Abstract. Irritable bowel syndrome (IBS) is a common health issue that is characterized by abdominal pain, abnormal bowel movements, altered visceral perception, and abnormal metabolism of 5-hydroxy triptamine (serotonin; 5HT). The use of prebiotics or probiotics treatment for IBS has become increasingly important as an adjunct to pharmaceutical options. The aim of this study was to determine the efficacy of enzymetreated rice fiber (ERF) on an IBS model. We obtained a new prebiotic from defatted rice bran that was developed as an insoluble dietary fiber through amylase and hemicellulase treatment followed by removal of the soluble fraction. Containing ~70% hemicellulose, ERF is utilized by lactobacilli and subsequently converted to butyrate using Eubacterium limosum. We employed a restraint stress IBS model which involved the continuous application of stress for 4 h per day for 3 days. Polycarbophil Ca (PC) (500 mg/kg body weight) was used as a positive control and ERF was added to the diet at 4% in diet. During restraint stress, ERF significantly attenuated urgent fecal excretion, colonic mucosal 5HT secretion, and hyperalgesthesia compared with the control. ERF also significantly increased cecal butyrate production as well as total organic acid content. PC was only effective in regard to preventing increases in 5HT levels. Furthermore, there were no significant levels of proinflammatory markers CINC-1 and TNF-· among the groups. Although more detailed studies are needed, the ERF prebiotic demonstrated potency in attenuating major symptoms of IBS.
BackgroundLymphatic malformations (LMs) are congenital malformations of the lymphatic system that commonly affect the head and neck region and cause marked cosmetic and functional complications. In this pilot study, we present eight children with LMs treated using an herbal medicine for this indication.MethodsBetween January 2009 and May 2014, eight children (four boys, four girls) with LMs were treated using oral administration of an herbal medicine, Eppikajyutsuto (TJ‐28; Tsumura, Tokyo, Japan), as monotherapy.ResultsFour of the cases were macrocystic and four were mixed micro‐ and macrocystic. The mean treatment duration was 7.2 ± 2.9 months (range 5–12 mos). The mean LM volume shrinkage on magnetic resonance imaging was 54.5 ± 38.3% (macrocystic 73.6 ± 27.0%; mixed micro‐ and macrocystic 35.4 ± 41.5%). One of four macrocystic lesions had a marked reduction, two had a moderate reduction, and one had no response. A marked reduction was observed in three of the four mixed micro‐ and macrocystic cases; the other mixed cystic case had no response. The treatment was well tolerated, without severe adverse events.ConclusionsThis preliminary study demonstrates the beneficial effects of TJ‐28. Further evaluations of this therapeutic modality are warranted.
Esophageal atresia (EA) is a life-threatening disorder associated with operative complications. Postoperative gastric electrical control activity detected by a non-invasive electrogastrography (EGG) technique was investigated in 13 children aged 1-17 years to clarify whether gastric motility disorders were present. EGG abnormalities were present in 5 patients; persistent dysrhythmias were found in 3. Roentgenographic examinations showed mild gastroesophageal reflux in 3 (60%) of the dysrhythmic patients; 2 others had postprandial dysrhythmias. The mean spectral frequency (MSF) of EA cases with dysrhythmia was significantly higher than that of patients without dysrhythmia in both fasting and postprandial states (P < 0.05). The variability of the peak spectral frequencies (PSFV) in patients with dysrhythmia was significantly higher than in those without dysrhythmia in both fasting and postprandial states (P < 0.05). There were no significant differences in MSF and PSFV between EA patients without dysrhythmia and controls. These results suggest that gastric motor activity may be disordered in patients following operative repair of EA, although they remain asymptomatic. EGG may be a useful screening examination for postoperative gastric functional disorders.
