This paper presents interdisciplinary consensus on the use of protocols of high-intensity magnetic stimulation for the treatment of pelvic floor muscles dysfunction with anal incontinence in Russia.AIM: to highlight the discussion and the decision-making on the basis of an interdisciplinary consensus on the use of a new algorithm of peripheral and transcranial magnetic stimulation in the treatment of pelvic floor muscles dysfunction with the clinic of anal incontinence.RESULTS: the adoption of this consensus can serve as a basis for further research of this problem and optimize the results of treatment of patients with pelvic floor muscle dysfunction with anal incontinence. The data may be interesting for a wide range of medical specialists: general practitioners, gastroenterologists, coloproctologists, surgeons, neurosurgeons, gynecologists, urologists — anyone who encounter with manifestations of this disorder in a routine practice and chooses diagnostic and treatment options.CONCLUSION: protocols for the treatment of anal incontinence using high-intensity magnetic stimulation aimed at improving the quality of treatment of patients with anal incontinence are based on the Russian experience of using the methods discussed and the analysis of the results obtained are validated.
Gradual recovery from a prolonged coma through vegetative state and different types of mutism are detected in one-third of patients with a severe brain injury. Event-related potentials (ERPs) could be used as correlates of mental functions. Aims: We focused on the correlation between ERP within first three months after severe traumatic brain injury (sTBI) and the degree of mental recovery one year, and several years later (up to 15 years after sTBI).
Тяжелая черепно-мозговая травма может вести к длительному нарушению сознания с различными клиническими проявлениями, которые трудны для дифференцировки и прогнозирования развития. В работе на основе записей биопотенциалов были реконструированы и проанализированы функциональные связи мозга, активирующиеся при прослушивании тонов у 20 пациентов с нарушением сознания вследствие травмы мозга. Показано, что у пациентов с затяжным нарушением сознания возможно увеличение числа функциональных связей в лобный отделах, тогда как число «вертикальных» связей снижено. У пациентов с хроническим нарушением сознания значительно снижено число функциональных связей, причем даже в тех случаях, когда отмечается анатомическая сохранность трактов мозга. Можно полагать, что выявленные особенности функциональных связей будут способствовать более точной оценке состояния пациентов, с которыми невозможен вербальный контакт, а также прогнозу возможности восстановления сознания и подбору наиболее адекватной терапии.
