Introduction The increasing prevalence of chronic kidney disease has made it a public health issue. Research on access to hemodialysis services is fundamental for appropriate and assertive approaches to the disease. This study analyzed the factors that influence access to hemodialysis services, from the dimensions of availability, accessibility, and acceptability. Methods This was a cross-sectional census epidemiological study involving 1024 individuals in the Metropolitan Region of Brazil in 2019. Data were analyzed using multinomial logistic regression. Results Factors that increase the chance of belonging to the lowest level of access were: age group from 30 to 59 years (OR 2.16, IC95% 1.377–3.383), female (OR 1.74, IC95% 1.11–2.72), and lower income or equal to two minimum wages (OR 1.80, IC95% 1.17–2.76); the factors medium coverage of the family health strategy or the gateway to public health policy in Brazil (OR 0.54, 95%CI 0.29–0.99), no previous conservative treatment (OR 0.59, 95%CI 0.38–0.91), lack of paid work (OR 0.35, 95%CI 0.15–0.85), retirement/sick leave (OR 0.27, 95%CI 0.12–0.64), and self-assessment of health status as bad or very bad (OR 0.62, 95%CI 0.40–0.96) reduced the chance of belonging to the lowest access level. Conclusion Access to hemodialysis services in a metropolis in the southeastern region of Brazil is influenced by contextual, predisposing, enabling, and health needs characteristics. Those who are female, aged between 30 and 59 years, having an income less than or equal to 2 times minimum wage in Brazil, are at the lowest levels of access, which reinforces the role social determinants in health.
ARTIGOS ORIGINAIS / ORIGINAL ARTICLES Avaliação do nível de conhecimento de portadores de diabetes mellitus sobre adoçantes Assessment of the level of knowledge of diabetes mellitus patients about sweeteners Resumo A terapia nutricional é essencial para o controle metabólico em diabéticos. O uso de adoçantes em substituição ao açúcar é comum, pois estes atribuem doçura aos alimentos sem aumentar a glicemia. O objetivo deste trabalho foi avaliar o nível de conhecimento de indivíduos portadores de diabetes mellitus atendidos no Centro de Referência Ambulatorial de Carapina, Serra/ES, quanto ao consumo de adoçantes. Os dados de 100 voluntários diabéticos foram coletados entre agosto e outubro de 2010, através de questionário sobre o consumo desse produto. Verificou-se que o uso de adoçante é frequente na população estudada, principalmente do tipo líquido, em comparação com outros produtos dietéticos, e também foi possível identificar que 91,2% dos entrevistados passaram a utilizar adoçante somente após o diagnóstico da doença. No que concerne à escolha do adoçante, observou-se que é principalmente influenciada pelo sabor (52,75%) e que os mesmos controlam a quantidade utilizada. Dentre os outros produtos dietéticos citados, o refrigerante foi o mais consumido. Também se constatou que 59% não sabem diferenciar diet de light, e que 63% não têm o hábito de ler o rótulo dos alimentos. Conclui-se que é necessário ofertar maiores esclarecimentos aos diabéticos quanto ao uso adequado dos adoçantes e produtos dietéticos por parte dos profissionais de saúde, com inclusão de informações nutricionais mais completas e compreensíveis nos rótulos desses produtos.
Introduction: The individuals with chronic kidney disease show low adherence to a diet rich in vegetables. Objective: To evaluate the association of minimally processed and ultra-processed food consumption with socioeconomic factors, lifestyle habits, and clinical characteristics of hemodialysis service users in southeastern Brazilian. Methods: Cross-sectional study with 1,024 individuals on hemodialysis from southeastern Brazil. The individuals answered a questionnaire of sociodemographic data, lifestyle habits, and food consumption. After stipulating the frequency of consumption, we classified the foods as minimally processed and ultra-processed. We investigated the association between independent variables and the consumption of minimally processed and ultra-processed foods through the binary logistic regression model with Odds Ratio (OR) and their confidence intervals (95%CI). Results: Users with less than eight years of education (OR=1.706; 95%CI1.125–2.589) and with income less than two minimum wages (OR=1.349; 95%CI1.007–1.806) had lower consumption of minimally processed foods. However, individuals aged 19 to 29 years (OR=2,857, 95%CI1.464–5.576), smokers (OR=2.349; 95%CI1.237–4.462), drinkers (OR=1.835; 95%CI1.122–3.001), and with more than 6 years on hemodialysis (OR=1.975; 95%CI1.227–3.180) were more likely to have higher consumption of ultra-processed foods. Individuals that did not practice physical activity were less likely to this consumption (OR=0.638; 95%CI0.459–0.888). Conclusion: Being younger, smoking, consuming alcohol, and having been on hemodialysis for more than 6 years increased the chances of greater consumption of ultra-processed foods. In addition, we associated less education and lower income with a lower consumption of minimally processed foods.
Introdução: O ganho de peso interdialítico (GPID) é um importante marcador de adesão às restrições dietéticas nos indivíduos em hemodiálise (HD). Objetivo: Avaliar a associação entre indicadores nutricionais e o GPID. Método: Trata-se de um estudo transversal desenvolvido com 1024 pacientes em HD. Resultados: O GPID foi elevado para 30,5% (n=311) dos indivíduos, sendo que aqueles com excesso de peso e perímetro da cintura elevado, apresentaram, respectivamente 46% (OR 0,540, IC95% 0,394-0,741) e 44% (OR 0,540, IC95% 0,394-0,741) menos chances de GPID inadequado. Entretanto, os que apresentaram depleção muscular segundo a circunferência muscular do braço (OR 1,421, IC95% 1,009-2,002) e pela área muscular do braço corrigida (OR 1,435, IC95% 1,052-1,958) tiveram aproximadamente 1,4 vezes mais chances de elevado ganho de peso entre as sessões de diálise. Conclusão: Indivíduos com depleção muscular apresentam maiores chances de terem GPID elevado, ao passo que apresentar maior massa corporal diminuiu as chances deste ganho de peso inadequado.
O Curso de Graduação em Nutrição deve ser centrado no aluno como sujeito da aprendizagem e apoiado no professor como facilitador e mediador do processo ensino aprendizagem. Dessa forma, é importante repensar a maneira como as atividades de ensino têm sido desenvolvidas para que os acadêmicos se preparem para o mercado de trabalho conforme as demandadas atuais. Este trabalho propõe descrever estratégias ativas de ensino, como a aprendizagem baseada em problemas e projetos que podem ser aplicáveis aos acadêmicos do curso de Nutrição, visando a tornar, estudante e professor, agentes comuns e colaborativos na busca e na construção do conhecimento a partir de experiências reais, mesmo diante do distanciamento social orientado pela Organização Mundial da Saúde devido a pandemia relacionada a COVID-19.
