Background: 206) is a fully human monoclonal antibody binding to cytotoxic T-lymphocyteassociated antigen 4 (CTLA4) on T cells that stimulates the immune system by blocking the CTLA4-negative regulatory signal. Combination with standard chemotherapy may strengthen antitumor therapy. This is a phase Ib, multisite, openlabel, nonrandomized dose escalation trial evaluating the safety, tolerability, and maximum tolerated dose (MTD) of tremelimumab combined with gemcitabine in patients with metastatic pancreatic cancer.Patients and methods: Gemcitabine (1000 mg/m 2 on days 1, 8, and 15 of each 28-day cycles) was administrated with escalating doses of i.v. tremelimumab (6, 10, or 15 mg/kg) on day 1 of each 84-day cycle for a maximum of 4 cycles. The first 18 patients had an initial 4-week gemcitabine-only lead-in period. Dose-limiting toxicities (DLTs) related to tremelimumab were evaluated during the first 6 weeks after the first dose of tremelimumab. No DLTs related to tremelimumab were observed at any dose, even when the maximum dose established for tremelimumab (15 mg/kg) was used. Most frequent grade 3/4 toxicities were asthenia (11.8%) and nausea (8.8%). Only one patient had a serious drug-related event (diarrhea with dehydration). The median overall survival was 7.4 months (95% confidence interval 5.8-9.4 months). At the end of treatment, two patients achieved partial response. Both patients received tremelimumab 15-mg/kg group (n = 2/19, 10.5%).
Conclusion:Tremelimumab plus gemcitabine demonstrated a safety and tolerability profile, warranting further study in patients with metastatic pancreatic cancer.ClinicalTrials.gov ID: NCT00556023.
A unique, symmetrical onychodystrophy is described in 18 dogs. A rather sudden onset of onychomadesis is followed by chronic onychodystrophy affecting all claws. Pain and lameness are recognized in half of the patients, but the dogs are healthy otherwise. Histopathologically, this disorder is characterized by hydropic and lichenoid interface dermatitis. Nine dogs were treated with a commercial, fatty-acid supplement and had good-to-excellent responses. Due to the clinicopathological characteristics of this disorder, the authors propose the name "symmetrical lupoid onychodystrophy."
Indoor air contamination by radon and its decay products is currently the focus of considerable attention and is considered by many to be the greatest potential cause of lung cancer in the human environment next to smoking. The bifurcations of the human respiratory tract are regions in which enhanced local deposition of particles (hot spots) can occur. These hot spots are important in estimating the risk from radon exposure but existing mathematical models do not characterize them accurately. In this study, radon progeny in the molecular size range were sampled through an aluminium model of a lung bifurcation. The parent and secondary tube diameters used correspond to the third and fourth generations in Weibel's lung model. Steady state, nominally laminar flows were used in the study. Deposition was measured along the inside, outside, top, and bottom walls of the secondary tubes. Experimental results indicate that the deposition along the inside wall is noticeably higher than that along the other walls. The results also show that along the inside, top, and bottom walls the deposition has its overall maximum at the carina. Other maxima are also observed along the secondary tubes downstream from the carina.
To improve the methodology for assessing neutron dose at Union Electric's Callaway Nuclear Power Plant, an intercomparison of neutron detectors and dosimeters was performed. Seven different neutron detectors and dosimeters were tested in four different neutron fields utilizing facilities at the Missouri University Research Reactor and at the Southwest Radiation Calibration Center at the University of Arkansas. In general, all results agree within a factor of 2 in predicting the neutron dose equivalent. It was concluded that measurements of dose in containment should utilize the Tissue-Equivalent Proportional Counter (TEPC), the Bonner-sphere system, and the proton recoil spectrometer to accurately assess the neutron dose. These data can then be used to provide correction factors for more traditionally used dosimeters in containment, such as thermoluminescent dosimeters and survey meters.
