This study seeks to estimate the status of serum 25-hydroxyvitamin D [25(OH)D] in a healthy section of the population in Tunisia and to compare the achieved results with other published data. A transverse descriptive inquiry was carried out between January and March 2002. Three hundred and eighty-nine subjects aged 20-60 years were included in the study. A questionnaire was used to investigate clinical characteristics: sunlight exposure to ultraviolet light and, for women, parity, breast feeding, whether or not they wore the veil, and menopause. A dietary investigation estimated calcium and vitamin D intake. Morning fasting blood was collected from each subject for the measurement of the following parameters: serum calcium, phosphorus, albumin, alkaline phosphatase, 25(OH)D, parathyroid hormone (PTH). Hypovitaminosis D was defined by a cut-off of 37.5 nmol/l. The population studied was largely female, for the greater part housewives. The accumulated prevalence of hypovitaminosis D was 47.6%, increasing with age. Hypovitaminosis is highly prevalent in women (P<0.001). Multiparity, menopause, wearing the veil, and calcium and vitamin D dietary intake are factors associated with hypovitaminosis D (P<0.05). Analysis of logistic regression shows that only multiparity and vitamin D dietary intake are independent predictive factors. The status of serum 25(OH)D in Tunisia resembles that in southern European countries rather than in Middle Eastern countries. Insufficient vitamin D diet intake, higher parity and wearing the veil explain this deficiency in Tunisia. Dietary enrichment or supplementation by vitamin D and a more outdoor lifestyle, especially for older people, should be seriously considered as a way to reduce this deficiency in Tunisia.

Ataxia with vitamin E deficiency is an autosomal recessive cerebellar ataxia caused by mutations in the a-tocopherol transfer protein coding gene localized on chromosome 8q, leading to lower levels of serum vitamin E. More than 91 patients diagnosed with ataxia with vitamin E deficiency have been reported worldwide. The majority of cases originated in the Mediterranean region, and the 744delA was the most common mutation among the 22 mutants previously described. We examined the clinical and molecular features of a large cohort of 132 Tunisian patients affected with ataxia with vitamin E deficiency. Of these patients, nerve conduction studies were performed on 45, and nerve biopsy was performed on 13. Serum vitamin E was dramatically reduced for 105 of the patients analysed. Molecular analysis revealed that 91.7% of the patients (n = 121) were homozygous for the 744delA mutation. Three other mutations were detected among the remaining patients (8.3%, n = 11) in the homozygous state. Two were previously reported (400C4T and 205-1G4T), and one was novel (553 + 1T4A). Age of onset was 13.2 AE 5.9 years, with extremes of 2 and 37 years. All described patients exhibited persistent progressive cerebellar ataxia with generally absent tendon reflexes. Deep sensory disturbances, pyramidal syndrome and skeletal deformities were frequent. Head tremor was present in 40% of the patients. Absence of neuropathy or mild peripheral neuropathy was noted in more than half of the cohort. This is the largest study of the genetic, clinical and peripheral neuropathic characteristics in patients with ataxia and vitamin E deficiency. The 744delA mutation represents the most common pathological mutation in Tunisia and worldwide, likely because of a Mediterranean founder effect. Our study led us to suggest that any patient displaying an autosomal recessive cerebellar ataxia phenotype with absent tendon reflexes and minor nerve abnormalities should first be screened for the 744delA mutation, even in the absence of a serum vitamin E measurement.

Decreased HDLc is the main biological abnormality that characterized MetS in Tunisian psoriatic patients. Moreover, contrary to men, psoriatic women have shown a significantly higher prevalence of MetS, which is, in addition to decreased HDLc, mainly attributed to abdominal obesity.

Fecal calprotectin seems to be a reliable marker of relapse in quiescent Crohn's disease patients.

To examine the effects of short high-intensity interval training (HIIT) on body composition, physical performance and plasma lipids in overweight/obese compared to normal-weight young men. Nine overweight/obese and nine normal-weight men (control group) aged 17 to 20 years underwent a HIIT programme three times per week for eight weeks. Body composition, indices of aerobic [maximal aerobic velocity (MAV) and maximal oxygen uptake (VO2max)] and anaerobic [squat jump (SJ), counter-movement jump (CMJ), five-jump test (FJT), 10-m and 30-m sprint] performances, as well as fasting plasma lipids, were assessed in the two groups at PRE and POST HIIT. The HIIT programme resulted in significant reductions in body mass (-1.62%, P=0.016, ES=0.11) and fat mass (-1.59%, P=0.021, ES=0.23) in obese, but not in normal-weight subjects. MAV (+5.55%, P=0.005, ES=0.60 and +2.96%, P=0.009, ES=0.82), VO2max (+5.27%, P=0.006, ES=0.63 and +2.88%, P=0.009, ES=0.41), FJT (+3.63%, P=0.005, ES=0.28 and +2.94%, P=0.009, ES=0.52), SJ (+4.92%, P=0.009, ES=0.25 and +6.94%, P=0.009, ES=0.70) and CMJ (+6.84%, P=0.014, ES=0.30 and +6.69%, P=0.002, ES=0.64) significantly increased in overweight/obese and normal-weight groups, respectively. 30-m sprint time significantly decreased in both groups (-1.77%, P=0.038, ES=0.12 and -0.72%, P=0.030, ES=0.16). Plasma total cholesterol (-11.8%, P=0.026, ES=0.96), LDL cholesterol (-11.9%, P=0.050, ES=0.77) and triglycerides (-21.3%, P=0.023, ES=1.08) significantly decreased in the obese group, but not in the normal-weight group. The eight-week HIIT programme resulted in a slight improvement in physical fitness and a significant decrease in plasma lipids in the obese. Short duration HIIT may contribute to an improved cardiometabolic profile in the obese.
