Atualmente, o uso exclusivo do indicador de resultado em pneumonia associada à ventilação mecânica (PAV) pode ser considerado controverso, por não expressar a real qualidade e segurança dos serviços prestados em saúde. Assim, pretende-se fazer a análise do indicador de resultado concomitante à análise do indicador de processo de um protocolo de prevenção da PAV (Bundle). Métodos: estudo retrospectivo, descritivo e comparativo. Na análise do indicador de resultado foram comparadas a densidade de incidência de PAV e a taxa de utilização da ventilação mecânica (VM), antes e após a implantação do Bundle, por meio do teste t pareado. Para a análise de processo foi avaliado o percentual de adesão aos itens do Bundle. Finalmente, buscou-se estabelecer uma análise crítica entre ambos os indicadores estudados. Resultados: a adesão aos componentes do Bundle mostrou-se inferior ao esperado: decúbito elevado = 62%; higiene oral = 58%; despertar diário = 34%. Objetivo: Propor a análise gerencial do processo assistencial utilizando a sepse como doença traçadora de qualidade. Resultados: Foram avaliados retrospectivamente os prontuários de 18 pacientes de um único hospital público brasileiro. A adesão aos indicadores de qualidade do processo assistencial da sepse, propostos pela Surviving Sepsis Campaign, foi baixa: coleta de lactato = 22%; coleta de hemoculturas = 33%; administração de antibióticos = 28%; uso de expansores ou vasopressores = 72%; medida de pressão venosa central = zero; medida da saturação venosa central de oxigênio = 6%. Em nenhum caso houve realização conjunta de todos os itens mencionados. Houve permanência prolongada do paciente no pronto socorro antes da internação hospitalar, atraso na percepção e no diagnóstico da sepse e indisponibilidade imediata de leito de terapia intensiva para internação. Conclusão: O atendimento em sepse parece ser sensível à qualidade da prestação de serviços em saúde.
Introduction: Infections related to health care are of great importance in the management of quality and safety, and represent a considerable financial burden for the health system. Aim: To establish a plan of action for prevention and control of urinary tract infection related to indwelling catheters (ITU-CVD) by one adult intensive care unit (ICU-A). Method: It was executed a plan of action, starting in January 2012, which included clinical interventions in multi-level, organized and coordinated, related to the management of people and processes to reduce the ITU-CVD and the catheter utilization rate of bladder delay (Tx-CVD). The internal references were the annual median of the two years immediately preceding the intervention (2010 and 2011). External references were fifty percentiles presented annually by the Surveillance Coordination Health of São Paulo (COVISA). Results: The results for each indicator temporally were compared by paired t-test (p <0.05). The results, expressed by the median annual incidence density of ITU-CVD were as follows: 2010 = 15.3; 2011 = 13.6; 2012 = 6.6. (P <0.05) As for Tx-CVD, the annual median found was: 2010 = 81.3; 2011 = 85.5; 2012 = 61.6. (P <0.05). Conclusion: The obtained results were not studied for the intervention time sufficient to reach the external reference. The proposed interventions can bring positive impact in reducing the Tx-CVD and the incidence density of ITU-CVD ICU-A.
