ABSTRAKNyeri adalah pengalaman sensorik dan emosional yang tidak menyenangkan akibat kerusakan jaringan, baik aktual maupun potensial atau yang digambarkan dalam bentuk kerusakan tersebut.Mekanisme timbulnya nyeri didasari oleh proses multipel yaitu nosisepsi, sensitisasi perifer, perubahan fenotip, sensitisasi sentral, eksitabilitas ektopik, reorganisasi struktural, dan penurunan inhibisi. Antara stimulus cedera jaringan dan pengalaman subjektif nyeri terdapat empat proses tersendiri : tranduksi, transmisi, modulasi, dan persepsi.Rangsang nyeri diterima oleh nosiseptor di kulit dan visera. Sel yang nekrotik akan melepaskan K + dan protein intrasel yang dapat mengakibatkan inflamasi. Mediator penyebab nyeri akan dilepaskan. Leukotrien, prostatglandin E 2 , dan histamine akan mensensitisasi nosiseptor selain itu lesi jaringan juga mengaktifkan pembekuan darah sehingga melepaskan bradikinin dan serotonin. Jika terdapat penyumbatan pembuluh darah, akan terjadi iskemia dan penimbunan K + dan H + ekstrasel yang diakibatkan akan semakin mengaktifkan nosiseptor yang telah tersensitasi. Perangsangan nosiseptor melepaskan substansi peptide P (SP) dan peptide yang berhubungan dengan gen kalsitonin (CGRP), yang meningkatkan respon inflamasi dan menyebabkan vasodilatasi serta meningkatkan permeabilitas vaskular. Between stimulus and the subjective experience of tissue injury pain, there were four separate processes: tranduksi, transmission, modulation, and perception.Nosiseptor pain stimuli received by the skin and viscera. Necrotic cells that will release K + and intracellular proteins that can cause inflammation. Mediators cause pain to be released. Leukotrienes, prostatglandin E2, and histamine will sensitization nosiseptor besides tissue lesions also activate blood clotting thus releasing bradykinin and serotonin. If there was a blockage of blood vessels, ischemia will occur and the accumulation of K + and H + was caused by extracellular will increasingly enable nosiseptor have sensitization. Stimulation nosiseptor releasing peptide substance P (SP) and peptide calcitonin gene-related (CGRP), which increases the inflammatory response and cause vasodilation and increased vascular permeability.
Carpal tunnel syndrome (CTS) adalah salah satu gangguan pada tangan karena terjadi penyempitan pada terowongan karpal, baik akibat edema fasia pada terowongan tersebut maupun akibat kelainan pada tulang-tulang kecil tangan sehingga terjadi penekanan terhadap nervus medianus dipergelangan tangan. National Health Interview Study (NIHS) memperkirakan bahwa prevalensi CTS yang dilaporkan sendiri diantara populasi dewasa adalah sebesar 1.55% (2,6 juta). Kejadian CTS pada populasi diperikrakan3% pada wanita dan 2% pada laki-laki dengan prevalensi tertinggi pada wanita tua usia > 55 tahun, biasanya antara 40 – 60 tahun. Penyebab CTS diduga oleh karena trauma, infeksi, gangguan endokrin, dan lain-lain, tetapi sebagian tidak diketahui penyebabnya. Penggunaan tangan yang berlebihan dan repetitif diduga berhubungan dengan sindroma ini. CTS bisa mengenai usia pertengahan, wanita lebih sering dari pada pria, biasanya pada tangan yang dominan dan prevalensi meningkat pada kehamilan. Pada tahap awal hanya gangguan sensorik, berupa parestesia, kurang merasa (numbness) atau rasa seperti terkena aliran listrik (tingling) pada jari dan setengah sisi radial jari, walaupun kadang-kadang dirasakan mengenai seluruh jari-jari. Bila penyakit berlanjut rasa nyeri dapat bertambah berat dengan frekuensi serangan yang semakin sering bahkan dapat menetap. Kadang-kadang nyeri dapat terasa sampai kelengan atas dan leher, sedangkan parestesia umumnya terbatas di daerah distal pergelangan tangan. Keluhan dirasakan terutama malam hari. Dapat pula dijumpai pembengkakan dan kekakuan pada jari-jari tangan dan pergalangan tangan terutama di pagi hari. Lebih lanjut lagi penderita mengeluh jari-jarinya menjadi kurang terampil misalnya saat memungut benda-benda kecil. Pada pemeriksaan fisik didapatkan Phalen’s test dan Tinel’s sign yang positif dan pada Pemeriksaan EMG dapat menunjukkan adanya fibrilasi, polifasik, gelombang positif dan berkurangnya jumlah motor unit pada otot-otot thenar. Penanganan faktor resiko akan memperbaiki gejala, penggunaan obat anti inflamasi untuk artritis tangan, mengurangi penggunaaan tangan yang berulang, mengistirahatkan pergelangan tangan, Pemasangan bidai pada posisi netral pada pergelangan tangan akan memperbaiki gejala. Pemberian obat anti inflamasi non steroid dan injeksi steroid dengan lidocain dan long acting steroid pada terowongan karpal akan mengurangi keluhan. Bila terapi konservatif gagal dilakukan tidakan operasi sebagi pilihan terakhir.
Dehidrasi adalah kondisi kekurangan atau kehilangan cairan dari seluruh kompartemen tubuh. Kekurangan cairan minimal 1% dapat mempengaruhi fungsi kognitif otak termasuk penurunan kemampuan daya ingat jangka pendek. Penelitian ini untuk mengetahui hubungan dehidrasi terhadap memori segera/atensi. Rancangan penelitian dengan True Experimental, Pre test and post Test with Control Group Design dengan dua kelompok, kontrol dan kelompok perlakuan (induksi dehidrasi ringan dengan treadmil 5,6 km/jam selama 40 menit, kemudian istirahat 20 menit). Instrumen yang digunakan adalah WAIS Digit Span Backward. Analisa data menggunakan Uji Chi-Square, Mann-Whitney Test, Nonparametric Correlations Spearman’s dan uji Regression. Dari penelitian ini didapatkan hasil prosentase penurunan memori segera/atensi pada kelompok tidak dehidrasi 16,1%, kelompok dehidrasi.adalah 30,6% dan Pada Uji Chi-Square signifikansi P= 0,02. uji NP Nonparametric Correlations Spearman’s signifikansi P=0.004 artinya ada korelasi antara beratnya dehidrasi dengan penurunan memori segera/Atensi dan pada uji regressi didapat korelasi negatif artinya semakin besar nilai dehidrasi maka semakin berat derajat penurunan memori segera/Atensi. Dari penelitian ini didapatkan hubungan dehidrasi terhadap memori segera/atensi dengan korelasi negatif.
ABSTRAKLatar belakang: CTS merupakan penyakit yang sering dijumpai pada pekerja industri dalam setiap kasus penyakit akibat kerja di beberapa negara. Terdapat beberapa faktor yang dapat mempengaruhi angka kejadian CTS, salah satunya yaitu masa kerjaTujuan penelitian: Mengetahui hubungan masa kerja terhadap kejadian CTS pada pekerja pemetik daun teh.Metode penelitian: Analitik observasional dengan pendekatan Cross Sectional. Teknik pengambilan sampel menggunakan metode consecutive sampling dan didapatkan sampel sebanyak 85 pekerja.Hasil penelitian dan Diskusi: Angka kejadian CTS pada pekerja pemetik daun teh adalah 56 ( 65,9%), kejadian CTS meningkat setelah umur 40 tahun dan kejadian CTS tertinggi pada umur 50-59 tahun (30,6%), kejadian CTS meningkat setelah masa kerja 30 tahun dan kejadian CTS tertinggi terjadi pada masa kerja 30-39 tahun (31,8%). Korelasi antara masa kerja dengan kejadian CTS menunjukan nilai korelasi positif (0,263) dengan nilai signifikansi 0,015 (<0,05),yang artinya semakin lama masa kerja maka angka kejadian CTS semakin besar.Kesimpulan: Semakin lama masa kerja semakin tinggi resiko terjadinya CTS Kata kunci: Masa kerja, pekerja pemetik daun teh, CTS.
