Background: Submandibular abscess is one of the most common deep neck abscesses and associated with fatal complications which require adequate management. Diabetes mellitus (DM) is a chronic and complex disease that necessitates ongoing medical care with a multifactorial risk reduction strategy in addition to blood sugar control. The neutrophil lymphocyte ratio (NLR) is a simple marker for the systemic inflammatory conditions. An increased NLR values were found in patients with submandibular abscesses and patients with DM. Purpose: To determine the correlation between NLR to the therapeutic response of patients with submandibular abscesses with and without DM. Method: A cross-sectional study design with secondary data collection from medical records of submandibular abscess patients with and without DM. NLR value was compared between two groups. Correlation between NLR value and therapeutic response was compared between DM and non DM group. Result: The mean NLR of patients with and without DM were respectively 7.65 ± 4.92 and 16.53 ± 11. There was no significant difference between NLR and good therapeutic response or not in submandibular abscess patients with and without DM (p > 0.05). Conclusion: NLR of submandibular abscess patients with DM was lower than without DM. A lower NLR in submandibular abscess patients with DM did not have better response to therapy. Lower LNR in submandibular abscess patients without DM had a better therapeutic response, but it was not statistically significant.Keywords: neutrophil lymphocyte ratio, response to therapy, submandibular abscess, diabetes mellitusABSTRAKLatar belakang: Abses submandibula merupakan salah satu abses leher dalam yang paling sering ditemui dan berkaitan dengan komplikasi yang fatal sehingga diperlukan penatalaksanaan yang memadai pada pasien. Diabetes mellitus (DM) merupakan penyakit kronis dan kompleks, yang memerlukan perawatan medis berkelanjutan dengan strategi mengurangi risiko multifaktorial selain kontrol gula darah. Rasio neutrofil limfosit (RNL) merupakan penanda sederhana adanya kondisi inflamasi sistemik. Peningkatan nilai RNL ditemukan pada pasien dengan abses submandibula dan pasien DM. Tujuan: Mengetahui hubungan RNL terhadap respon terapi penderita abses submandibula dengan DM dan tanpa DM. Metode: Penelitian cross-sectional, sampel penelitian dari arsip rekam medis rawat inap pasien dengan diagnosis abses submandibula baik dengan komorbid DM dan tanpa komorbid DM. Nilai RNL dibandingkan diantara kedua kelompok. Hubungan nilai RNL dan respon terapi dibandingkan diantara kelompok DM dan tanpa DM. Hasil: Rerata nilai RNL pada pasien abses submandibula dengan komorbid DM sebesar 7,65 + 4,92 dan pada pasien tanpa komorbid DM sebesar 16,53 + 11. Tidak ada perbedaan yang signifikan antara nilai RNL terhadap respon terapi baik dan tidak baik pada pasien abses submandibula dengan komorbid DM dan tanpa DM (p>0,05). Kesimpulan: RNL penderita abses submandibula dengan DM lebih rendah daripada penderita tanpa DM. RNL yang lebih rendah tidak memiliki respon terapi yang lebih baik pada penderita abses submandibula dengan DM. RNL yang lebih rendah memiliki respon terapi yang lebih baik pada penderita abses submandibula tanpa DM, namun tidak bermakna secara statisitik.Kata kunci: rasio neutrofil limfosit, respon terapi, abses submandibula, diabetes mellitus
Myiasis adalah infestasi larva Diptera (lalat) pada jaringan hidup manusia atau hewan dalam periode tertentu. Kasus myiasis banyak terjadi didaerah tropis terutama pada masyarakat golongan sosio-ekonomi rendah. Myiasis pada trakeostomi jarang didapatkan, literatur di Inggris menyebutkan hanya 2 kasus myiasis pada trekeostomi. Dilaporkan kasus laki-laki 60 tahun dengan myiasis pada peristoma trakeostomi dan karsinoma laring T4N2cMo. Trakeostomi dilakukan 1 tahun yang lalu, datang dengan keluhan keluar belatung dari kanul trakea, terasa seperti ada benda asing dileher, dan rasa nyeri yang menggigit, Pada kassa kanul trakea  sering merembes darah,  dan kanul trakea tidak ditutup oleh kassa atau sapu tangan. Pasien menyatakan sering membersihkan sekitar kanul trakea menggunakan bulu ayam.  Penatalaksanaan dilakukan dengan ekstraksi larva secara manual dan debridemen, serta diberikan antibiotik intravena. Selain itu juga dilakukan perawatan luka dan penggantian anak kanul secara berkala dan  menutup kanul trakea dengan kassa. Larva teridenfikasi sebagai larva lalat Chrysomya sp. Myiasis pada trakeostomi jarang diterjadi, faktor predisposisi myiasis pada luka trakeostomi berupa kebersihan kanul trakea, bau dari luka trakeostomi, dan kebersihan lingkungan tempat tinggal.
Latar belakang: Penyakit rinitis alergi (RA) banyak dijumpai dan memberi dampak negatif terhadap produktivitas, kualitas hidup, gangguan tidur, aktivitas di luar rumah, serta gangguan sekolah pada anak. Vitamin E bekerja sebagai antioksidan pada membran sel dan berikatan dengan radikal bebas dengan cara menghambat peroksidasi Poly Unsaturated Fatty Acid (PUFA). Tujuan: Mengetahui pengaruh vitamin E terhadap jumlah eosinofil mukosa hidung dan perbaikan klinis pada penderita rinitis alergi persisten sedang berat. Metode: Menggunakan Randomized Clinical Trial, double blind, pre and post control group dengan perlakuan pemberian terapi semprot hidung triamcinolone acetonide, cetirizine 10 mg, dan vitamin E 400IU selama 4 minggu. Dilakukan penilaian Visual Analogue Scale (VAS) dan Total Nasal Symptom Score (TNSS), serta pemeriksaan eosinofil mukosa hidung pada awal dan 4 minggu setelah perlakuan. Hasil: Didapatkan perbedaan bermakna antara jumlah eosinofil mukosa hidung sebelum dan sesudah pemberian vitamin E pada kelompok perlakuan (p=<0,001). Penurunan jumlah eosinofil mukosa hidung lebih besar dan bermakna pada kelompok perlakuan dibandingkan dengan kelompok kontrol (p=<0,001). Didapat perbaikan kualitas hidup berdasarkan perbaikan nilai VAS dan TNSS yang bermakna setelah perlakuan pada kelompok kontrol maupun perlakuan (p=<0,001), serta didapatkan perbaikan kualitas hidup berdasarkan nilai VAS dan TNSS pada kedua kelompok dengan selisih nilai perbaikan gejala yang lebih besar pada kelompok perlakuan, namun dari perhitungan statistik tidak didapatkan perbedaan yang bermakna. Kesimpulan: Vitamin E 400IU menurunkan jumlah eosinofil mukosa hidung secara bermakna pada pasien rinitis alergi persisten sedang berat. Perbaikan nilai TNSS pada penggunaan vitamin E 400IU lebih baik dibandingkan subjek yang mendapat plasebo, walaupun tidak berbeda secara bermakna.Kata kunci: rinitis alergi, Vitamin E, Total Nasal Symptom Score ABSTRACT Background: Allergic rhinitis (AR) is an allergic disease frequently found and has a negative impact on productivity, quality of life, sleep disorder, outdoors activities, and school disruption in children. Vitamin E works as an antioxidant in cell membranes through binding free radicals by inhibiting peroxidation of Poly Unsaturated Fatty Acids (PUFA). Purpose: To find out the effect of vitamin E administration on nasal mucosa eosinophil counts and the clinical improvement in patients with moderate severe persistent AR. Methods: A double blind randomized clinical trial, pre and post control group, given therapy of triamcinolone acetonide nasal spray, cetirizine 10 mg, and vitamin E 400 IU orally for 4 weeks. Evaluation by Visual Analogue Scale (VAS) and Total Nasal Symptom Score (TNSS), as well as nasal mucosal eosinophils count at baseline and 4 weeks after treatment. Results: There was a significant difference on the number of nasal mucosal eosinophils before and after vitamin E administration in the treatment group (p = <0.001). A greater and more significant decrease in the number of nasal mucosal eosinophils in the treatment group compared to control group (p = <0.001). Significant improvement of VAS and TNSS after treatment in the control and treatment groups (p = <0.001) and quality of life improvement based on VAS and TNSS values in both control and treatment groups, but the statistical calculations showed no significant differences between the two groups. Conclusion: Vitamin E 400IU significantly reduce the number of nasal mucosal eosinophils in patients with moderate severe persistent AR.
Hiperplasia pseudokarsinomatus merupakan proliferasi epitel reaktif jinak yang secara histopatologi mirip karsinoma sel skuamosa. Salah satu penyebabnya adalah infeksi jamur. Kami melaporkan 1 kasus hiperplasia pseudokarsinomatus hipofaring oleh karena sporotrikosis. Wanita 57 tahun mengeluh tenggorok terasa mengganjal disertai nyeri ulu hati dan sensasi pahit/kecut naik ke tenggorok. Pasien menderita refluks laringofaringeal,alergi seafood, dan riwayat Steven Johnson Syndrome. Pada pemeriksaan laringoskopi, tampak massa berdungkul pada hipofaring dengan kesan jinak. Dari hasil pemeriksaan histopatologi tampak infiltrasi epitel menuju dermis (mirip karsinoma sel skuamosa). Dengan pemeriksaan ulang secara patologi anatomi dan mikrobiologi (baku emas) serta komunikasi antara klinisi, ahli patologi, dan mikrobiologi, massa tersebut diidentifikasi sebagai hiperplasia pseudokarsinomatus oleh karena Sporothrix schenckii. Pasien menjalani eksisi massa dan diberikan ketokonazol dan lanzoprazol selama 6 minggu. Saat evaluasi ulang, pasien merasa rasa mengganjal hilang dan tidak ditemukan massa pada hipofaring. Hiperplasia pseudokarsinomatus hipofaring oleh karena sporotrikosis jarang terjadi. Di indonesia, belum ada laporan kasus mengenai hal ini. Kesalahan diagnosis sebagai karsinoma dapat berakibat fatal. Akan tetapi, dengan diagnosis yang lebih teliti dan tatalaksana yang tepat, prognosis pasien sangat baik.
