Mục tiêu: Xác định giá trị của mức độ tăng βhCG 48 giờ và siêu âm đường âm đạo ở bệnh nhân theo dõi chửa ngoài tử cung.
Đối tượng và phương pháp: mô tả, tiến cứu 160 bệnh nhân được nhập viện theo dõi chửa ngoài tử cung tại Khoa Phụ 1- Bệnh viện Phụ Sản Trung ương từ 9/2017 đến 01/2018, xử lý số liệu bằng chương trình SPSS 16.0. Tính giá trị trung bình, so sánh hai giá trị trung bình, độ nhậy, độ đặc hiệu và xây dựng đường cong ROC.
Kết quả: 160 bệnh nhân theo dõi chửa ngoài tử cung có 112 bệnh nhân chửa ngoài tử cung (CNTC), 21 bệnh nhân chửa trong tử cung (CTTC), 27 bệnh nhân sẩy thai tự nhiên/thai tự tiêu. Nồng độ trung bình của βhCG nhóm CNTC sau 48 giờ (778,43, 95%CI:601,85-955,02) thấp hơn so với nhóm CTTC (3119,04; 95%CI:1221,36-5016,72) có ý nghĩa thống kê với p <0.0001. Theo dõi sau 48h: tỉ lệ βhCG tăng dưới 53%, chẩn đoán CNTC với độ nhạy 83,0%, độ đặc hiệu 39,5%, giá trị tiên đoán dương 76,2%, giá trị tiên đoán âm 50,0%. Siêu âm có khối hỗn hợp âm cạnh tử cung (TC) tiên lượng chẩn đoán CNTC với độ nhạy 84,8%, độ đặc hiệu 64,6%, diện tích dưới đường cong 74,7%.
Kết luận: Mức độ tăng của βhCG trên 53% tiên lượng CTTC; dưới 53% tiên lượng CNTC hoặc sẩy thai sớm. Siêu âm sau 48 giờ có khối hỗn hợp âm cạnh tử cung, tiên lượng CNTC.
