We present an agent-based distributed decision support system for the diagnosis and prognosis of brain tumours developed by the HealthAgents project. HealthAgents is a European Union funded research project, which aims to enhance the classification of brain tumours using such a decision support system based on intelligent agents to securely connect a network of clinical centres. The HealthAgents system is implementing novel pattern recognition discrimination methods, in order to analyse in vivo Magnetic HealthAgents intends not only to apply forefront agent technology to the biomedical field, but also develop the HealthAgents network, a globally distributed information and knowledge repository for brain tumour diagnosis and prognosis.

Abstract.Quantitative magnetic resonance analysis often requires accurate, robust and reliable automatic extraction of anatomical structures. Recently, template-warping methods incorporating a label fusion strategy have demonstrated high accuracy in segmenting cerebral structures. In this study, we propose a novel patch-based method using expert segmentation priors to achieve this task. Inspired by recent work in image denoising, the proposed nonlocal patch-based label fusion produces accurate and robust segmentation. During our experiments, the hippocampi of 80 healthy subjects were segmented. The influence on segmentation accuracy of different parameters such as patch size or number of training subjects was also studied. Moreover, a comparison with an appearance-based method and a template-based method was carried out. The highest median kappa value obtained with the proposed method was 0.884, which is competitive compared with recently published methods.

Multisite and temporal variability in data distributions affects DQ, hindering data reuse, and an assessment of such variability should be a part of systematic DQ procedures.

Knowledge discovery on biomedical data can be based on on-line, data-stream analyses, or using retrospective, timestamped, off-line datasets. In both cases, changes in the processes that generate data or in their quality features through time may hinder either the knowledge discovery process or the generalization of past knowledge. These problems can be seen as a lack of data temporal stability. This work establishes the temporal stability as a data quality dimension and proposes new methods for its assessment based on a probabilistic framework. Concretely, methods are proposed for 1) monitoring changes, and 2) characterizing changes, trends and detecting temporal subgroups. First, a probabilistic change detection algorithm is proposed based on the Statistical Process Control of the posterior Beta distribution of the Jensen-Shannon distance, with a memoryless forgetting mechanism. This algorithm (PDF-SPC) classifies the degree of current change in three states: In-Control, Warning, and Out-of-Control. Second, a novel method is proposed to visualize and characterize the temporal changes of data based on the projection of a non-parametric information-geometric statistical manifold of time windows. This projection facilitates the exploration of temporal trends using the proposed IGT-plot and, by means of unsupervised learning methods, discovering conceptually-related temporal subgroups. Methods are evaluated using real and simulated data based on the National Hospital Discharge Survey (NHDS) dataset.

New biomedical technologies enable the diagnosis of brain tumours by using non-invasive methods. HealthAgents is a European Union-funded research project that aims to build an agent-based distributed decision support system (dDSS) for the diagnosis of brain tumours. This is achieved using the latest biomedical knowledge, information and communication technologies and pattern recognition (PR) techniques. As part of the PR development of HealthAgents, an independent and automatic classification framework (CF) has been developed. This framework has been integrated with the HealthAgents dDSS using the HealthAgents agent platform. The system offers (1) the functionality to search for distributed classifiers to solve specific questions; (2) automatic classification of new cases; (3) instant deployment of new validated classifiers; and (4) the ability to rank a set of classifiers according to their performance and suitability for the case in hand. The CF enables both the deployment of new classifiers using the provided Extensible Markup Language 1 classifier specification, and the inclusion of new PR techniques that make the system extensible. These features may enable the rapid integration of PR laboratory results into industrial or research applications, such as the HealthAgents dDSS. Two classification nodes have been deployed and they currently offer classification services by means of dedicated servers connected to the HealthAgents agent platform: one node being located at the Katholieke Universiteit Leuven, Belgium and the other at the Universidad Polite´cnica de Valencia, Spain. These classification nodes share the current set of brain tumour classifiers that have been trained from in vivo magnetic resonance spectroscopy data. The combination of the CF with a distributed agent system constitutes the basis of the brain tumour dDSS developed in HealthAgents.

Clinical Information Models (CIMs) expressed as archetypes play an essential role in the design and development of current Electronic Health Record (EHR) information structures. Although there exist many experiences about using archetypes in the literature, a comprehensive and formal methodology for archetype modeling does not exist. Having a modeling methodology is essential to develop quality archetypes, in order to guide the development of EHR systems and to allow the semantic interoperability of health data. In this work, an archetype modeling methodology is proposed. This paper describes its phases, the inputs and outputs of each phase, and the involved participants and tools. It also includes the description of the possible strategies to organize the modeling process. The proposed methodology is inspired by existing best practices of CIMs, software and ontology development. The methodology has been applied and evaluated in regional and national EHR projects. The application of the methodology provided useful feedback and improvements, and confirmed its advantages. The conclusion of this work is that having a formal methodology for archetype development facilitates the definition and adoption of interoperable archetypes, improves their quality, and facilitates their reuse among different information systems and EHR projects. Moreover, the proposed methodology can be also a reference for CIMs development using any other formalism.

Abstract. Clinical guidelines contain recommendations based on the best empirical evidence available at the moment. There is a wide consensus about the benefits of guidelines and about the fact that they should be deployed through clinical information systems, making them available during consultation time. However, one of the main obstacles to this integration is still the interaction with the electronic health record. In this paper we present an archetype-based approach to solve the interoperability problems of guideline systems, as well as to enable guideline sharing. We also describe the knowledge requirements for the development of archetype-enabled guideline systems, and then focus on the development of appropriate guideline archetypes and on the connection of these archetypes to the target electronic health record.
