Standards for culturally competent care should be adopted by all care delivery settings. Public and organizational policies openly declaring healthcare settings as cultural safe zones, and explicit organizational commitment to culturally safe care, would set clear expectations for providers and help ensure a positive patient experience.
GirişDeğişen toplumsal yapılar, yaşlanan dünya ve biyoteknolojik geliş-meler sonucunda artık ölümler evde değil hastanelerde, hastanelerin yoğun bakım ünitelerinde gerçekleşmektedir. Yeni teknolojik uygulamalar ölüm kavramını değiştirmekte, belli durumlarda aileleri ve sağlık ça-lışanlarını ölüm süreçleri hakkında karar vermek zorunda bırakmaktadır. Bunun sonucunda da hasta, hasta yakınları ve sağlık çalışanları özellikle de hekimler pek çok etik ikilemle karşı karşıya kalmaktadır. Yaşamı tehdit eden hastalıklarda palyatif bakımla ilgili gereklilikler çalışmaların ve tartışmaların arttığı bu dönemde palyatif bakım etiği ile gündeme gelen konu başlıkları bu makalede tartışmaya açılacaktır. Aynı zamanda, yaşam sonu kararları başlığı altında incelenen kavramlar palyatif bakımın kendine has dinamikleri çerçevesinde değerlendirilecektir.Dünya Sağlık Örgütünün (DSÖ) tanımına göre, palyatif bakım yaşamı tehdit eden hastalıklarla mücadele eden hasta ve hasta yakınlarının yaşam kalitelerini yükseltme hedefinde olan bir yaklaşımdır. Bu yaklaşım-da ağrıyı, acıyı hafifletmeye yönelik çalışmaların yanı sıra fiziksel, psikososyal ve ruhsal (manevi) bakım da yer almaktadır. DSÖ palyatif bakımın yaşamı onayladığını ve ölüme normal bir süreç olarak saygı duyduğunu ifade etmektedir. Aynı zamanda palyatif bakımın ne ölümü hızlandırmak ne de ertelemek niyetinde olmadığını vurgulamaktadır (1). Bu makalede değerlendirilen konu başlıklarının, bu çerçeve içinde ele alınması gerektiğini vurgulamak uygun olacaktır. Palyatif bakım eğitimlerinin önemli bir kısmını iletişim, etik, psikosoyal bakım ve kültür, dil, din ve maneviyat gibi konular oluşturmaktadır. Bu bağlamda palyatif bakım etiği, üzerinde durulması gereken özellikli bir konudur (2, 3). Palyatif Bakımın Hizmet SınırlarıPalyatif bakım genelde sağlık hizmetleri belirli bir düzeye ulaşmış ülkelerde verilen bir hizmet olarak görülmektedir. Düşük ekonomik geliri olan ülkelerde palyatif bakım masrafları sorun olmaktadır. Hekim olarak "öncelikle zarar verme" ilkesinden yola çıkarak hastanın ağrısını acını dindirmeye çalışmak gereklidir (4). Palyatif bakım sadece kanser hastalarına verilen bir hizmet değildir. Yaşamı tehdit eden her türlü hastalık için geçerlidir. Sadece hastayı değil hasta yakınını da kapsar.Palyatif bakımın hastaya, yaşamı tehdit eden hastalığın teşhisinden itibaren aktif tedavi ile birlikte verilmesi, hastanın ölümünden sonra da yakınlarının yas sürecinde desteklenmesi ile devam etmesi gerektiği ifade edilmektedir (5). Hawley (6) palyatif bakımın sonunda hastanın her zaman ölmeyebileceğini, bu durumda hastanın rehabilitasyon sürecinin devreye girdiğini belirtmektedir. Papyon modeli olarak önerilen bu bakım sisteminin (Şekil 1), ölümcül olsun olmasın ciddi bir hastalık tanısı alan hastanın palyatif bakım sürecini kabullenmesini kolaylaştırdığı düşünülmektedir (6). Introduction: Hospital Ethics Committees (HECs) were established to solve the ethical dilemmas experienced by health personnel in clinics. However, HECs also provide training on ethics, and develop ethics guidelines and institutional policies. HECs first appeared in the USA in the 1960s and had become widespread in Europe by the 1990s. In Turkey, the introduction of HECs is relatively recent and the number of committees is limited. The aim of this study was to identify the need for a HEC at the Hacettepe University Hospitals (HUHs) and to develop an appropriate model for the HEC. Methods:Both qualitative and quantitative research methods were used. As a part of the qualitative research, focus group interviews were conducted with physicians in the intensive care unit of the HUHs. Survey forms were prepared, with reference to the data, which were collected during these interviews. The quantitative research was conducted by distributing survey forms to all physicians working at the HUHs. Data were evaluated using SPSS v.11.5 software. Results: In total, 250 surveys were completed (participation ratio 25%). Physicians who participated in the research confirmed the need for an organization to provide training on ethics, to prepare ethics guidelines and to help solve ethical dilemmas. Conclusion: Here a model for a HEC is proposed, with reference to the needs and suggestions of the physicians who work at the HUHs. In developing countries, the patriarchal social construct, unsatisfactory legal protection, and underlying cultural bias against women have resulted in increasing intimate partner violence, which largely goes unaddressed, undocumented, and unreported in healthcare settings. This case study presents a common scenario of intimate partner violence and carefully analyzes its ethical implications in an emergency case at a tertiary care hospital in Karachi, Pakistan. In this study, basic ethical principles are used to discuss important ethical challenges faced daily by healthcare workers in the medical, social, and legal context. It further highlights the dire need of patient education, safety, and a solid framework for addressing, documenting, and reporting intimate partner violence for healthcare workers.
Objective: Film screenings are important tools of education in preparing students for their career and they found a place in the curriculum of the discipline of medical ethics, which deals mainly with the value-related problems that arise during medical practice. This study presents the viewpoints of the students of Hacettepe University, Faculty of Medicine on one of the most controversial issues in the field of medical ethics, euthanasia, by examining their ideas on the movie titled "You Don't Know Jack". The purpose of this research was to review the elective course "Representations of Physicians in Cinema" in terms of the competency areas proposed in the National Undergraduate Medical Core Curriculum in Turkey. Method: A total of 145 papers prepared by the students on the movie "You Don't Know Jack" was analyzed by using the thematic analysis method. An approval from the Hacettepe University Ethics Commission was obtained prior to the commencement of analysis (No, 3861; Date: 26.05.2017).
