El SIC es una reducción congénita o adquirida por resección quirúrgica en la longitud del ID ocasionando malabsorción. Las causas más comunes son atresia intestinal, enfermedad de Hirschsprung, gastrosquisis, enterocolitis necrotizante, vólvulo, trombosis vascular, tumor, traumatismo y síndrome de intestino inflamatorio. Su fisiopatología, presentación clínica, manejo y pronóstico son influenciados por determinantes específicos. Requiere de un proceso de adaptación intestinal para recuperar funcionalidad. Presenta múltiples complicaciones y el manejo se basa en programas de rehabilitación intestinal, NP, NE, suplementación de déficits nutricionales, terapia farmacológica y procedimientos quirúrgicos, con lo cual su pronóstico y prevalencia han aumentado en las últimas décadas. Sin embargo, persiste con una alta morbimortalidad.
La bronquiolitis aguda por el Virus Respiratorio Sincitial por lo general es una enfermedad auto limitada del tracto respiratorio inferior. Suele manifestarse inicialmente por síntomas respiratorios superiores, fiebre y posteriormente, síntomas respiratorios inferiores. Tiene una alta prevalencia mundial, con brotes epidemiológicos anuales según la zona geográfica. Se presenta sobre todo en niños menores 2 años y puede ocasionar infecciones repetidas a lo largo de la vida, ya que parece no otorgar inmunidad protectora. Existen factores de riesgo predisponentes para mayor severidad de la enfermedad. La evaluación se realiza principalmente por la historia clínica y examen físico, aunque existen ciertos estudios complementarios que se pueden realizar. El tratamiento es principalmente de soporte, no existe mayor evidencia científica para un tratamiento específico. Existe la profilaxis con Palivizumab, esta se reserva para casos específicos. Hay vacunas en etapa de desarrollo. El pronóstico suele ser bueno, aunque puede asociarse a morbilidad y mortalidad.
Fibromialgia (FM) se define como dolor musculoesquelético generalizado crónico usualmente acompañado por fatiga y alteraciones del sueño por mínimo 3 meses que no se explican por otra condición médica. Es más común en mujeres adultas, pero se puede dar desde niños a adultos y hombres. La prevalencia es de 2.7% en la población general. La etiología en sí es desconocida, pero se clasifica como una sensibilización central por cambios neuroendocrinológicos en sistema nervioso central y periférico en individuos genéticamente susceptibles, en respuesta a desencadenantes ambientales, resultando en percepción del dolor aumentada e hipersensibilidad a numerosos estímulos. Se caracteriza por dolor musculoesquelético generalizado y sensibilidad de tejidos blandos, acompañado por fatiga, alteraciones del sueño, trastornos somáticos, cognitivos y psiquiátricos. Pueden existir trastornos coexistentes que estimulan o exacerban la sintomatología. El diagnóstico se basa en la historia clínica y síntomas, examen físico normal, estudios complementarios normales y criterios AAPT o 2010 CAR o de Yunus y Masi positivos. Se deben valorar posibles diagnósticos diferenciales según el grupo etáreo. El manejo se basa en educación, identificación de comorbilidades, ejercicio, terapia cognitiva conductual, fármacos como antidepresivos y anticonvulsivantes y terapias alternativas. Los pacientes tienen un deterioro en la calidad de vida y morbilidad, sin un aumento significativo en la mortalidad.
La lactancia materna (LM) es el método preferido y es la fuente nutricional óptima. Se recomienda LM exclusiva los primeros 6 meses, posteriormente acompañada por alimentación complementaria por lo menos hasta los primeros 2 años. Existen factores parentales que influyen en el inicio y en el mantenimiento. La leche materna es rica nutricionalmente, tiene propiedades inmunológicas, antimicrobianas y de desarrollo. La iniciación y mantenimiento se ven influenciados por el ambiente hospitalario, social, cultural y familiar. Existen 10 pasos recomendados para que un hospital sea promotor. Se debe dar a demanda y se debe evaluar la ingesta adecuada. Existen múltiples beneficios a corto y largo plazo asociados para la salud del niño y la madre, la economía, sociedad y los sistemas de salud. Puede prevenir el riesgo de enfermedades agudas y crónicas. La LM también tiene influencia sobre la madre y ocasiona un incremento en los requerimientos nutricionales. Existen problemas comunes como ingesta inadecuada, dolor, infecciones, secreción, sobreproducción e ictericia neonatal que pueden derivar de la LM, pero con manejo adecuado pueden resolverse y no ser impedimento. Se deben valorar las precauciones y contraindicaciones para LM en cada paciente, como infección materna, madres VIH positivas, uso de sustancias maternas y enfermedades infantiles. Se debe promocionar y apoyar la LM, educando sobre la manera adecuada para hacerlo, sus beneficios y evacuando dudas, aprovechando cada consulta.
