Introducción: Las enfermedades periodontales (EPs) son comunes en niños y adolescentes, y algunas son rápidamente destructivas. El consenso internacional de Periodontología de 1999, vigente hasta hoy día, incluyó a la gingivitis como parte de las EPs. Este estudio se enfocó en la gingivitis inducida por placa dentobacteriana (PDB). Estudios previos reportan porcentajes similares de prevalencia en niños Nigerianos, Polacos y Tanzaneses de alrededor del 54.4%. Sin embargo Oh TJ y col. encontró una presencia del 70% en niños Estadounidenses. En México, hay una amplia discrepacia en su prevalencia que oscila entre el 20.6% -91.3%. Resultados: Los niños presentaron una prevalencia de 64.2%, con una mediana grado I de gingivitis, sin embargo, se observó una tendencia a incrementarse el grado de ésta, conforme el niño tenía más edad, así como un incremento en la presencia de PDB de acuerdo con el índice de O'Leary. No se observaron diferencias en el grado de gingivitis entre sexos. Conclusión:El índice gingival en los niños estudiados es grado I, que es considerado leve y éste es independiente del sexo. Introducción: La anestesia local es el uno de los procedimientos más utilizados en la práctica quirúrgica. En la extracción dental sirve para producir efectos analgésicos y anestésicos. Sin embargo, hay controversia acerca de la seguridad al aplicar un anestésico local combinado con un vasoconstrictor (epinefrina) en pacientes hipertensos controlados. El objetivo es determinar cambios de presión arterial media, frecuencia cardiaca y oxigenación sanguínea en pacientes normotensos e hipertensos controlados después de la administración de anestesia troncular utilizando articaína con epinefrina al 1:200 000 para realizar cirugía oral mandibular.Método: Ensayo clínico controlado en 30 sujetos que asistieron a la Universidad De La Salle Bajío AC para cirugía oral mandibular. Se formaron 2 grupos de 15 sujetos cada uno de acuerdo a los criterios de ASA. A ambos grupos se les registró la presión arterial media, la frecuencia cardiaca y la oxigenación sanguínea basalmente, a los 5 y 10 minutos después de aplicar el anestésico, y al terminar el procedimiento quirúrgico. Para evaluar la efectividad del analgésico se mostró la escala de EVA. Para evaluar los cambios se utilizó la prueba t y ANOVA de dos vías.Resultados: La muestra estuvo conformada por 14 mujeres y 16 hombres con edades 18-89 años, con una media ± desviación estándar de 48.53 ± 19.37 años. Al dividir por grupos, los hipertensos mostraron una diferencia significativa en la edad (t=-2.75; p=0.01). No hubo diferencias estadísticamente significativas entre los grupos para la frecuencia cardiaca, presión arterial media y oxigenación sanguínea en ninguno de los tiempos quirúrgicos después de la administración del anestésico local. En la escala de Eva, tampoco se observó diferencia entre los grupos (p=0.246).Conclusión: No se observaron cambios hemodinámicos inducidos por la inyección local troncular de articaina con epinefrina al 1:200 000 en pacientes con hipertensión controlada durante los procedimientos quirúrgicos. Sin embargo, consideramos fundamental que el Odontólogo realice las precauciones necesarias en el paciente hipertenso controlado y promueva estrategias que disminuyan las complicaciones cardiovasculares.
El sistema inmune del sujeto diabético sufre alteraciones debido a la enfermedad, repercutiendo en los tejidos periodontales. El objetivo fue evaluar los marcadores de inflamación de interleucina (IL)-2, IL-6 e IL-8 en fluido crevicular gingival de pacientes con diabetes mellitus tipo 2 (DMT2). Los participantes se agruparon en cuatro grupos: 1) sanos (n = 20), 2) con periodontitis (PCM, n = 20), 3) con diabetes ≤1 año y periodontitis (DMT2≤1+PCM, n = 20) y 4) con diabetes de ≥10 años con periodontitis (DMT2≥10+PCM, n = 20). Los marcadores se cuantificaron por citometría de flujo. Como resultado, el grupo de DMT2≥10+PCM mostró niveles mayores de IL-2 con relación a los demás grupos, al igual que niveles elevados de IL-6 con respecto del PCM. Los niveles de IL-8 resultaron mayores en el grupo de sanos frente a los demás grupos. Se concluye que los sujetos con DMT2≥10+PCM mostraron una respuesta inmune adaptativa-destructiva del tejido periodontal en comparación con DMT2≤1+PCM que muestran una alteración inmunológica.
