Recent widespread application of hepatectomy for liver cancer is remarkable by applying techniques developed in vascular surgery. We experienced a case of solitary metastatic caudate lobe cancer which could be resected with combined partial resction of inferior vena cava using Bio-pump_??_. Primary hepatocellular carcinomas arising in the caudate lobe are rare, especially metastatic ones are, of course, so rare that only 2 cases have been reported.A 74-year-old male under postoperative follow-up for sigmoid colon cancer was indicated an elevated CEA level and diagnosed as solitary metastatic caudate lobe cancer. As the inferior vena cava was involved, venovenous bypass operation using Bio-pump_??_ was performed between right axillar vein and right major sapherous vein, followed by safe and sure caudate lobectomy combined with partial resection of inferior vena cava. The tumor was histologically confirmed as liver metastasis of sigmoid colon cancer.
