One hundred and eighty-one patients with single aneurysms involving the anterior circulation were treated either by wrapping (60 cases) or clipping (121 cases), and 96.1% of them were followed for 10 years. The rate of early rebleeding (less than 6 months) from wrapped aneurysms was 8.6% (confidence interval 1.4% to 15.8%) and the late rebleeding (6 months to 10 years) rate was 1.5% per annum (confidence interval 0.3% to 2.5%/yr). Wrapping an aneurysm offers some protection from rebleeding, particularly during the first 6 months when the risk of rebleeding is high.
✓ Of 1000 patients with nonmissile depressed skull fractures, 10% had early epilepsy (first week) and 15% developed late epilepsy. The risk of late epilepsy varied widely but could be predicted from clinical data 1 week after injury. In more than one-third of the patients with depressed fractures the risk of late epilepsy is less than 4%, but in some patients it is over 60%. The risk is increased if posttraumatic amnesia exceeds 24 hours, if the dura is torn, if there are focal signs, or if there has been early epilepsy.
varicella vaccine may be highly indicated for children who are known to have genital warts. Alternatively, in theory, the varicella vaccine itself could have an adverse effect on the clinical course of HPV. The use of varicella vaccine in children with HPV infection deserves more study.There are many questions about HPV infections that cause genital condyloma acuminata in children. The epidemiology and natural history is perplexing in adults and children. More clinical studies and basic research are needed to provide a more comprehensive knowledge of the problem.
