La intoxicación crónica por mercurio es una entidad poco diagnosticada en la práctica clínica. Lo anterior no se debe exclusivamente a la baja incidencia de casos, sino también a un difícil diagnóstico asertivo y oportuno al enfrentarse a la amplia sintomatología inespecífica y multisistémica presentada con la intoxicación de este metal. Los vacíos de conocimiento y la falta de indagación en la historia clínica inicial sobre factores de riesgo provocan la no inclusión dentro de los posibles diagnósticos diferenciales. Se expone el caso de un paciente joven vecino de una zona rural con una sintomatología crónica poco específica, primordialmente neurológica y con estudios complementarios iniciales poco reveladores, con la posterior indagación por una intoxicación con metales dentro de los posibles diagnósticos, revelando cantidades sumamente elevadas de mercurio en orina.
