From 2-amino-3-ethoxycarbonyl-4,5-dimethyl-, -polymethylenethiophenes (1-4) were synthesized 2,3-disubstituted thieno[2,3-d]dihydropyrrolo-, tetrahydropyrido-, and tetrahydroazepino[1,2-a]pyrimidin-4-ones (5-16) for pharmacological investigations. The 12 compounds (5-16) were individually evaluated for their antiproliferative activities on mammalian cancer cell models. All tested compounds showed weak affection on human cervix adenocarcinoma cells (HeLa) whereas some of the tested compounds exhibited more consistent inhibition of cell growth on murine myeloma cells (P3X). In both cases some compounds enhanced cell proliferation.
Vor dem Hintergrund der hohen Sturzhäufigkeit bei älteren Menschen, insbesondere bei Osteoporosepatienten, war es zentrales Anliegen dieser Studie, ein sensomotorisches Trainingsprogramm auf dem Aero Step zu entwickeln und in einer kontrollierten Längsschnittuntersuchung im Baselinedesign zu evaluieren. In die Studie wurden gesunde Ältere (n = 20; Durchschnittsalter 64,1 Jahre) und Osteoporosepatienten (n = 27; Durchschnittsalter 68,5 Jahre) eingeschlossen und bezüglich der Parameter Sensomotorik (Haltungs-und Gleichgewichtsregulation), Sturzgefahr und Lebensqualität untersucht. Als Untersuchungsinstrumentarien kamen die Posturographie sowie ein selbst entwickeltes Fragebogenset zum Einsatz. Die Erfassung der Lebensqualität erfolgte mit dem WHOQOL-Bref (Deutsche Version). Die Haltungsregulation (Stabilitäts-indikator) verbesserte sich durch das sensomotorische Training bei 80 % (Osteoporosegruppe) bzw. 75 % (Gruppe Gesund) signifikant (p = 0,030; p = 0,013). Die erzielten Verbesserungen basierten auf Anpassungen im somatosensorischen Bereich, wie die signifikanten Veränderungen in der Fourier-Analyse, v. a. in der Gruppe Gesund (p = 0,004), dokumentieren. 76 % der Osteoporosepatienten und 90 % der gesunden Älteren konnten nach Absolvierung des Trainingsprogramms eine Verbesserung ihres gesundheitlichen Befindens feststellen. Auf die Lebensqualität hatte die Intervention hingegen keinen signifikanten Einfluss. Insbesondere die zahlreichen signifikanten Verschlechterungen im Follow up 1 (12 Wochen nach der Intervention) in der Osteoporosegruppe sowie im Follow up 2 (2 Jahre nach der Intervention) in beiden Gruppen und nahezu allen Parametern unterstreichen den Wert eines kontinuierlichen sensomotorischen Trainings im späteren Lebensalter, v. a. bei Osteoporosepatienten. So können beispielsweise Arteriosklerose, Arthrose oder Osteoporose sowohl hinsichtlich des Zeitpunkts ihres Auftretens als auch hinsichtlich ihrer Schwere und ihres Verlaufs durch eine gesundheitsbewusste Lebensführung erheblich beeinflusst werden [1]. Gerade für ältere Menschen mit Osteoporose stellen Stürze ein besonderes Risiko dar, da es im progredienten Verlauf der Erkrankung zu einer Abnahme der Belastbarkeit der Knochenstrukturen kommt. Interventionsstudien zielten bislang über-wiegend auf die Erhöhung der Belastbarkeit der Knochenstrukturen (Gewinn an Knochenmasse) durch körperliche Aktivität und Bewegungstraining ab. Die in Längsschnittstudien ermittelten Adaptationseffekte sind insgesamt allerdings eher gering und können das Risiko einer Fraktur nicht vermindern [2]. The study comprised of healthy elderly people (n = 20; mean age 64.1 years) and osteoporotic patients (n = 27; mean age 68.5 years) examined with regard to the parameters; sensomotoric (posture and balance regulation) falls risk and quality of life. The instruments of the research included posturography, as well as a self-devised questionnaire. The assessment of life quality was carried out by means of WHOQOL-Bref (German version). The sensomotoric training led to a significant improvement of posture regulation (stability indicator); 80 % in the Osteoporotic group and 75 % in the healthy group (p = 0.030; p = 0.013). The improvements were based on the adaptation in the somatosensoric region as documented by significant changes in the Fourier-Analysis, especially in the healthy group (p = 0.004).
