Current surgical treatment of idiopathic scoliosis involves the use of various segmental instrumentation. Various pedicle screws have allowed for improved correction. Although monoaxial screws have improved rotational control compared with polyaxial screws, their use may increase screw-bone interface or vertebral endplate forces if not inserted in an exactly straight trajectory. Uniaxial screws potentially decrease these forces while retaining the advantages of monoaxial screws with respect to better rotational control. The purpose of this study was to compare the vertebral endplate forces with monoaxial or uniaxial screws when being reduced to a rod. Thirty-two plastic, surrogate T11 vertebrae were prepared with monoaxial (n=16) or uniaxial (n=16) screws. Screw angles relative to inferior vertebral endplates were assessed with lateral radiographs. The vertebrae were fixed to a load cell, and loads were measured as the screw was reduced to a rod. Monoaxial screws demonstrated a linear progression of endplate force with increasing screw angle. Uniaxial screws demonstrated minimal endplate force until approximately 20°, coinciding with screwhead excursion angle. As this maximum excursion angle was passed, uniaxial screws demonstrated a force slope similar to the monoaxial screws.The measured endplate forces should be equivalent to forces at the screw-bone interface. The reduced force with uniaxial screws is expected to have less cranial-caudal plow potential as the screw is coupled to a rod for deformity correction. This could have potential implications for screw failure, especially in less dense bone.
WHIPS reduced peak T1 horizontal acceleration by 39 percent compared to sled acceleration. This was within the range previously reported for WHIPS, between 30 and 60 percent, but higher than the 16 percent reduction previously reported due to active head restraint. Absorption of crash energy occurred during the initial 75 ms and the onset of head support occurred at 114 ms. Differential head-torso motions occurred prior to and during head support, indicating the potential for neck injury even with WHIPS.
Objective
To investigate the surgical indication and tactics for liver hemangioma in the caudate lobe
Methods
From January 1994 to July 2019, 137 patients, including 51 males and 86 females with the average age of 49.2 years old were diagnosed with liver hemangioma in caudate lobe and received treatment at five tertiary referral hospitals. Clinical features, correlations between tumor size and clinical manifestations, treatments, and prognosis were analyzed.
Results
Of the 137 patients identified, 40 (29.20%) patients were asymptomatic, whereas other 94 patients had clinical symptoms mainly presented as upper abdominal discomfort, epigastric distention, upper abdominal dull pain, nausea, and vomiting. Fifteen (93.75%), 18 (39.13%), and 7 (10.45%) patients presented no clinical symptoms among those tumor size was less than 3 cm (D ≤ 3 cm, n = 16), 3 cm < D ≤ 6 cm (n = 46), and 6 cm < D ≤ 9 cm (n = 67), respectively, while all 8 patients with tumor larger than 9 cm were symptomatic. Tumor diameter was obviously associated with the presence of clinical symptoms. In follow-up period, 7 patients in the conservative group (n = 39) received surgery because of tumor growth or symptom appearance. Totally 105 patients received operation including partial resection or isolated complete resection of caudate lobe and caudate lobe resection combined with liver segment resection, right liver resection, or left liver resection. All operations went smoothly, and no severe complications appeared.
Conclusion
Tumor diameter was obviously associated with the presence of clinical symptoms in patients with hemangioma in caudate lobe. Surgical therapy is not recommended for asymptomatic patients and available for patient who has symptoms. Effective surgical strategies should be put into use to reduce operative bleeding.
