INTRODUCTION AND OBJECTIVES:In a significant number of cases of trans urethral resection of bladder tumor(TURBT)spontaneous initiation of obturator jerk results an incomplete resection and some times results a complications like bleeding or perforation of bladder wall. This multi center prospective cohort was conducted to confirm the efficacy of endoscopic injection of 2% lignocaine into the bladder wall at the base of tumor to eliminate the obturator jerk during the bladder tumor resection to achieve a complete resection in bladder cancer cases for perfect staging and grading.
Voiding dysfunction and lower urinary tract symptoms (LUTS) in children due to congenital anomalies of urogenital system is common, but congenital anterior urethral diverticulum (CAUD) in male is very rare. Sometimes the problem is associated with anterior urethral valve. CAUD with secondary stone is rare. Here we report a case of a 12-year-old male child with obstructive symptoms. Objective: To compare the outcome of optical internal urethrotomy (OIU) with or without intralesional triamcinolone acetonide injection for the treatment of short segment anterior urethral stricture.
Methods: This prospective quasi experimental study was carried out in the department of Urology, DMCH, Dhaka from November 2015 to April 2017 on 50 patients with short segment of anterior urethral stricture. Cases were randomly allocated to group A (OIU without Triamcinolone) and group B (OIU with Triamcinolone). Each group consisted of 25 patients. Data were analyzed and compared by statistical tests.
Results: There were no significant differences in the baseline characteristics of the patients. Recurrences of stricture urethra were higher among those without Triamcinolone than cases with Triamcinolone which was statistically significant (p=0.033) at 6 month and (p=0.016) at 9 months. Regarding mean time interval in month of development of recurrence of stricture was 6.1±1.66 months in Group A and 7.66±1.52 months in Group B which was statistically significant (p=0.001)
Conclusion: OIU with intralesional triamcinolone is better than OIU alone. It significantly reduced and delayed the recurrence of anterior urethral stricture.

Bladder cancer is the second most common malignancy of genitourinary tract. About 20% cases are muscle invasive and most of the non-muscle invasive bladder cancer progressed to muscle invasion with time. The standard care for organ confined muscle invasive bladder cancer is radical cystectomy followed by urinary diversion. Orthotopic bladder substitution is considered standard urinary diversion after radical cystectomy in patients who have no urethral malignancy and intact external urethral sphincter. Neobladder constructed from ileum as described by Studer is considered standard and followed by most centers worldwide since 1985. We modified the procedure to improve the outcome and minimize post-operative complications by reducing the length of ileum used for construction of neobladder in 37 patients. Voiding, storage function and complications were evaluated during follow-up period up to 24 months after surgery. Three patients died during follow-up period of which 2 from metastatic disease and one from unrelated cause. Daytime and night-time continence rates were 86.49% and 78.38% respectively. Average functional capacity (437ml), maximum flow rate(15.7ml/s) and mean post void residual urine (44ml) during the period of evaluation. Clean intermittent self-catheterization needed in 67.57% patients due to incomplete emptying during early postoperative period but after 3 months most of the patients learnt to void without any aid. Upper tract dilatation found in one patient due to stricture at urethrovesical junction and required endoscopic incision and dilatation later. 12 patients had an urodynamic study at 12 months after surgery and found low pressure reservoir with small residual urine in all cases. Modified ileal neobladder is a safe and reliable procedure with satisfactory short-term result.

The Horseshoe kidney is not uncommon entity with an incidence of 0.25% of general population with increased association of genitourinary anomalies. Horseshoe kidney with single ureter is extremely rare. We report a case of a patient with a Horseshoe kidney with a single ureter with stone that was diagnosed incidentally.
