Sažetak. Cilj: Utvrditi stupanj suradljivosti pri higijeni ruku među zdravstvenim djelatnicima Odjela za intenzivno liječenje Klinike za anesteziologiju, reanimatologiju i intenzivno liječenje Kliničkog bolničkog centra Rijeka te steći uvid o praksi i kvaliteti izvođenja postupaka za higijenu ruku. Ispitanici i metode: Heterogena skupina zdravstvenih radnika, medicinskih sestara i liječnika čije je zajedničko obilježje mjesto rada. Izravno promatranje prema konceptu "Pet trenutaka za higijenu ruku" Svjetske zdravstvene organizacije provođeno je tijekom tri mjeseca, do postizanja najmanje 200 prilika, jedna opservacija u trajanju do dvadeset minuta. Bilježene su opažene indikacije, akcije i nošenje rukavica. Postotak suradljivosti predstavlja omjer opserviranih akcija (higijenskog pranja i higijenskog utrljavanja) i opserviranih prilika kada je bilo potrebno provesti higijenu ruku. Podaci su bilježeni kodovima, raspodijeljeni prema godinama praćenja od 2011. do 2016. godine. Rezultati: Ukupna suradljivost u higijeni ruku tijekom šestgodišnjeg praćenja u prosjeku iznosi 78 %. Higijensko pranje ruku koristi se češće (68 %) od higijenskog utrljavanja alkoholnog pripravka (32 %). Ukupni postotak suradljivosti prema indikacijama iznosi: prije kontakta s pacijentom 71 %, prije aseptičnih postupaka 79 %, nakon rizika izlaganju tjelesnim tekućinama 84 %, nakon kontakta s pacijentom 78 % te nakon napuštanja pacijentove okoline 76 %. Zaključci: Higijena ruku provodi se češće higijenskim pranjem ruku nego utrljavanjem alkoholnog pripravka. Potrebno je uložiti dodatne napore u promociju alkoholnog utrljavanja kao bržeg i boljeg načina održavanja higijene ruku. Potrebno je potaknuti veću suradljivost u svim indikacijama s ciljem postizanja suradljivosti od ≥ 90 %.
Transplantirani i ostali urološki pacijenti imaju češće potrebu za uvođenjem ureteralnog stenta, odnosno urinarnih katetera, te su izloženi povećanom riziku za stjecanje infekcija mokraćnog sustava (IMS). To je najčešća infektivna komplikacija u primatelja alogeničnog grafta, glavni izvor pijelonefritisa, bakterijemije i sepse. Također, IMS su najzastupljenije infekcije povezane sa zdravstvenom skrbi, a glavni rizični čimbenik za njihov nastanak je upravo prisustvo katetera. Vodeći uzročnici ovih infekcija su E. coli, E. faecalis, vrste iz roda Klebsiella, Proteus i Pseudomonas koje često karakterizira i višestruka antimikrobna rezistencija, što dodatno ugrožava klinički ishod. Radi praćenja IMS-a povezanih s uporabom katetera svake se godine provodi jednodnevno presječno istraživanje kojim su obuhvaćeni svi pacijenti hospitalizirani taj dan. Rezultati pridonose sagledavanju problema, unaprjeđenju mjera prevencije, planiranju empirijske antimikrobne terapije, odnosno sigurnosti hospitaliziranih uroloških pacijenata.
