Background: Despite consistent pharmacogenetic effects of CYP2D6 on tamoxifen exposure, there is considerable controversy regarding the validity of CYP2D6 as a predictor of tamoxifen outcome. Understanding the current state of evidence in this area and its limitations is important for the care of patients who require endocrine therapy for breast cancer. Materials and Methods: A total of 101 patients with breast cancer who received tamoxifen therapy for at least 3 years, were genotyped for common alleles of the CYP2D6 gene by nested-PCR and restriction fragment length polymorphism PCR. Patients were classified as extensive or poor metabolizers (PM) based on CYP2D6*4 alleles in 3 different groups according to the menopause, Her2-neu status, and stage 3. Results: The mean age of the patients with the disease recurrence was 50.8±6.4 and in non recurrent patients was 48.2±6.8. In this study 63.3% (n=64) patients were extensive metabolizers and 36.6% (n=37) were poor metabolizers. Sixty four of the 101 patients (63.3%) were Her2-neu positive. For tamoxifentreated patients, no statistically significant difference in rate of recurrence observed between CYP2D6 metabolic variants in stage 3 and post-menopausal patients. However, there was a significant association between CYP2D6 genotype and recurrence in tamoxifen-treated Her2-neu positive patients. Compared with other women with breast cancer, those with Her2-neu positive breast cancer and extensive metabolizer alleles had a decreased likelihood of recurrence. Conclusions: This study for the first time demonstrated significant effects of CYP2D6 extensive metabolizer alleles on risk of recurrence in Her2-neu positive breast cancer patients receiving adjuvant tamoxifen therapy. Therefore, CYP2D6 metabolism, as measured by genetic variation, can be a predictor of breast cancer outcome in Her2-neu positive women receiving tamoxifen.

Objective: Many published data on the association between p53 codon 72 G>C polymorphism and thyroid carcinoma risk showed inconclusive results. The aim this study was to assess the association between p53 codon 72 G>C polymorphism and thyroid cancer risk. Methods: A literature search of PubMed, EMBASE, Google scholar and Web of Science databases for case–control studies examining the association between p53 codon 72 G>C polymorphism and thyroid cancer susceptibility up October 2016 was performed. Odds ratios (OR) with 95 % confidence intervals (95 % CI) were used to assess the strength of the association. Results: A total of 12 case–control studies involving 2,062 thyroid cancer patients and 3,027 controls were included. There was a significant association between the p53 codon 72 G>C polymorphism and thyroid cancer susceptibility in the overall populations under homozygote (CC vs. GG: OR = 1.18, 95% CI 1.12-3.05, P = 0.01) and recessive model (CC vs. GC+GG: OR = 1.73, 95% CI 1.16-2.59, P = 0.007). Subgroup analysis by ethnicity showed that there was no significant association between p53 codon 72 G>C polymorphism and thyroid cancer risk in Caucasians, Asians and mixed Brazilian. No significant publication bias was observed by using Begg’s funnel plot and Egger’s test. Conclusion: Present meta-analysis indicated that the p53 codon 72 G>C polymorphism may be associated with thyroid cancer risk. However, more studies with large sample size are needed to further assess the associations.

Introduction: ESR (Erythrocyte Sedimentation Rate) is the sedimentation rate of red blood cells per unit of time that is expressed in millimeters. ESR test is simple and inexpensive and fast and can be somewhat diagnostic manual to confirm the diagnosis, rule out the diagnosis, follow-up course of the disease and so on, so it is of interest to practitioners.Methods: Descriptive and cross-sectional study on 124 patients in all wards of Shahid Sadooghi  hospital in the second 6 months of 1393, that had raised ESR of 100 at least one time. All information including gender, age, diagnosis of the presence or absence of fever and indigenous of them cases in the archive hospital patients were taken.Results: 56% female and 44% were male. 16% of patients in the age range 0-19 years, 56% aged 20-64 years and 28% were aged over 64 years. 41% of patients were admitted with fever. 59% were native and 41 % were non-native. Infectious causes (45.9%), malignancy (14.5%), collagen vascular disease (16.9%), internal disease and hematology (16.9%) and other causes (5.6%) were formed cases. Diabetic foot (7%), pneumonia (7%), RA (6%), ESRD (5.6%), metastatic lung cancer (4%), TB (3%) formed the majority of cases.In feverish cases pneumonia (11.5%), diabetic foot (7.6%), tuberculosis (5.7%), pyelonephritis (5.7%) and RA (5.7%) were the most frequent form. Infectious causes of fever (65.3%), collagen vascular (13.4%), malignancies (5.7%) and other causes (15.6%).conclusion: In patients with extremely high sediment in terms of gender segregation is almost the same of hospital commiunity.
