higher in patients (p < 0.001). There was no difference between the patients and control group in terms of IG and eosinophil percentage. Only a positive correlation was observed between the clinical severity of the AB and IRF (p=0.003).The ROC curve analysis indicated that the IRF% cut-off point for predicting severity AB was >12.4, with a sensitivity of 53% and specificity of 88% (Areas under the curves (AUC):0.701,p<0.001). The WBC count, PLT count, and IRF value increased in the AB group. The IRF percentage can be used to predict the clinical severity of AB in children.
Amaç: Toplum kökenli pnömoni (TKP), çocukları etkileyen önemli bir hastalıktır. Her yıl beş yaş altı yüz elli beş milyon çocuğa zatürre teşhisi konmakta, 20 milyonu hastaneye kaldırılmakta ve 2 milyonu ölmektedir. Erken tanı ve şiddet değerlendirmesi mortalite ve morbiditeyi azaltır. Bu çalışmada temel hemogram parametreleri, nötrofil-lenfosit oranı, immatür granülosit, immatür granülosit yüzdesi, CRP ve oksijen satürasyonunun hastalık şiddetine etkisinin belirlenmesi amaçlanmıştır. Gereç ve Yöntem: Bu vaka-kontrol çalışması, Kasım 2018-Mayıs 2019 tarihleri arasında Erciyes Üniversitesi Tıp Fakültesi Çocuk Göğüs Hastalıkları Anabilim Dalı’nda yürütülmüştür. Klinik ve radyolojik bulgulara göre TKP tanısı konan altmış dokuz hasta çalışmaya dahil edildi. Hastalar hafif-orta pnömoni ve şiddetli pnömoni olmak üzere iki alt gruba ayrıldı. Bulgular: Hafif-orta pnömonisi (%32/46,3) ve şiddetli pnömonisi (%37/53,6) olan hastaların lökosit, nötrofil, IGn, IG% ve satürasyonu karşılaştırıldı. İki grup arasında istatistiksel olarak anlamlı fark vardı (p 0,05). Ancak lenfosit sayısı, nötrofil lenfosit oranı (NLO) veya CRP’de istatistiksel olarak anlamlı bir fark yoktu (p>0,05). Pnömoninin şiddetini etkileyen bağımsız faktörleri belirlemek için tek değişkenli analiz kullanıldı.
